A Rare Case of Erosive Esophagitis Due to Sarcina Ventriculi Infection.

Sarcina ventriculi is a Gram-positive anaerobic coccus found in soil that is a rare cause of inflammatory infections of the GI tract. This bacterium has a propensity for causing gastritis in patients with delayed gastric emptying. Of the 66 reported cases in the literature, 10 involved the esophagus. Symptoms of an esophageal infection are non-specific and may be mistaken for long-standing gastroesophageal reflux. We present a case of a 67-year-old female with chronic dysphagia and reflux diagnosed with erosive esophagitis caused by Sarcina ventriculi. Treatment strategies documented in the li

Gastric Perforation Due to Sarcina ventriculi: A Histologically Confirmed Case.

We present a case of a rare cause of gastric perforation and pneumoperitoneum, associated with Sarcina ventriculi. An 88-year-old male presented to the emergency room with significant abdominal pain as his chief complaint. Abdominal radiograph showed extensive free intraperitoneal gas under the diaphragms. Computed tomography (CT) of the abdomen and pelvis showed pneumatosis, portal venous gas, and extensive free intraperitoneal gas with free fluid. Immediate surgical intervention ensued. The gastric biopsies obtained proved valuable for confirming the diagnosis of S. ventriculi. In conjunctio

Concomitant Sarcina-Associated Erosive Esophagitis and Refractory Helicobacter pylori Gastritis.

For unclear reasons, there has been an increasing number of reported cases of Sarcina infections in the gastrointestinal tract over the past several years. Associated clinical conditions with the infection most commonly include delayed gastric emptying from diabetes mellitus, a history of previous gastrointestinal surgery, and ulcer disease. The precise pathogenetic role of Sarcina infection in humans remains unclear. Because of the ubiquitous environmental presence of Sarcina and limited previously reported clinical cases, the link between symptoms along with endoscopic findings to Sarcina ca

Sarcina ventriculi in blood: the first documented report since 1872.

BACKGROUND: In 1872, in British Medical Journal (BMJ) Dr. David Ferrier published that Sarcina ventriculi (Goodsir) constantly occurred in the blood of man and the lower animals. His observation was based on bleeding experiments, incubation of blood at 100°F (37.8°C) and later examination. He found "immense numbers of beautifully formed sarcinæ". In the next issue of BMJ Dr. Charlton Bastian expressed concerns that Sarcina might indeed be "really a living thing" or "might be partly organic and partly mineral in its constitutions". CASE PRESENTATION: Anaerobic gram-positive giant coccae assemb

The Spectrum of Sarcina Colonization in the Gastrointestinal Tract of Pediatric and Adolescent Patients.

INTRODUCTION: Sarcina organisms are rare, gram-positive, sugar-fermenting cocci, identifiable in tissues only by histologic examination or molecular testing. Since its discovery, the pathogenicity and relevance of Sarcina in the human gastrointestinal tract has remained ill-defined. A recent literature review of 66 reported examples demonstrated the potential for severe complications such as emphysematous gastritis and gastric perforation. In pediatrics, colonization is associated with mucosal alterations and/or gastrointestinal dysmotility/obstruction with variable outcomes, including death s

Sarcina ventriculi in gastric biopsies of two patients with an underlying neoplasia.

We present the cases of two women aged 75 and 71 years under study for dyspepsia and weight loss. The initial gastroscopy of both patients showed a stomach full of food content, with thickened gastric folds that were hard on contact with the forceps. Gastric biopsies showed the presence of Sarcina ventriculi on active chronic gastritis, so antibiotic treatment was started (ciprofloxacin and metronidazole), without significant clinical improvement. In both patients, gastroscopy was repeated without showing macroscopic changes compared to the initial gastroscopy, which led to the suspicion of an

Ulcerative Gastritis and Esophagitis in Two Children with Sarcina ventriculi Infection.

Sarcina ventriculi is a Gram-positive, obligate anaerobic coccus, with a characteristic morphology. Only 22 cases of human infections by this microorganism, including 7 in children, have been reported in literature so far. Affected subjects usually present with abdominal pain, nausea, vomiting, and delayed gastric emptying. However, life-threatening complications, like emphysematous gastritis and gastric perforation have also been described. Gastroparesis and gastric outlet obstruction have been considered as a potential etiologic factor. All pediatric cases described thus far presented with c

Sarcina Ventriculi infection: a rare but fearsome event. A Systematic Review of the Literature.

