EBF1 Limits the Numbers of Cochlear Hair and Supporting Cells and Forms the Scala Tympani and Spiral Limbus during Inner Ear Development.

Early B-cell factor 1 (EBF1) is a basic helix-loop-helix transcription factor essential for the differentiation of various tissues. Our single-cell RNA sequencing data suggest that Ebf1 is expressed in the sensory epithelium of the mouse inner ear. Here, we found that the murine Ebf1 gene and its protein are expressed in the prosensory domain of the inner ear, medial region of the cochlear duct floor, otic mesenchyme, and cochleovestibular ganglion. Ebf1 deletion in mice results in incomplete formation of the spiral limbus and scala tympani, increased number of cells in the organ of Corti and

Vascular trauma during cochlear implantation: a contributor to residual hearing loss?

OBJECTIVE: This study was performed to investigate the vascular structures of the cochlea that are potentially vulnerable to mechanical trauma during cochlear implant surgery. BACKGROUND: Despite improvements in surgical technique and electrode design, residual hearing is lost in a significant percentage of cochlear implant patients. Although a variety of factors may contribute, it is widely believed that mechanical trauma plays an important role. This study focused on the vasculature of scala tympani and its potential susceptibility to injury during implant surgery. METHODS: Anatomic study

Pressure-induced basilar membrane position shifts and the stimulus-evoked potentials in the low-frequency region of the guinea pig cochlea.

We have used the guinea pig isolated temporal bone preparation to investigate changes in the non-linear properties of the tone-evoked cochlear potentials during reversible step displacements of the basilar membrane towards either the scala tympani or the scala vestibuli. The position shifts were produced by changing the hydrostatic pressure in the scala tympani. The pressures involved were calculated from measurements of the fluid flow through the system, and the cochlear DC impedance calculated (1.5 x 10(11) kg m-4 s-1, n = 10). Confocal microscopic visualization of the organ of Corti showed

Cochlear vascular pathology and hydrops in otosclerosis.

Three ears with otosclerosis were found incidentally in a series of human temporal bones examined to evaluate cochlear sensorineural degeneration. Otosclerosis was identified with microdissection, surface preparation technique and transmission electron microscopy. Vascular abnormalities were present in all ears, and otosclerosis involved the cochlear endosteum extensively, mainly in the scala tympani of the basal turn. In the scala tympani of the lower half of the basal turn, shunts had formed so that venules deviated abruptly from their normal radiating course towards the spiral vein, left th

Multichannel cochlear implantation in the scala vestibuli.

OBJECTIVE: Sensorineural hearing loss resulting from otosclerosis, meningitis, chronic otitis media, autoimmune ear disease, and trauma can be associated with partial or total obstruction of the cochlear scalae. Multichannel cochlear implantation may be difficult in a cochlea with an obstructed scala tympani. The purpose of this study is to determine the safety and efficacy of scala tympani electrode insertion. STUDY DESIGN: Retrospective chart review. SETTING: Academic medical center. PATIENTS: Eight children and adults with profound sensorineural hearing loss who underwent cochlear implan

An evaluation of per-scalar cochlear electrode implantation techniques. An histopathological study in cats.

This study was carried out to evaluate the effects of infection and trauma on the organ of Corti and spiral ganglion cells following per-scalar cochlear electrode implantations in cats. The results showed that the incidence of infection with a per-scalar electrode inserted through an opening made directly into the apical and middle turns is low, provided the procedure is carried out aseptically with an antibiotic cover. Trauma could also be kept to a minimum by inserting the electrode into the scala tympani, taking care to insert the electrode without applying force, and ensuring that the firs

Pathology of prelingual profound deafness: magnitude of labyrinthitis fibro-ossificans.

Quantitative histologic studies were performed on 15 temporal bones from eight adult persons who were known to have prelingual bilateral profound hearing loss. The pathologic changes are characterized by severe degeneration of the structures of the cochlear duct, often with degeneration of the vestibular sense organs, causing a reparative host response that features osteoneogenesis and fibrous proliferation followed by retrograde neuronal degeneration. The pathology is consistent with meningogenic bacterial or viral labyrinthitis that occurred subclinically or went undiagnosed. Bone and fibrou

Cochlear dialysis for Menière's disease. An update.

