Skin cancer in organ transplant recipients.

Organ transplant recipients (OTR) are at a significantly increased risk for developing a wide variety of skin cancers, particularly epithelial skin cancer, Merkel cell carcinoma and Kaposi's sarcoma. Melanoma, skin adnexal neoplasm and cutaneous lymphomas are also more common in OTR and may differ in their clinicopathologic presentation from tumors in immunocompetent patients. The accuracy of clinical diagnosis of suspected premalignant and malignant skin lesions in OTR is modest. Therefore, histopathological diagnosis is an essential element for the diagnostic workup of skin cancers and, in a

[Skin cancers after organ transplants].

Squamous cell carcinoma and basal cell carcinoma are the most common cancers after transplants, affecting more than half of all patients in the long term. In the 5 years after a first squamous cell carcinoma, 90 to 100% of transplant patients subsequently develop multiple skin carcinomas of various types and at least 20% develop noncutaneous cancers. Management of these patients requires, beyond the usual dermatologic treatments, a revision of their immunosuppression regimen to reduce the tumor risk. New immunosuppressants that inhibit the m-TOR protein (sirolimus and everolimus) appear to off

Skin disorders in patients transplanted in childhood.

Only few data are available on skin disorders in pediatric organ transplant recipients. In order to describe the whole range of dermatological diseases in a population of pediatric organ transplant recipients, we studied a group of 217 consecutive organ transplant recipients (168 kidney, 29 heart, 19 liver, one lung) aged <18 years at transplantation followed at a single center. A total of 193 patients showed at least one skin disorder; 149 had more than one skin disease. The most common skin infections were warts (24.4%), pityriasis versicolor (20.7%), folliculitis (12.9%), intertrigo (6.5%);

[Skin lesions in renal transplant recipients].

Renal transplantation is considered the only good long-term therapy for chronic renal failure. Better surgical techniques and recent advances in immunosuppressive therapy allow patients to survive for many years, free from any complications due to rejection. However, cutaneous lesions can be a significant problem for this group of patients. Immunosuppression predisposes them to infections--predominantly viral, but also fungal and bacterial ones, as well as malignant tumours like basal cell carcinoma, squamous cell carcinoma, melanoma and Kaposi's sarcoma. The review of the current literature o

Skin cancer in liver transplant recipients.

Skin cancer is the most common malignancy arising in the posttransplantation setting. Multiple factors contribute to the high risk for cutaneous carcinoma in immunosuppressed organ-transplant recipients. We review the phenomenon of skin cancer in solid-organ transplant recipients and further delineate the problem in the context of liver transplantation. Skin cancer is a significant medical and surgical problem for organ-transplant recipients. With prolonged allograft function and patient survival, the majority of solid-organ transplant recipients will eventually develop skin cancer. Although s

Skin Cancers in Organ Transplant Recipients.

Long-term utilization of immunosuppression in organ transplant recipients (OTRs) leads to decreased immune-mediated tumor surveillance and development of malignant tumors. A delicate balance needs to be maintained in the intensity of immunosuppression to keep the risk of malignancy low without jeopardizing life-saving graft function. OTRs are prone to developing skin cancers that exhibit unique epidemiologic, pathophysiologic, and prognostic characteristics. In this review, we discuss the most commonly reported skin cancers in OTRs: squamous cell carcinoma (SCC), basal cell carcinoma (BCC), Ka

[Skin carcinogenesis in renal transplant recipients].

Organ transplants are bound to the induction and upkeep of immunosuppression. The immunomodulatory regimen modalities have changed over time with an improvement of the overage graft survival. The effects are also present on the skin and its cancerogenesis which is particularly activated for the squamous cell carcinoma. A review of the recent medical literature reveals the importance of searching squamous cell carcinomas, actinic keratoses, seborrheic keratoses and keratocanthomas before and after organ transplants. The severity of the carcinomas is due to their multiplicity and to the progress

[Skin cancer in renal transplant recipients].

