193 nm excimer laser sclerostomy in pseudophakic patients with advanced open angle glaucoma.

A modified open mask system incorporating an en face air jet to dry the target area during ablation and a conjunctival plication mechanism, which allows ab externo delivery of the 193 nm excimer laser without prior conjunctival dissection, has been developed to form small bore sclerostomies accurately and atraumatically. Full thickness sclerostomies, and sclerostomies guarded by a smaller internal ostium can be created. A pilot therapeutic trial was conducted in pseudophakic patients with advanced open angle glaucoma. Six full thickness sclerostomies (200 microns and 400 microns diameter) and

Spontaneous and pharmacologically-modulated wound healing after Nd:YAG laser sclerostomy ab interno in rabbits.

UNLABELLED: The course of tissue repair evoked by Nd:YAG laser sclerostomy ab interno and the effect of mitomycin-C on same were investigated ultrastructurally in 15 rabbits. In ten of them, the dynamics of the spontaneous postoperative healing response (i.e. in the absence of medication) was analyzed at two-day intervals by light- and electron microscopy. In two, fibroblast activity was assessed autoradiographically. In the last three rabbits, the sclerostomy site was exposed transconjunctivally to mitomycin-C (0.5 mg/ml; 5 minutes) before surgery, and morphological analysis was done on posto

Internal sclerostomy using laser ablation of dyed sclera in glaucoma patients: a pilot study.

Twelve eyes of 12 patients with refractory glaucoma were treated with internal sclerostomy using laser ablation of dyed sclera. The technique involves iontophoretic impregnation of the sclera at the limbal region with 1% methylene blue and subsequent delivery of a pulsed dye laser beam to the stained area through a goniolens. The red wavelength of 664 nm generated by the laser is maximally absorbed by the stained sclera and creates a through and through sclerostomy. Successful complete sclerostomy was achieved in seven eyes (58.3%) but there was some reduction of intraocular pressure (IOP) in

Use of the giant-pulse Nd:YAG laser for ab-externo sclerostomy in rabbits and humans.

The giant-pulse Neodymium:YAG laser, a recently developed laser source, delivers up to 200 W of peak power at pulse widths of 20 or 40 msec. The instrument can create a sclerostomy with minimal surrounding damage and, consequently, relatively little postsurgical inflammation and scarring. We tested its efficacy in performing ab-externo sclerostomy in 10 adult pigmented rabbits and in five patients with end-stage glaucoma. All of the animal eyes were hypotonus immediately after the procedure. However, the intraocular pressure increased gradually, and by the third day, returned to presurgical le

Accuracy of scleral transillumination techniques to identify infant ciliary body for sclerostomy and intravitreal injections.

IMPORTANCE: There is variation in the literature for sclerotomy and intravitreal injection placement in young children, ranging from 0.5 to 3.0 mm from the limbus. We assess the accuracy of scleral transillumination to identify the ciliary body in infants for safe sclerotomy and intravitreal injections in young children. BACKGROUND: The study compares the perilimbal "dark band" seen on scleral transillumination (STI) with the ultrasound biomicroscopy (UBM), and compares these measurements with the current guidelines for sclerotomy in infants. DESIGN: Prospective case series in a tertiary pae

A temporary dissecting seton for trabeculectomy: delayed re-creation of the glaucoma filtering sclerostomy and bleb.

Presented is a new surgical technique to re-create the entire filtering pathway 2 weeks following trabeculectomy. The dissecting seton is a suture loop of 9-0 nylon, silk, or polypropylene placed in the potential outflow space of the sclerostomy and bleb area during trabeculectomy. The seton is pulled out at the slit lamp, under topical anesthesia, 1 to 3 weeks after the initial surgery in order to re-create the filtration pathway. By pulling on the seton, a cheesewiring dissection severs newly formed fibrovascular tissue that may have occluded or constricted the previously created aqueous out

193-nm excimer laser sclerostomy using a modified open mask delivery system in rhesus monkeys with experimental glaucoma.

Excimer laser sclerostomy is a new glaucoma filtration procedure in which the argon fluoride excimer laser at 193 nm is delivered ab externo through a modified open mask system incorporating an en-face air jet to dry the target area and preserve hemostasis during ablation and a conjunctival plication mechanism, which allows the conjunctival and scleral wounds created by through-and-through ablation to separate once the mask is removed. No preparatory dissection of the conjunctiva is required. Five 200-microns and five 500-microns sclerostomies were formed by ablation at a pulse repetition rate

Transconjunctival application of mitomycin C in combination with laser sclerostomy ab interno: a long-term morphological study of the postoperative healing process.

The precise mechanism whereby mitomycin C enhances IOP reduction in glaucoma filtering surgery still eludes us. Ten rabbits received full-thickness Nd:YAG laser sclerostomy ab interno and adjunctive intraoperative treatment with mitomycin C (MMC) applied topically over the intact conjunctiva (0.5 mg ml-1 for 5 min). A systematic ultrastructural analysis of the fistulas and surrounding tissue was then conducted in conjunction with clinical observations, over the ensuing 10 weeks. In order to investigate also the extent to which MMC impedes fistula occlusion in the absence of percolating aqueous

Trabeculectomy with resection of ciliary processes in glaucoma following scleral buckling.

We report a case of glaucoma following scleral buckling that required filtering trabeculectomy. Despite anterior entry to the anterior chamber, swollen ciliary processes were incarcerated in the sclerostomy site, blocking aqueous flow. Resection of the occluding ciliary processes resulted in restoration of aqueous flow through the sclerostomy. This case demonstrates that anterior rotation of the ciliary body and processes plays an important role in the pathogenesis of glaucoma following scleral buckling operation.

Secondary management of suprachoroidal hemorrhages.

Ten cases of postoperative suprachoroidal hemorrhages involving the macular area, following eight cataract extractions and two trabeculectomies, were drained through an anterior temporal sclerostomy. Vitrectomy was performed only in case of vitreous incarceration in the corneal wound and through the same sclerostomy. This simple procedure gave good anatomical results in eight cases and good visual results in seven cases. Since this postoperative complication can spontaneously heal well, we limit the indications for drainage to cases of kissing choroidal detachments, of associated serous retina
