Antibiotic prophylaxis for preventing meningitis in patients with basilar skull fractures.

BACKGROUND: Basilar skull fractures predispose patients to meningitis because of the possible direct contact of bacteria in the paranasal sinuses, nasopharynx or middle ear with the central nervous system (CNS). Cerebrospinal fluid (CSF) leakage has been associated with a greater risk of contracting meningitis. Antibiotics are often given prophylactically, although their role in preventing bacterial meningitis has not been established. OBJECTIVES: To evaluate the effectiveness of prophylactic antibiotics for preventing meningitis in patients with basilar skull fractures. SEARCH METHODS: We s

Traumatic chiasmal syndrome associated with midline basilar skull fractures.

We studied two young males who had visual field defects consistent with optic chiasmal injury after blunt frontal head trauma. One patient also had a unilateral optic neuropathy. Long-term follow-up disclosed complete bi-temporal hemianopsias in these patients. Both patients had midline basilar skull fractures that traversed the midclivus through the sella turcica floor, dorsum sellae, and sphenoid sinus. Magnetic resonance images did not identify intrachiasmal hemorrhage as the cause of the visual field defect. We believe this fracture pattern, in conjunction with the magnetic resonance image

Basilar skull fracture: a risk factor for transverse/sigmoid venous sinus obstruction.

In trauma practice, basilar skull fracture is an extremely common finding while transverse/sigmoid venous sinus thrombosis is generally considered quite a rare complication. During evaluation of cervical computed tomography (CT) angiography after trauma, we identified five patients in just three months with unexpected transverse/sigmoid venous sinus obstruction ipsilateral to a basilar skull fracture. This number represented a surprisingly high percentage of our neurosurgical trauma consults for the study period (31%). Three of the five patients were found to have sinus thrombosis: two with ri

Analysis of carotid artery injury in patients with basilar skull fractures.

OBJECTIVES: Determine the prevalence of carotid artery injury (CAI) in patients with basilar skull fractures and describe significant demographic and radiographic risk factors for CAI. STUDY DESIGN: From January 2004 to December 2008, medical records of 1,279 consecutive adult patients treated for basilar skull fractures at a tertiary care academic hospital were retrospectively reviewed. Diagnostic angiography was performed in 112 patients because of concern for CAI. Computed tomographic studies of the head and cranial base were reviewed for evidence of pneumocephalus, petrous carotid canal f

Predicting recovery of facial nerve function following injury from a basilar skull fracture.

Twenty-five patients with posttraumatic facial nerve palsy were studied. Partial recovery of function had occurred in 95% of these patients by 18 months after injury. At 5 months posttrauma, there was some recovery in 92.5% of those with a partial lesion compared with 10% of those with a complete lesion. This difference attains statistical significance. Complete recovery of nerve function had occurred by 10.5 months in 53.5% of the patients; in 62% of patients with a partial lesion, complete recovery had occurred by 4 months compared with 0% in those with a complete lesion. This difference als

Persistent neutrophilic meningitis in an immunocompetent patient after basilar skull fracture: case report.

BACKGROUND: Persistent neutrophilic meningitis is an unusual form of chronic meningitis that is defined as clinical meningitis with a neutrophilic pleocytosis that persists for greater than 7 days despite empiric antimicrobial therapy. Although numerous disease processes can cause this syndrome, the majority of cases are due to opportunistic pathogens infecting immunocompromised hosts. CASE PRESENTATION: A 47 year-old female presented after basilar skull fracture with persistent neutrophilic meningitis unresponsive to empiric broad-spectrum antibiotics. After more than weeks of intensive ther

Clinical Signs of Basilar Skull Fracture and Their Predictive Value in Diagnosis of This Injury.

Although clinical signs for the diagnosis of basilar skull fracture (BSF) are ambiguous, they are widely used to make decisions on initial interventions involving trauma patients. We aimed to assess the performance of early and late (within 48 hr posttrauma) signs for BSF diagnosis and to verify the correlation between the presence of these signs and head injury severity. We conducted a prospectively designed follow-up study at a referral hospital for trauma care in Sao Paulo, Brazil, and performed structured observations for 48 hr post-blunt head injury in patients aged 12 years or older. The

Basilar skull fractures in children. The incidence of CNS infection and the use of antibiotics.

Forty-six cases of basilar skull fractures in children were reviewed to determine the incidence of CNS infection following injury and the possible value of antimicrobial chemoprophylaxis. The clinical course of the children who were treated with antibiotics was compared with that of patients who received no antimicrobial therapy. Included in the study were patients with hemotympanum alone or with hemotympanum plus additional clinical or roentgenographic signs of basilar skull fracture; patients with tympanic membrane perforation without otorrhea but with blood in the auditory canal; and childr

Air in the right ventricle and vein after basilar skull fracture: a case report.

BACKGROUND: Air in the venous system may cause vascular air embolism, which is a potentially life-threatening event. The presence of air in venous system after basilar skull fracture is very rare. CASE PRESENTATION: A 77-year-old man fell from a truck bed and suffered head and neck trauma. On hospital arrival, his consciousness was clear and his vital signs were stable. His chief complaint was pain in the back of his head and neck. Head CT showed traumatic subarachnoid hemorrhage in the right frontal area and basilar skull fracture of the occipital bone. Whole body CT showed pneumocephalus an

Influence of a combo side airbag on the risk for basilar skull fracture in a far-side occupant.

OBJECTIVE: The impact force to the head and neck were measured in sled tests with and without inflation of a combo airbag for a far-side occupant to determine the risk for basilar skull fracture. METHODS: Sled tests were run at 24 and 32 km/h (15 and 20 mph) with and without inflation of a combo side airbag to analyze the effect of cross-car diving into the side interior. The matched tests involved one pair at 24 km/h and another at 32 km/h. The 24 km/h pair involved a lap-shoulder-belted 5th percentile female Hybrid III and the series at 32 km/h involved an unbelted 5th percentile Hybrid III

[Special aspects in diagnosis and treatment of open craniocerebral injuries including basilar skull fractures].

The possibility of CSF leakage means that open head injuries and skull base fractures are serious and sometimes dangerous. An operation should be considered in cases of open depressed skull fractures with or without space-occupying haematoma, compound fractures of the rhinobasis and orbita, dislocated fractures of the dorsal wall of the frontal sinus, CSF leakage for longer than 1 week, and posttraumatic intracranial infection. Operative and posttraumatic intracranial infection is also potentially dangerous. The operative approach depends on the type and localization of the fracture. The trans