Kidney transplant is the best treatment option for renal failure. Over the years the 1-year patient survival has gradually increased, along with a reduction in acute rejection. The main causes of death in the late post-transplant period are cardiovascular disease, infections and malignancies. It is known that the incidence of cancer increases with the duration of post-transplant follow-up. Twenty years after transplantation, approximately 70% of patients on continuous immunosuppressive therapy present one or more tumor types. Some of the tumors that occur with a significantly higher frequency

Skin infections in organ transplant recipients.

In contrast to the well-described high risk of skin cancer in organ transplant recipients, skin infections in these patients are not as well explored. Skin infections caused by viruses, bacteria or fungi represent a growing diagnostic and therapeutic challenge in the dermatological aftercare of organ transplant recipients. Differing immunosuppressive drugs and their variable dosage in chronologic sequence after transplantation probably influence the type and appearance of skin infections. The typical chronology of skin infections are wound infections, pyoderma or the reactivation of herpes vir

Skin diseases in kidney transplant recipients.

INTRODUCTION: The aim of this study was to evaluate the frequency of skin diseases in kidney transplant recipients. MATERIALS AND METHODS: This cross-sectional study was performed on 233 kidney transplant recipients in Ekbatan Hospital of Hamedan in 2004. The patients were examined by a dermatologist and diagnosis was made on the basis of clinical observations. Biopsies and scraping of the lesions were taken whenever necessary. RESULTS: Of the patients, 226 (97%) suffered from one or more skin lesions. The most common lesions were drug related, including hypertrichosis, gingival hyperplasia,

Skin infections in renal transplant recipients.

BACKGROUND: Skin infection is a frequent complication in renal transplant recipients. The purpose of the study was to acquire long-term, period-specific incidence data on the most commonly occurring skin infections in renal transplant recipients. METHODS: A retrospective analysis was performed using medical records of 134 patients, covering a period between 10 and 29 yr. Cumulative incidences of the skin infections were calculated by counting the infections per patient for different time periods and were expressed as a percentage of the total group of patients. The incidence of the skin infec

Skin diseases in children with organ transplants.

BACKGROUND: Skin diseases are frequent in organ transplant recipients, but studies concerning children are sparse. OBJECTIVE: We assessed skin diseases in children who had received organ transplants. METHODS: A total of 145 children referred to our dermatologic consultation were studied. RESULTS: Steroid-induced striae distensae and acne occurred only in adolescents; severe cyclosporine-related side effects were more frequent in younger children. The most common findings were warts (53.8%), tinea versicolor (14.5%), herpes simplex/zoster (9.6%), molluscum contagiosum (6.9%), and impetigo co

Prolonged skin allograft survival by rM180 amelogenin in a murine skin transplantation model.

INTRODUCTION: Amelogenin, used as a periodontal tissue regeneration material, promotes healing after periodontal surgery. A previous study has demonstrated that amelogenin is taken up by macrophages into the nucleus and inhibits major histocompatibility class II (MHC II) expression at the transcriptional level, thereby suppressing subsequent T cell activation. Therefore, in this study, we focused on the suppressive effect of amelogenin on MHC II expression and examined the effect of amelogenin on graft rejection following allogeneic skin transplantation in mice with different MHC II haplotype

[Allogeneic skin transplantation in chronic graft-versus-host disease following bone marrow transplantation].

A 20-year old patient with acute myelogenous leukemia was transplanted with allogeneic bone marrow transplantation from his HLA-identical brother. Chronic graft-versus-host disease with ulcerous skin lesions developed at both lower legs. The lesions were extensive and conservative therapy was unsuccessful. Therefore, allogeneic skin transplantation from the marrow donor was performed. At present, four years later, these skin grafts are still vital.

Skin changes following organ transplantation: an interdisciplinary challenge.

BACKGROUND: The immunosuppressants used in transplantation medicine significantly elevate the incidence of neoplasia, particularly in the skin. The cumulative incidence of non-melanocytic skin cancer (NMSC) in renal transplant recipients was 20.5% in a study carried out in German centers. Data on more than 35 000 renal transplant recipients in the USA document a cumulative NMSC incidence of over 7% after 3 years of immunosuppression. METHOD: The authors selectively review publications obtained by a PubMed search to discuss the incidence of, and major risk factors for, skin tumors and infectio
