Midline depressed skull fracture presenting with quadriplegia: A rare phenomenon.

BACKGROUND: Midline depressed skull fractures (MDSFs) deserve a special mention among skull fractures and should always be treated with caution. Here, an extremely unusual clinical presentation of a case of MDSF is highlighted along with its successful surgical management. CASE DESCRIPTION: A 26-year-old male presented with quadriplegia following assault on the head with sharp weapons. The patient had multiple lacerated wounds on the scalp with underlying cranial fractures. On evaluation, computerized tomography (CT) of the brain showed a midline depressed skull fracture compressing the super

Radiolucent wooden foreign body masquerading as a depressed skull fracture.

The following case describes a head injury in an older male patient in which a large wooden foreign body, forming a subcutaneous tract in the scalp, was mistaken for a depressed skull fracture on initial clinical assessment. This foreign body was not visualised on CT brain imaging until specialised views were used retrospectively, after surgical exploration of the scalp laceration. Appropriate radiological techniques for the detection of radiolucent foreign bodies are discussed.

Analysis of Factors Influencing Outcome of Depressed Fracture of Skull.

CONTEXT: The outcome of patients with depressed fracture varies and depends on multiple factors. There has been no previous study on the significance of these factors on the outcome of depressed fracture of the skull and hence this study. Aims: The primary aim of our study is to find the factors affecting the outcome in cases of depressed skull fracture (DSF). This will help us improve outcomes and give more accurate prediction of long-term outcomes. SETTINGS AND DESIGN: Prospective observational study. SUBJECTS AND METHODS: Institutional Ethics Committee approval was taken for doing this ob

Depressed skull fracture in the neonate. Report of three cases.

The authors describe three cases of neonatal depressed skull fracture that were elevated by means of an obstetrical vacuum extractor. In one case, a transparent breast pump shield replaced the metal vacuum extractor cup, permitting direct observation as the depression was elevated. Neonatal depressed skull fractures not associated with neurological signs may be safely elevated without surgery using the obstetrical vacuum extractor.

Delayed occurrence of intracranial supratentorial chondroma following compound depressed skull fracture.

We describe an exceptional case of a frontal convexity chondroma arising at the site of a compound depressed skull fracture operated on 12 years earlier. We conclude that intracranial chondroma should be included in the differential diagnosis of a calcified mass for the patients who had had a compound, depressed skull fracture along the suture line, especially in cases of dural laceration by the fragmented bone.

Open depressed skull fracture missed on computed tomography: a case report.

A case of an open depressed skull fracture that was missed on standard computed tomographic (CT) scan is presented. The fracture was seen on a CT generated lateral scout film, and after repositioning the gantry, further CT images clearly showed the fracture and underlying brain injury. The authors recommend that all patients with head trauma undergoing head CT have either a CT generated lateral scout film or a lateral skull radiograph to assess the vertex of the skull, an area poorly visualized on standard axial images.

Depressed Skull Fracture in Infants: The Role of Vacuum-Assisted Intervention.

Elevation of depressed skull fractures in pediatric patients is indicated when the depression extends below the level of the inner table. Although open surgical reduction is an option for treatment, closed vacuum-assisted elevation has been found to have a high success rate and low complication rates when employed in children less than 6 months of age without disruption of the cortex. In this article, we describe two recent cases from our institution and summarize the reported cases in the literature including information about fracture specifics, device used, technical approach, and success r

A Case of Depressed Skull Fracture of the Inner Table Alone.

A depressed skull fracture (DSF) is comminuted fractures in which the broken bones displace inward due to a blunt head trauma, such as when the broken outer table is located below the normal anatomical position of the inner table. The author presents an extremely rare case of a 38-year-old man with a DSF that affected only the inner table. In the literature, only 2 cases of DSF involving only the inner table have been reported, and the underlying mechanism remains unclear. The author believes that this phenomenon was not enough for the impact force to depress the outer table, but the tensile s

Delayed Intracerebral Hemorrhage from a Pseudoaneurysm Following a Depressed Skull Fracture.

A 26-year-old male presented with delayed intracerebral hemorrhage from a ruptured distal middle cerebral artery pseudoaneurysm that followed a compound depressed skull fracture from years ago. The brain protrusion through the skull defect likely resulted in stretching and subsequent tearing of the arterial wall resulting in the pseudoaneurysm formation. No prior report of such a clinical occurrence exists in the literature. We highlight an unusual but treatable cause for intracerebral hemorrhage following surgery for traumatic brain injury.

[A Case of Depressed Skull Fracture Involving only the Inner Table].

Depressed skull fracture commonly results from trauma and usually occurs following high-speed impact with a small object. The outer and inner tables of the skull typically break concurrently. We present a case of depressed skull fracture involving only the inner table. The case resulted in a good outcome with only conservative treatment, although the mechanism remained unclear. Fracture models of cadavers have been the main tool for biomechanical investigation, but this classical method cannot accurately measure mechanical factors. We utilized a computer simulation model to assess the human he

[Depressed skull fracture in the newborn infant. To operate or not?].

In recent literature the question of the convenience of neurosurgical treatment of cranial depressed fracture of the newborn has been a subject of discussion. The authors review their experience with 4 cases of this condition, that stress the necessity of the operation. Most reported series have dealt with the short-term results of the different ways of treatment. In this article the outcome is evaluated at least 10 years after surgery.

Camel Bite Associated with Depressed Skull Fracture with Rapidly Spreading Subgaleal Cellulitis.

Camel bite represents a minimal proportion, and most of them are from the Middle East countries. Their infectious potential is poorly understood, and the guidelines for antimicrobial treatment are not well developed. We describe a 40-year-old male, who works as a camel herder and was bitten by a camel while he was tying it down which led to a unilateral depressed skull fracture and multiple bilateral teeth-puncture wounds in the scalp. He arrived to our emergency department 3 hours after injury. All the wounds were dry and the skin around them was healthy looking with no subcutaneous collectio

Depressed skull fracture in a newborn successfully managed conservatively: a case report.

A preterm baby was born to a multiparous mother by emergency caesarean section at 36 weeks of gestation. Apart from a depression on the right temporo-parietal region measuring 3 cm x 3 cm x 0.5 cm, no other abnormality was noted. A CT scan of the brain excluded the presence of intracranial haematoma and pressure effect on the brain. Spontaneous reduction of the fracture without any adverse neurological sequelae suggests that these fractures can be managed conservatively in some instances.

Diphenylhydantoin induced granulocytopenia following a seizure prophylaxis after a depressed skull fracture.

The article deals with a rare case of granulocytopenia following a seizure prophylaxis after a severe head injury. Although fatal complications like the one described occur extremely seldom, we want to emphasize the necessity of regular controls during the critical time period between the 10th and the 67th day of diphenylhydantoin treatment. The blood level should not exceed 10-20 micrograms/ml to prevent any toxic reaction to the myelopoetic system. If this treatment cannot be provided, we suggest to administer Clonazepam or Convulex.

Single-Stage Reconstruction with Titanium Mesh for Compound Comminuted Depressed Skull Fracture.

OBJECTIVE: Traditionally, staged surgery has been preferred in the treatment of compound comminuted depressed fracture (FCCD) after traumatic brain injury (TBI) and involves the removal of primarily damaged bone and subsequent cranioplasty. The main reason for delayed cranioplasty was to reduce the risk of infection-related complications. Here, the author performed immediate reconstruction using a titanium mesh in consecutive patients with FCCD after TBI, reported the surgical results, and reviewed previous studies. METHODS: Nineteen consecutive patients who underwent single-stage reconstruct