OBJECTIVES: This study is aimed to report a case of SV-related gastritis and the results of a systematic literature review of SV infections. METHODS: Following a case presentation, we systematically searched different databases (MEDLINE, PubMed, Scopus, Web of Science, EMBASE, google scholar) for the items "sarcina," "ventriculi," "clostridium" with AND/OR. RESULTS: A total of 55 articles reporting 65 cases of Sarcina Ventriculi were found. Thus, 66 patients, including our case, were reviewed. The median age was 51 years (IQR: 0-87 years). Females accounted for 51% of cases. 68% of patients

Spontaneous Gastric Perforation in a Healthy Child Associated With Sarcina ventriculi Infection.

Spontaneous gastric perforation is a rare but morbid condition in children. Recent reports have suggested that finding Sarcina ventriculi, a rare, Gram-positive anaerobic bacterium in the stomach, could be associated with gastric ulcers and rarely gastric perforation, predominantly in adults. Here, we describe a case of an 11-year-old otherwise healthy child who presented with acute onset of severe, diffuse abdominal pain, tenderness, and distension and was found to have peritonitis and shock. The emergent exploratory laparotomy revealed a 4-cm gastric perforation, and histopathologic examinat

Sarcina ventriculi: An incidental finding or a byproduct of a more concerning underlying disease?

Sarcina ventriculi is a rare, acid-resistant, carbohydrate-fermenting, Gram-positive bacterium most often reported in the setting of delayed gastric emptying or gastric stasis and obstruction. Its clinical relevance is variable, ranging from incidental colonisation to potentially severe complications such as emphysematous gastritis and perforation. Diagnosis is typically suggested by the characteristic basophilic tetrads on routine hematoxylin-eosin staining. An 81-year-old male presented with a 3-month history of atypical epigastric pain, postprandial vomiting, and general deterioration. Init

Unusual Localization of Presumptive Sarcina ventriculi in the Terminal Ileum: A Case Report.

BACKGROUND: Sarcina ventriculi is a bacterium predominantly reported in the stomach and associated with emphysematous gastritis, delayed gastric emptying, gastroparesis, or gastric outlet obstruction. Its prevalence is increasing among patients with a history of organ transplants, immunosuppression, and graft-versus-host disease (GVHD). This bacterium can be detected on histology with characteristic tetrad packet morphology; however, confirmation requires PCR and molecular studies. The role of Sarcina ventriculi in human diseases is not fully understood and has unclear clinical significance. W

A Sarcina bacterium linked to lethal disease in sanctuary chimpanzees in Sierra Leone.

Human and animal infections with bacteria of the genus Sarcina (family Clostridiaceae) are associated with gastric dilation and emphysematous gastritis. However, the potential roles of sarcinae as commensals or pathogens remain unclear. Here, we investigate a lethal disease of unknown etiology that affects sanctuary chimpanzees (Pan troglodytes verus) in Sierra Leone. The disease, which we have named "epizootic neurologic and gastroenteric syndrome" (ENGS), is characterized by neurologic and gastrointestinal signs and results in death of the animals, even after medical treatment. Unusual Presentation: Chronic Gastritis Caused by Sarcina ventriculi Identified via Helicobacter pylori Immunohistochemistry.

This case report presents a rare occurrence of Sarcina ventriculi in a 15-year-old male patient with a clinical history of renal failure due to chronic rejection status post kidney transplantation, with persistent symptoms of diarrhea, nausea, vomiting, and fatigue. Despite exhibiting normal gastrointestinal mucosa upon endoscopy, biopsy analysis revealed chronic gastritis accompanied by the presence of Sarcina ventriculi in a tetrad arrangement across the stomach, duodenum, and distal esophagus. Interestingly, immunohistochemistry (IHC) staining targeting the Helicobacter pylori organism (H.

Non-operative Management of Sarcina ventriculi-Associated Severe Emphysematous Gastritis: A Case Report.

We report a case of a 64-year-old Jehovah's Witness male, who was post-operative day five of laparoscopic cholecystectomy. He presented with anemia, severe ischemic gastritis, and pneumatosis seen on CT with intravenous contrast. A subsequent upper endoscopy revealed patchy gastric ulceration with bleeding but no overt evidence of perforation. Biopsies were taken, and immunohistological staining identified Sarcina ventriculi. The patient was treated non-operatively with fluconazole and piperacillin-tazobactam for the infection and with sucralfate tablets and pantoprazole injections for ulcer t

Unveiling the Culprit: Sarcina Infection in the Stomach and Its Link to Unexplained Weight Loss.

Sarcina species (spp.) infections in humans are relatively rare; however, reported cases have recently increased. We have presented the case of a 56-year-old female with diabetes who presented with bloating, dysphagia, and substantial weight loss, ultimately diagnosed with reactive gastritis secondary to Sarcina spp. infection. Administration of antibiotics and a proton pump inhibitor led to symptom alleviation and weight gain. This case underscores the significance of considering Sarcina spp. infection in patients experiencing unexplained weight loss and nonspecific gastrointestinal symptoms,
