Pacemaker current (I(f)) in the human sinoatrial node.

AIMS: Animal studies revealed that the hyperpolarization-activated pacemaker current, I(f), contributes to action potential (AP) generation in sinoatrial node (SAN) and significantly determines heart rate. I(f) is becoming a novel therapy target to modulate heart rate. Yet, no studies have demonstrated that I(f) is functionally present and contributes to pacemaking in human SAN. We aimed to study I(f) properties in human SAN. METHODS AND RESULTS: In a patient undergoing SAN excision, we identified SAN using epicardial activation mapping. From here, we isolated myocytes and recorded APs and I(

[Direct recording of electrical activity of human sinoatrial node].

The authors developed a method for recording the electrical activity directly from the human sinus node through the right subclavian vein. Electrical activity of the sinoatrial node (SA) was studied in 179 patients with CHD (angina pectoris, myocardial infarction). The sick-sinus syndrome (SSS) was revealed in 86 patients. The duration of slow deflection Sd, Ss -- P interval (from the onset of Ss to the onset of P wave), P -- A1 interval (from the onset of P wave to the first atrial spike) was prolonged in cases with SSS. The duration of Sd, Ss -- P and P -- A1 decreased after nitroglycerine a

Structural remodelling of the sinoatrial node in obese old rats.

During ageing, the function of sinoatrial node (SAN), the pacemaker of the heart, declines, and the incidence of sick sinus syndrome increases markedly. The aim of the study was to investigate structural and functional remodelling of the SAN during ageing. Rats, 3 and 24 months old (equivalent to young adult and approximately 69-year-old humans), were studied. Extracellular potential recording from right atrial preparations showed that (as expected) the intrinsic heart rate was slower in the old animals. It also showed a shift of the leading pacemaker site towards the inferior vena cava in the

[Functional state of the sinoatrial node in patients with intraventricular block].

The function of sinus node (SN) was studied in 101 subjects with intraventricular blocks (IVB) and 30 controls. The patients with IVB showed a significant increase in the corrected time of SN function recovery and that of sinoatrial conduction both before and after medicamentous++ autonomic block, as compared to the controls. In general, signs of SN dysfunction were detected in 47 (46.5%) subjects with IVB. In 18.8% of the examinees, SN dysfunction was due to abnormal autonomic exposures, 27.7% had abnormalities in SN proper or perinodal tissue. ++Tachy-bradycardia was found in 53.5% patients

Local opiate receptors in the sinoatrial node moderate vagal bradycardia.

Met-enkephalin-arg-phe (MEAP) interrupts vagal bradycardia when infused into the systemic circulation. This study was designed to locate the opiate receptors functionally responsible for this inhibition. Previous observations suggested that the receptors were most likely located in either intracardiac parasympathetic ganglia or the pre-junctional nerve terminals innervating the sinoatrial node. In this study 10 dogs were instrumented with a microdialysis probe inserted into the sinoatrial node. The functional position of the probe was tested by briefly introducing norepinephrine into the probe

[Does the sinoatrial node conduction time depend on heart rate?].

The informations about the dependence of sinoatrial conduction time (SACT) on heart rate (HR) are unequivocal. The aim of the study was to establish if such a dependence really exists. The study group comprised 24 patients (9 women and 15 men) aged 42-71 years, mean 55.8. All the patients presented atrial premature contractions of various etiology. No agents affecting heart electrophysiology were administered until the study was completed. In every patient SACT was estimated by means of the Strauss formula from the 24-hr ecg and from transoesophageal single impulse stimulation. SACT obtained f

[Sinoatrial node disease in cardiac amyloidosis. Description of 2 cases].

We describe two patients with probable amyloid infiltrative cardiomyopathy and episodes of syncope. A sick sinus syndrome was suspected by Holter monitoring in both patients. The diagnosis was supported by the finding of a marked prolongation of sinus node recovery time in one case. In both patients a permanent pacemaker was implanted which prevented recurrence of syncopal episodes.

Insulin signaling is critical for sinoatrial node maintenance and function.

Insulin and insulin-like growth factor 1 (IGF-1) signaling regulate cellular growth and glucose metabolism in the myocardium. However, their physiological role in the cells of the cardiac conduction system has never been explored. Therefore, we sought to determine the spatiotemporal function of insulin/IGF-1 receptors in the sinoatrial node (SAN). We generated cardiac conduction cell-specific inducible IGF-1 receptor (IGF-1R) knockout (KO) (CSIGF1RKO), insulin receptor (IR) KO (CSIRKO), and IR/IGF-1R double-KO (CSDIRKO) mice and evaluated their phenotypes. Telemetric electrocardiography reveal

Current aspects of the basic concepts of the electrophysiology of the sinoatrial node.

