Self-expanding metal stent for tracheobronchial strictures.

To determine the role of expandable metal stent (Wallstent) in treating tracheobronchial strictures, 12 patients with recurrent symptoms of airway obstruction due to either benign or malignant strictures were studied. The seven benign strictures were anastomotic stricture following sleeve resection (2) and single lung transplant (1), tracheal amyloidosis (1), idiopathic chondritis (2), and post-tracheostomy stricture (1). The five malignant strictures were due to recurrent adenoid cystic carcinoma of trachea (1), large-cell carcinoma of lung (1), recurrent laryngeal squamous carcinoma (1), squ

[Self-expandable metal stents in the GI tract].

Self-expandable metal Stents (SEMS), first introduced in the late 90s for the palliation of malignant obstructive jaundice and unresectable esophageal malignancy, are now being variably placed for high risk surgical patients in the setting of malignant gastric outlet and colon obstruction. This review attempts to place SEMS into perspective with respect to surgery and other palliative modalities when treating a variety of GI malignancies and attempts to define the role, if any, of SEMS placement in patients with refractory benign GI stenoses.

Placement of retrievable self-expandable metallic stents with barbs into patients with obstructive prostate cancer.

OBJECTIVES: To evaluate the technical feasibility and clinical effectiveness of retrievable self-expandable metallic stents with barbs in patients with obstructive prostate cancer. METHODS: Retrievable self-expandable metallic stents with eight barbs each were inserted into eight consecutive patients with obstructive prostate cancer. Patient ages ranged from 55 to 76 years (mean, 69 years). All eight patients had previously received hormone therapy, and three had undergone palliative transurethral prostatectomy. Stents were removed using a 21-F stent removal set if they caused complications.

[Treatment of malignant colorectal obstruction by means of endoscopic insertion of self-expandable metallic stents].

BACKGROUND AND AIM: Self-expandable metallic stents are being used increasingly to treat the obstruction of different segments of the digestive tract and biliary tree. We present our centre experience on the initial resolution of malignant colorectal obstruction by means of this type of stents. PATIENTS AND METHODS: During a 18-month period, 13 patients patients suffering from malignant obstruction at the level of rectum, sigmoid or descending colon tried to be initially treated by means of endoscopic insertion of stents (non covered enteral Wallstents). Ten procedures were performed with bot

Self-expandable metallic stents in malignant gastric outlet obstructions--an alternative approach using modified techniques.

Malignant gastric outlet obstructions are commonly present in an advanced tumor stage. Surgery and other therapy options are often accompanied with substantial problems and reduced quality of life. We therefore investigated the endoscopic palliation with self-expandable metallic stents. This report documents the clinical benefit of new stent systems. During a period of eleven months we implanted eleven self-expandable metallic stents (one Ultraflex Esophageal Stent/five Ultraflex Duodenal Diamond Stents/five Enteral Wallstents) in eight patients with malignant gastric outlet stenoses (five fem

Application of Y-shaped, coated self-expandable metallic stents for anastomotic stenosis after gastrojejunostomy (Billroth II).

BACKGROUND: Anastomotic stenosis is an infrequent but life-threatening complication after gastrojejunostomy (Billroth II). Tubular or single tubular stents have limited efficacy due to the particular anatomy. PURPOSE: To assess the feasibility of a Y-shaped, fully-coated, self-expandable, metallic stent (SEMS) for anastomotic stenosis after gastrojejunostomy (Billroth II). MATERIAL AND METHODS: Between January 2008 and August 2014, 14 patients (10 with gastric carcinoma and four with duodenal ulcers) had anastomotic stenoses following Billroth II reconstructions. Eight patients with gastric

The role of self-expandable metallic stents for the treatment of airway complications after lung transplantation.

BACKGROUND: Airway complications continue to be an important source of morbidity and mortality after lung transplantation (LTx). Different approaches have been used for their nonsurgical management. We describe our experience using self-expandable metallic stents (SEMSs) in patients with airway complications post-LTx. METHODS: We present a retrospective analysis of stent related-data of all the LTx patients who received SEMSs to treat postoperative airway complications. RESULTS: Between January 1992 and December 2001, 36 of 253 patients (14.2%) developed post-LTx airway complications involvi

Management of malignant carinal involvement using 'side-by-side' method of bilateral self-expandable metallic stents.

