Disorders of Arousal and timing of the first period of slow wave sleep: Clinical and forensic implications.

The timing of first period of slow wave sleep (SWS) is often used as a proxy for determining if and when Disorders of Arousal (DOA) such as sleepwalking are likely to occur or did occur in the past. In criminal cases employing a "sleepwalking defense" the prosecution may argue that nocturnal violence or sexually aggressive behavior occurred too early in the sleep period to be associated with SWS. Expert witness opinion on the expected latency to SWS (LSWS) has varied from minutes after sleep onset to ≥60 min. A search of PubMed was conducted for LSWS and for any reports of DOAs occurring from

Sleep-wake and arousal dysfunctions in post-traumatic stress disorder: Role of orexin systems.

Post-traumatic stress disorder (PTSD) is a trauma-related condition that produces distressing fear memory intrusions, avoidance behaviors, hyperarousal/startle, stress responses and insomnia. This review focuses on the importance of the orexin neural system as a novel mechanism related to the pathophysiology of PTSD. Orexinergic neurons originate in the lateral hypothalamus and project widely to key neurotransmitter systems, autonomic neurons, the hypothalamic-pituitary-adrenal (HPA) axis, and fear-related neural circuits. After trauma or stress, the basolateral amygdala (BLA) transmits sensor

Persistent mirror movement associated with disabling pain and sleep arousal disorder: a case report.

Persistent mirror movement (PMM) is a rare pathological condition characterised by involuntary synkinetic movements on one side of the body mirroring the volitional and intentional movements of another side. We present an unusual case of PMM associated with disabling pain, tiredness and headache. 24 years old girl with PMM suffered from pain in mirroring hand during performing fine intended motion by contralateral hand. Pain was so severe that she was not able to write, draw or play the guitar. Intensive physical activity and handiwork was leading her to tiredness and severe headache most alik

Arousal characteristics in patients with Parkinson's disease and isolated rapid eye movement sleep behavior disorder.

STUDY OBJECTIVES: Patients diagnosed with isolated rapid eye movement (REM) sleep behavior disorder (iRBD) and Parkinson's disease (PD) have altered sleep stability reflecting neurodegeneration in brainstem structures. We hypothesize that neurodegeneration alters the expression of cortical arousals in sleep. METHODS: We analyzed polysomnography data recorded from 88 healthy controls (HC), 22 iRBD patients, 82 de novo PD patients without RBD, and 32 with RBD (PD + RBD). These patients were also investigated at a 2-year follow-up. Arousals were analyzed using a previously validated automatic sy

Reliability and diagnostic accuracy of home video recording in differentiating sleep-related hypermotor epilepsy from disorders of arousal.

OBJECTIVE: This study evaluates the interobserver reliability (IR) and diagnostic accuracy of home video in distinguishing sleep-related hypermotor epilepsy (SHE) from disorders of arousal (DoA) among neurologists with varying levels of expertise. Additionally, it assesses the learning effect in nonexpert raters and extrapolates key diagnostic elements and confounding factors. METHODS: We selected 24 home videos capturing sleep-related events recognized as typical from 12 patients with confirmed SHE and 12 with DoA. During a full-day video session, a panel of 18 experts in epileptology or in

The ventilatory response to arousal from sleep is elevated in young individuals with post-traumatic stress disorder symptoms.

PURPOSE: Obstructive sleep apnea (OSA) is reported to be more prevalent in posttraumatic stress disorder (PTSD) compared to the general population for unknown reasons. Most prior studies have assessed military populations who commonly possess other OSA risk factors. This study aimed to determine whether: 1) OSA prevalence is increased in young adults with PTSD symptoms exposed to non-military traumas; and 2) the ventilatory response to arousal from sleep - an OSA pathogenic trait - differs according to PTSD symptoms. METHODS: Individuals with a range of PTSD symptoms (PTSD symptom checklist fo

Sleep-related cognitive processes, arousal, and emotion dysregulation in insomnia disorder: the role of insomnia-specific rumination.

OBJECTIVE: Insomnia-specific rumination has presented in subjects with insomnia. Research has identified hyperarousal as a key factor, with both trait and state components. It has been shown that emotion dysregulation also plays a role in insomnia. Hence, the aim was to investigate how insomnia rumination is associated with both trait- and state-dependent arousal and emotion dysregulation in insomnia. METHODS: Sixty-eight subjects with insomnia disorder (DSM-5) and 36 good sleepers were evaluated using: Insomnia Severity Index (ISI), Daytime Insomnia Symptom Response Scale (DISRS), Arousal Pr

Night terrors, sleepwalking, and confusional arousals in the general population: their frequency and relationship to other sleep and mental disorders.

