Image-guided surgery for skull base neoplasms using the ISG viewing wand. Anatomic and technical considerations.

Image-guided surgery of the cranial base requires a knowledge of both surgical anatomy and the technical aspects of the particular system being used. The authors report on their experience using the ISG Viewing Wand over the last two years, concentrating on the technical considerations for the use of the system and lessons learned. Image-guided surgical systems may well replace the current forms of intraoperative imaging used for anatomic orientation and localization and are considered a valuable addition to the armamentarium of the skull base surgeon.

Case Report: Intracranial hemophilic pseudotumor mimicking an aggressive neoplasm: a rare skull-invasive presentation.

Hemophilic pseudotumor is a rare complication of hemophilia in which a chronic, expanding hemorrhagic mass erodes adjacent bone; intracranial presentations are exceedingly uncommon and can mimic neoplasms. We describe a 36-year-old man with severe hemophilia A who developed a five-year, progressively enlarging left frontoparietal swelling with worsening right-sided hemiparesis. MRI demonstrated a heterogeneously enhancing extra-axial lesion destroying the left cranial vault with "mushroom-like" extracranial extension, marked mass effect, and midline shift, initially interpreted as a possible m

Unusual presentation of a skull base mass lesion in sarcoidosis mimicking malignant neoplasm: a case report.

BACKGROUND: Sarcoidosis is a multi-organ disease of unknown etiology characterised by the presence of epithelioid granulomas, without caseous necrosis. Systemic sarcoidosis is rare among children, while neurosarcoidosis in children is even rarer whether it is systemic or not. CASE PRESENTATION: We described the case of a 12-year-old boy who presented with monocular vision loss accompanied by unusual MRI features of an extensive meningeal infiltrating mass lesion. The patient underwent surgical resection (biopsy) via a frontotemporal craniotomy to establish a definitive diagnosis based on the

Rare and rapidly growing benign neoplasm arising in skull: Melanotic neuroectodermal tumor of infancy: A case report.

Melanotic neuroectodermal tumor of infancy (MNTI) is a rare distinctive neoplasm of infants with rapid expansile growth and a high rate of recurrence. Most commonly, the lesion affects the maxilla followed by the skull and orbit. One such case was diagnosed in a 5-month-old boy who presented with rapidly enlarged swelling over the back of the skull. Surgical excision was done. Typical histological features and immunohistochemical studies confirmed the diagnosis.

Invasive Fungal Granuloma of the Skull Base Mimicking Spindle Cell Neoplasm in a Young Immunocompetent Patient: A Diagnostic Challenge.

Skull base lesions presenting with proptosis and sinonasal involvement in young patients are commonly attributed to vascular neoplasms such as juvenile nasopharyngeal angiofibroma. However, rare infectious etiologies such as invasive fungal granulomas may present with overlapping radiological features, leading to diagnostic challenges. We report a rare case of a 19-year-old immunocompetent male presenting with progressive bilateral proptosis and a large skull base mass involving the clivus, nasal cavity, and orbits. Initial imaging suggested a vascular neoplasm, and biopsy revealed a spindle c

The importance of magnetic resonance imaging in the evaluation of vertigo and imbalance.

Vertigo and imbalance are believed to be rare manifestations of skull base neoplasms. Patients with skull base neoplasms can present with vague otolaryngological complaints, including diplopia, facial numbness, facial weakness, hearing loss, tinnitus, hoarseness, headache, and otalgia. Physical examination of these patients can sometimes reveal paralysis or paresis of cranial nerves. Magnetic resonance imaging (MRI) is the gold standard for evaluation of cranial nerve involvement in skull base diseases. Vertigo and imbalance can be manifestations of a neuropathy or lesion within the vestibular

[The Relevance of Abducent Nerve Palsy in ENT].

Patients suffering from abducent nerve palsy are usually primary seen by a conservative medical Specialist. In most cases the ENT specialist is secondary involved for treatment. In the majority of cases abducent nerve palsy is a temporary symptom associated with neurologic or vascular diseases. Rarely inflammation, neoplasm or fracture of the skull base cause this symptom and lead to an intervention done by the ENT surgeon. This case series describes retrospectively the abducent palsy seen through the eyes of an ENT surgeon. From 2008 to 2011 15 patients suffering from abducent nerve palsy. On

Novel 3-Dimensional Printed Lock-and-Key Cutting Guide Mechanism for Single-Stage Customized Reconstruction After Skull Tumor Resection.