Central airway obstruction caused by cancer requires urgent interventional pulmonology. Malignant main carinal involvement is one of the most challenging situations, usually treated by rigid bronchoscopic intervention under general anaesthesia. However, these patients tend to be in poor condition due to underlying malignancy. Therefore, less-invasive strategies are needed. Six patients with lung cancer exhibiting malignant carinal involvement treated using the 'side-by-side' method of bilateral self-expandable metallic stents using fibre-optic bronchoscopy under topical anaesthesia were retros

Self-expandable metallic stents in oesophago-respiratory fistulas treatment in neoplasms - case reports and literature review.

Self-expandable metallic stents (SEMS) insertion to the oesophagus is the method of choice in palliative treatment of malignant oesophago-respiratory fistulas (ERF). ERF evolve as a result of 0.2-5% of advanced oesopghageal or lung cancer. They also appear after radiotherapy. EFR are serious and lethal complications. SEMS insertion is an effective, safe, technically simple and minimally invasive solution. Potential complications of SEMS include thoracic pain, bleeding, perforation, stent migration, tumour overgrowth or ingrowth and food bolus impaction. The article presents three cases of pati

Spontaneous thrombosis of a saccular iliac artery aneurysm induced by overlapping self-expandable bare metallic stents.

Isolated iliac artery aneurysms are rare lesions. Prompt diagnosis and timely intervention are essential in their management. In elderly patients with coexisting medical problems, endovascular procedures are preferred to avoid the risks of morbidity and mortality associated with general anesthesia and surgery. We report a case of a true saccular external iliac artery aneurysm that underwent spontaneous thrombosis after treatment with placement of two overlapping bare self-expandable metallic stents only.

Self-expanding metal stents in postoperative esophageal leaks.

BACKGROUND: Postoperative esophageal leaks have a high morbidity and mortality. Self-expanding metal stents (SEMS) have been used as an alternative to re-operation. AIM: Evaluating predictors of success of SEMS in postoperative esophageal leaks. METHODS: Retrospective study of patients with postoperative esophageal leaks referred for SEMS placement in a reference center during a period of 3 years. Technical success was defined as closure of the leak in barium swallow at 15 days. Clinical success was considered as endoscopic and/or radiographic confirmation of closure after stent removal. RE

Self-expandable metal stents in malignant biliary obstruction.

Malignant biliary obstruction can be due to direct tumor infiltration, extrinsic compression, adjacent inflammation, desmoplastic reaction from tumors or, more commonly, a combination of the above factors. Pancreatic cancer is the most common cause of malignant biliary obstruction, and jaundice occurs in 70-90% of the patients during the course of the disease. Compared with the uncovered metal stents, covered metal stents have longer patency and a lower rate of tumor ingrowth, but have a higher rate of stent migration. To combat the occlusion and provide an antitumor effect, drug-eluting stent

Self-expanding metallic stent insertion in the proximal colon.

OBJECTIVE: Colonic obstruction may be relieved by the insertion of a self-expanding metallic stent (SEMS), either for permanent palliative relief or as a bridge to surgery. Lesions proximal to the descending colon can be more difficult to intubate and stent [1]. SEMS placement in the more proximal colon lesions has been reported in only a few cases [2,4]. The aim of this study was to review the outcome of SEMS for obstruction at the splenic flexure and above. METHOD: A study of all colonic stents inserted in one specialist unit was undertaken. [Self-expandable metallic stent in the treatment of colorectal obstruction].

Self-expandable metallic stent (EMS) treatment for colorectal disease was delayed compared with that for other organs. Recently, however, there have been many reports on the use of EMS in colorectal disease, mainly from the West. EMS is generally used as a palliative treatment for malignant stricture of the colon and rectum and before bridge as a surgery for obstructing colorectal cancers. Some investigators have reported on the usefulness of EMS for stricture of benign disease. The indications for EMS treatment must be considered carefully because of the lack of long-term results of the effec