The early results of hyperosmolar perfusion of the scala tympani for advanced Meniere's disease were reported in 1987. Thirty patients were treated in this way between March 1981 and March 1986; follow-up is now 2 to 7 years. The rationale of cochlear dialysis is to reduce the endolymphatic hydrops of the pars inferior by filing the scala tympani with a sterile solution of mannitol in 5% dextrose that has an osmolality of 350 m.osm per liter, which compares with a scala media osmolality of circa 300 m.osm per liter. The technique is a qualitative and quantitative extension of the "salt in the

Perilymph pharmacokinetics of locally-applied gentamicin in the guinea pig.

Intratympanic gentamicin therapy is widely used clinically to suppress the vestibular symptoms of Meniere's disease. Dosing in humans was empirically established and we still know remarkably little about where gentamicin enters the inner ear, where it reaches in the inner ear and what time course it follows after local applications. In this study, gentamicin was applied to the round window niche as a 20 μL bolus of 40 mg/ml solution. Ten 2 μL samples of perilymph were collected sequentially from the lateral semi-circular canal (LSCC) at times from 1 to 4 h after application. Gentamicin concent

Cochlear otosclerosis: does bone formation affect cochlear implant surgery?

This study aimed to demonstrate that new bone formation in the scala tympani of patients deaf from otosclerosis does not preclude cochlear implant surgery. In seven temporal bones from patients with otosclerosis, we measured the extent of new bone from the round window to the distal part of the new growth. We compared results to surgical data on the extent of drilling and depth and ease of placement of the electrode in 20 patients deaf from otosclerosis. We also examined clinical performance and voltage requirements for long-term implant use in patients with and patients without ossification o

Contribution of macrophages to intracochlear tissue remodeling responses following cochlear implantation and neural survival.

INTRODUCTION: Cochlear implants (CIs) restore hearing to deafened patients. The foreign body response (FBR) following cochlear implantation (post-CI) comprises an infiltration of macrophages, other immune and non-immune cells, and fibrosis into the scala tympani; a space that is normally devoid of cells. This FBR is associated with negative effects on CI outcomes including increased electrode impedances and loss of residual acoustic hearing. This study investigates the extent to which macrophage depletion by an orally administered CSF-1R specific kinase (c-FMS) inhibitor, PLX-5622, modulates t

Middle Ear Actuator Performance Determined From Intracochlear Pressure Measurements in a Single Cochlear Scala.

HYPOTHESIS: Intracochlear pressure measurements in one cochlear scala are sufficient as reference to determine the output of an active middle ear implant (AMEI) in terms of "equivalent sound pressure level" (eqSPL). BACKGROUND: The performance of AMEIs is commonly calculated from stapes velocities or intracochlear pressure differences (PDiff). However, there are scenarios where measuring stapes velocities or PDiff may not be feasible, for example when access to the stapes or one of the scalae is impractical. METHODS: We reanalyzed data from a previous study of our group that investigated the

Immunohistochemical analysis of proliferating cells in a sterile labyrinthitis animal model.

Inflammatory reactions within the cochlea lead to the formation of fibrotic tissue and bone. To determine which cells are involved in the proliferation of the inflammatory response within the cochlea, sterile labyrinthitis was created by inoculating keyhole limpet hemocyanin (KLH) into the scala tympani of systemically sensitized animals. Cellular proliferation was assessed immunohistochemically using the monoclonal antibody Ki-67. Proliferating cells were identified among inflammatory cells and fibroblasts within the matrix, as well as in endosteal cells lining the scala tympani. Inflammatory

Hydrops of the organ of Corti.

OBJECTIVES: The authors would like to confirm a fluid pathway from the scala tympani to the organ of Corti, and to observe its morphological changes. METHODS: A staining solution for succinic dehydrogenase was perfused with phenazine methosulfate in the scala tympani of living guinea pigs (n = 5) under deep anesthesia. After fixation, the cochleas were eventually embedded in epon. Sections were observed under a light microscope. RESULTS: Blue-stained tissue is indicative of the pathway taken by the solution. The staining solution entered the organ of Corti through Hensen-Deiters' slit. The s

Effects of bacterial endotoxin applied to the guinea pig cochlea.

The ototoxic potential of bacterial endotoxin present in middle ear effusions was evaluated by inoculating endotoxin solutions of 10 micrograms/mL or 1000 micrograms/mL directly into the scala tympani of the guinea pig cochlea. Median auditory brainstem response threshold shifts of 12.5 and 20 dB were observed in the experimental ears exposed to the higher dose at days 2 and 4, respectively, with gradual improvement to near baseline by day 28. Histologic examination revealed inflammatory infiltrates in the scala tympani and scala vestibuli of several of these cochleas. The lower concentration,