Cardiac pacemaker cells, also named P-cells (pale cytoplasm, pacemaker, phylogenetically primitive), including cells of the sinoatrial node, are heterogeneous in size, morphology, and electrophysiological characteristics. The exact extent to which these cells differ electrophysiologically in the human heart is unclear, yet it is critical for the understanding of normal cellular function. In this review, we describe major ionic currents and Ca2+ clocks acting on Ca2+ release in the sarcoplasmic reticulum. We also explain the external regulation of the heart rate controlled by the two branches o

The role of the calcium and the voltage clocks in sinoatrial node dysfunction.

Recent evidence indicates that the voltage clock (cyclic activation and deactivation of membrane ion channels) and Ca(2+) clocks (rhythmic spontaneous sarcoplasmic reticulum Ca(2+) release) jointly regulate sinoatrial node (SAN) automaticity. However, the relative importance of the voltage clock and Ca(2+) clock for pacemaking was not revealed in sick sinus syndrome. Previously, we mapped the intracellular calcium (Ca(i)) and membrane potentials of the normal intact SAN simultaneously using optical mapping in Langendorff-perfused canine right atrium. We demonstrated that the sinus rate increas

Endo-Epicardial Mapping of In Vivo Human Sinoatrial Node Activity.

OBJECTIVES: The aim of the current study was to examine electrophysiological characteristics of sinoatrial node (SAN) activity from an endo-epicardial perspective. BACKGROUND: Electrophysiological properties of the in vivo human SAN and its exit pathways remain poorly understood. METHODS: Twenty patients (75% male; median age 66 years [59 to 73 years]) with structural heart disease underwent simultaneous endo-epicardial mapping (256 unipolar electrodes, interelectrode distance 2 mm). Conduction times, endo-epicardial delays (EEDs), and R/S ratio were examined in the surrounding 10 mm of SAN

SARS-CoV-2 Infection Induces Ferroptosis of Sinoatrial Node Pacemaker Cells.

BACKGROUND: Increasing evidence suggests that cardiac arrhythmias are frequent clinical features of coronavirus disease 2019 (COVID-19). Sinus node damage may lead to bradycardia. However, it is challenging to explore human sinoatrial node (SAN) pathophysiology due to difficulty in isolating and culturing human SAN cells. Embryonic stem cells (ESCs) can be a source to derive human SAN-like pacemaker cells for disease modeling. METHODS: We used both a hamster model and human ESC (hESC)-derived SAN-like pacemaker cells to explore the impact of severe acute respiratory syndrome coronavirus 2 (SA

Sinoatrial node toxicity after stereotactic ablative radiation therapy to lung tumors.

PURPOSE: Stereotactic ablative radiation therapy (SABR) is an established treatment for selected lung tumors. Sinoatrial node (SAN) toxicity after thoracic SABR has not been reported in the literature. We sought to understand the risk of SAN toxicity owing to incidental dose to the SAN from SABR. METHODS AND MATERIALS: We conducted a retrospective review of patients with early-stage lung cancer or limited pulmonary metastases who underwent thoracic SABR to a right-sided central lung tumor (within 2 cm of the mainstem bronchus or other mediastinal structures) between January 2008 and December

Culture and adenoviral infection of sinoatrial node myocytes from adult mice.

Pacemaker myocytes in the sinoatrial node of the heart initiate each heartbeat by firing spontaneous action potentials. However, the molecular processes that underlie pacemaking are incompletely understood, in part because of our limited ability to manipulate protein expression within the native cellular context of sinoatrial node myocytes (SAMs). Here we describe a new method for the culture of fully differentiated SAMs from adult mice, and we demonstrate that robust expression of introduced proteins can be achieved within 24-48 h in vitro via adenoviral gene transfer. Comparison of morpholog

Arterial supply of the sinoatrial node: a CT coronary angiographic study.

We aimed to investigate the variances in especially the origin, course and termination of the sinoatrial node (SAN) artery in this study, using coronary CT angiography. The coronary CT angiography images of 251 patients (190 men and 61 women; age range, 20-82 years; mean age, 54.4 ± 13.6 years) were retrospectively analyzed. The SAN artery (arteries) in each case was named according to a special nomenclature with regard to their origin, course and termination. The sinoatrial node was being vascularized by a single artery in 241 (96%) cases and by two arteries in 10 (4%) cases. It was arising f