BACKGROUND AND OBJECTIVES: Skull neoplasms that require surgical resection and alloplastic reconstruction often offer challenges for neurosurgeons in reconstruction. Single-stage reconstruction offers several main advantages and, in fact, alleviates patient burden. Custom cranial implants can be designed to match the anticipated defect, optimizing contour and esthetics. However, reported techniques still require intraoperative custom cranial implant resizing/reshaping. We report the outcomes of a cohort of patients with skull-involving neoplasms who underwent single-stage resection and reconst

Periampullary Carcinoma: Unusual Sites of Metastasis.

BACKGROUND: Periampullary neoplasms include carcinomas of the duodenum, ampulla of Vater, distal common bile duct, and pancreas. The aggressive course of these neoplasms is attributable to the delay in diagnosis, as patients have no symptoms until advanced stages of the disease. More than half of patients have distant metastasis at the time of diagnosis. The most frequent sites of metastasis are the liver, lymph nodes, peritoneum, lung, bone, kidney, and, rarely the skin. CASE REPORT: We report the case of a 45-year-old female patient with adenocarcinoma of the ampulla of Vater metastasizing

New developments in the pathology of skull base tumors.

As an anatomical interface between various tissues, the skull base harbors an exceptionally broad variety of neoplasms, some of which pose a major challenge for surgical pathology. The characterization of distinct immunohistochemical expression profiles and the identification of molecular genetic alterations associated with different tumor entities have significantly advanced this field. The new World Health Organization (WHO) classification of tumors of the nervous system lists 15 histopathological variants of meningioma. Of clinical importance are those entities that carry an increased risk

[Parapharyngeal space neoplasms].

OBJECTIVE: To study parapharyngeal space (PPS) neoplasms which are rarely seen clinically. METHODS: A retrospective review was made of 59 patients with PPS neoplasm operated on from 1986 to July, 1997 in our institution. RESULTS: Neurogenic tumors were the most common neoplasms, which constituted about 60 percent in the primary parapharyngeal space neoplasms. High-resolution computed tomography or magnetic resonance imaging was essential for diagnosis and presurgical planning. The transcervical approach was the preferred procedure for 75% benign parapharyngeal tumors. Middle mandibulotomy wa

Skull base chondrosarcoma: a case treated by the CyberKnife system.

Skull base chondrosarcomas represent 2% of all chondrosarcomas. They usually have a low grade of malignity and a complete resection may be curative. However, often they are considered irresectable because of the difficult approach and another kind of treatment is needed. We present a 54-year-old male case who, after weeks suffering from diplopia, was diagnosed with skull base chondrosarcoma and treated with the CyberKnife system. A review of radiosurgery techniques for skull base neoplasms is carried out.

Head and neck manifestations of plasma cell neoplasms.

Multiple myeloma, solitary plasmacytoma of bone, and extramedullary plasmacytoma are plasma cell neoplasms. They represent distinct manifestations of a disease continuum, whereby the clinical findings are critical to diagnosis. Plasma cell neoplasms are histologically similar, and distinguishing one from the other has significant implications for treatment and survival. Plasma cell neoplasms are relatively unusual malignancies of the head and neck region. We present a case series of plasma cell neoplasms involving the skull base, paranasal sinus, larynx, and mandible as an introduction to a co

Imaging of the Central Skull Base.

The skull base is a complex bony and soft tissue interface that is divided anatomically into compartments. This article will focus specifically on the central skull base, which has a complex embryologic development and anatomy. Multiple entities from notochord remnants, neoplasm, infection, and other abnormalities may occur, and imaging is critical for depicting skull base pathology.

[Dissection of the cranial base en bloc with the infratemporal fossa].

A high incidence of morbidity and mortality was related with skull-base neoplasm surgery. Several advances have permitted, in recent years, the total excision of such neoplasms with minimal patient morbidity. Due to an improved understanding of the surgical anatomy of the skull-base and to the collaboration of the neurosurgeon and maxillofacial surgeon and, moreover, to the improvements of imaging (CT and MR) in the past decades new combined approaches were planned and performed to allow en bloc resections of extensive lesions. Extensive exposure of the tumor, improved management of the intern
