The serum of 134 patients infected with Schistosoma mansoni, including both acute and chronic forms of the disease, were tested for the presence of cold lymphocytotoxins. These were found in 4.5% of acute forms of the disease, in 88.4% of chronic infections and in only 9.8% of the uninfected control group (blood donors). An analysis of the results suggests a probable role for cold lymphocytotoxins in the immune response of the different phases of human schistosomiasis.

Haematuria was found in 27% of mice experimentally infected with Schistosoma mansoni. The presence of red blood cells in the urine was detected only after the sixth week of infection. Haematuria was not due to the direct action of the parasite, since no eggs were found in the urinary system. The blood in the urine disappeared after two weeks of curative treatment with an anti-schistosomal compound.

In endemic regions, Mansoni schistosomiasis is responsible for high morbidity-mortality rates due to diseases associated with infestation of the hepatic system. Genital involvement caused by Mansoni schistosomiasis is rare. We report the first case of Mansoni schistosomiasis in the seminal vesicle, which was diagnosed incidentally by means of histopathological study of the prostate and seminal vesicles after surgical removal.

Schistosomiasis is a major public health problem in Yemen. In 1922 Schistosomiasis mansoni was first reported to be found in Yemeni patients. A 36-year-old patient, proceeding from an endemic area was admitted with episodes of total microscopic hematuria and terminal dysuria. Cytoscopy showed a protuberant tumor in the bladder anterior wall, suggestive of cancer. Biopsy revealed schistosomiasis mansoni. Transurethral resection confirmed the diagnosis. The patient was clinically treated and cured with oxamniquine. The case presented in this work emphasizes that not all the tumors of bladder with microscopic hematuria are bladder cancers.

The role of anti-idiotype Ab2 in regulation of the immune response in schistosomiasis was studied. Eleven patients with schistosomiasis mansoni received a renal transplant. In 5 patients, the schistosomiasis was asymptomatic and had been diagnosed by routine examinations and had no relationship to nephropathy. In 4 patients, the nephropathy was suggestive of being of schistosomal origin. Three of them had symptomatic hepatosplenic schistosomiasis, and histologic studies of original kidneys disclosed chronic glomerulonephritis in 2 and membranous glomerulonephritis in 1 patient. Schistosomal periportal fibrosis produced a typical pattern on computed tomography in five patients. Low-density periportal tissue, present throughout the liver, enhanced strongly after the administration of contrast medium. While rounded in cross section, the thickened periportal tissue produced linear and branching patterns when imaged in longitudinal section. In all cases, the sonographic features were typical of schistosomal periportal fibrosis. A lack of awareness of the distinctive features of periportal fibrosis may result in a mistaken diagnosis of hepatic metastases.

Ultrasound (US) was performed in 103 patients with hepatosplenic schistosomiasis mansoni. The abnormal US findings are reported and correlated with the pathologic and anatomic characteristics. The main US findings are periportal fibrosis, left lobe hypertrophy, thickening of the wall of the gallbladder, granulomas, splenic nodules, and widening of the portal vessel system. Frequent but nonspecific findings include splenomegaly and the presence of collateral vessels. Portal vein thrombosis was present in a few cases. It is concluded that US is a major tool in the diagnosis of this disease.