BACKGROUND: Arousal parasomnias (night terrors, sleepwalking, and confusional arousals) have seldom been investigated in the adult general population. Clinical studies of parasomnias, however, show that these disorders may be indicators of underlying mental disorders and may have serious consequences. METHOD: A representative sample of the United Kingdom population (N = 4972) was interviewed by telephone with the Sleep-EVAL system. RESULTS: Night terrors were reported by 2.2% (95% CI = 1.8% to 2.6%) of the sample, sleepwalking by 2.0% (1.6% to 2.4%), and confusional arousals by 4.2% (3.6% to

Sex differences in blackout: evidence of a relationship between disorders of arousal during sleep and alcohol-related blackout in females.

RATIONALE: Disorders of arousal (DoA) constitute a class of related, heritable conditions that includes sleepwalking, sleep terrors, and confusional arousals. These disorders are defined by behavioral features which are shared with a separate phenomenon, alcohol-related blackout (ARB). Both ARB and DoA are characterized by full or partial amnesia and disinhibited behavior with maintenance of nearly normal motor function. While these disorders share phenomenology, to our knowledge no previous studies have examined the relationship between them. This is an important gap in the literature as the

Association between air pollutant exposure, body water distribution and sleep disorder indices in individuals with low-arousal-threshold obstructive sleep apnoea.

BACKGROUND: Air pollution may alter body water distribution, it may also be linked to low-arousal-threshold obstructive sleep apnoea (low-ArTH OSA). Here, we explored the mediation effects of air pollution on body water distribution and low-ArTH OSA manifestations. METHODS: In this retrospective study, we obtained sleep centre data from healthy participants and patients with low-ArTH OSA (N=1924) in northern Taiwan. Air pollutant exposure at different time intervals (1, 3, 6 and 12 months) was estimated using the nearest station estimation method, and government air-quality data were also obt

Nocturnal sleep-related scratching disorder as a possible variant of NREM arousal disorder: Clinical features and polysomnographic study of four newly reported cases.

A clinical variant of NREM arousal disorder is described manifesting with predominantly injurious, and exclusively sleep-related scratching without any conscious awareness, and without a history of dermatologic disease. Four patients were clinically evaluated and underwent videopolysomnography (vPSG). Case 1, a 30-year-old woman had a 20-yr history of non-injurious arm scratching/rubbing during sleep. vPSG: forearm rubbing against the chin during N2/N3. Two years later she developed restless bladder symptoms and leg movements before sleep onset. Serum ferritin level: 20.7 Intravenous iron ther

Nocturnal cognitive arousal is associated with objective sleep disturbance and indicators of physiologic hyperarousal in good sleepers and individuals with insomnia disorder.

BACKGROUND: Cognitive arousal is central to models of sleep disturbance and insomnia, but findings remain mixed regarding whether cognitive arousal is associated with objective sleep disturbance and physiologic hyperarousal. This study explored associations of objective nocturnal wakefulness and indicators of physiologic hyperarousal with cognitive arousal in healthy sleepers and individuals with insomnia. METHODS: In sum, 52 adults (51.9% women; 18 with insomnia disorder, 34 healthy sleepers) underwent two overnight polysomnography (PSG) studies (adaptation + baseline nights) and a multiple

Elevated gamma activity in left frontotemporal regions preceding sleep signals emotional arousal in Bipolar Disorders: Insights from a high-density EEG investigation.

BACKGROUND: While changes in sleep architecture during depression and mania are well-established, the extent to which they persist during euthymia in Bipolar Disorders (BD) remains unclear. Here we investigate pre-sleep cortical arousal and its correlation with sleep architecture and subjective sleep quality in BD patients. METHODS: Subjective sleep measures and whole-night, high-density sleep electroencephalography (EEG) recordings were obtained from 16 euthymic BD patients and 16 age and sex-matched healthy control subjects. Sleep architecture was determined according to standard guidelines

Violent and Complex Behaviors and Non-Restorative Sleep Are the Main Features of Disorders of Arousal in Adulthood: Real Picture or a More Severe Phenotype?

Disorders of arousal (DoA) are NREM parasomnias characterized by motor and emotional behaviors emerging from incomplete arousals from deep sleep. DoA are largely present in pediatric populations, a period during which they are labeled as self-limited manifestations. However, an extensive literature has shown that DoA can persist in adulthood, with different characteristics from childhood DoA. Adult DoA patients usually report excessive daily sleepiness, sleep-related violence during DoA episodes or potentially harmful behaviors, which are rare in childhood. The semeiological features of DoA ep

Bupropion SR reduces periodic limb movements associated with arousals from sleep in depressed patients with periodic limb movement disorder.

BACKGROUND: Antidepressant-induced periodic limb movement disorder (PLMD) may limit the tolerability of some antidepressant medications and interfere with treatment response. Given the role of dopamine in PLMD and the effects of bupropion sustained-release (SR) on central dopaminergic function, we hypothesized that bupropion SR would not be associated with antidepressant-induced PLMD. METHOD: In an expanded case-series design, we compared the effects of bupropion SR, after about 10 weeks of treatment, on measures of PLMD, depression, and sleep in 5 depressed (Research Diagnostic Criteria) pat
