Regulation of wakefulness by astrocytes in the lateral hypothalamus.

The lateral hypothalamus (LH) is an important brain region mediating sleep-wake behavior. Recent evidence has shown that astrocytes in central nervous system modulate the activity of adjacent neurons and participate in several physiological functions. However, the role of LH astrocytes in sleep-wake regulation remains unclear. Here, using synchronous recording of electroencephalogram/electromyogram in mice and calcium signals in LH astrocytes, we show that the activity of LH astrocytes is significantly increased during non-rapid eye movement (NREM) sleep-to-wake transitions and decreased durin

Electroencephalographic and autonomic alterations in subjects with frequent nightmares during pre-and post-REM periods.

Abnormal arousal processes, sympathetic influences, as well as wake-like alpha activity during sleep were reported as pathophysiological features of Nightmare Disorder. We hypothesized that in Nightmare Disorder, wake-like cortical activity and peripheral measures linked to arousals would be triggered by physiological processes related to the initiation of REM periods. Therefore, we examined electroencephalographic (EEG), motor and autonomous (cardiac) activity in a group of nightmare (NM) and healthy control (CTL) subjects during sleep-state-transitions while controlling for the confounding e

A physiologically based model of orexinergic stabilization of sleep and wake.

The orexinergic neurons of the lateral hypothalamus (Orx) are essential for regulating sleep-wake dynamics, and their loss causes narcolepsy, a disorder characterized by severe instability of sleep and wake states. However, the mechanisms through which Orx stabilize sleep and wake are not well understood. In this work, an explanation of the stabilizing effects of Orx is presented using a quantitative model of important physiological connections between Orx and the sleep-wake switch. In addition to Orx and the sleep-wake switch, which is composed of mutually inhibitory wake-active monoaminergic

Increased connectivity of the anterior cingulate cortex is associated with the tendency to awakening during N2 sleep in patients with insomnia disorder.

STUDY OBJECTIVES: To investigate the relationship between sleep transition dynamics and stage-specific functional connectivity (FC) of the anterior cingulate cortex (ACC) in patients with insomnia disorder (ID). METHODS: Simultaneous electroencephalography-functional magnetic resonance imaging (EEG-fMRI) data from 37 patients with ID and 30 well-matched healthy controls (HCs) were recorded during wakefulness and different sleep stages and subsequently analyzed. A Markov chain model was used to estimate the transition probability between each stage. The FC between the ACC (set as the seed) and

Interpretation of the adult polysomnogram.

Polysomnography provides information on the physiological changes occurring in many different organ systems in relation to sleep stages and wakefulness. It allows qualitative and quantitative documentation of abnormalities of sleep and wakefulness, of sleep-wake transition, and of physiological function of other organ systems that are influenced by sleep. Polysomnography is considered to be the "gold standard" for diagnosing sleep disordered breathing (SDB) and other sleep disorders; however, as with most other diagnostic tests, polysomnography is not ideal, but is rather the best available me

[Research progress of sleep-wake neural circuit mediated by hypothalamus].

The hypothalamus is a critical central hub for regulating sleep-wake patterns in organisms, maintaining the balance between daily activity and rest by modulating sleep-wake-related neural circuits. This article reviews research on hypothalamus-mediated neural circuits involved in sleep-wake regulation. Studies demonstrate that activating specific neuronal subtypes in the lateral hypothalamic area (LHA), paraventricular nucleus of the hypothalamus (PVH), supramammillary nucleus (SuM), preoptic area (POA) and its subregions, and dorsomedial hypothalamic nucleus (DMH) can induce sleep or wakefuln

Sleep problems and daytime somnolence in a German population-based sample of snoring school-aged children.

Habitual snoring is associated with daytime symptoms like tiredness and behavioral problems. Its association with sleep problems is unclear. We aimed to assess associations between habitual snoring and sleep problems in primary school children. The design was a population-based cross-sectional study with a nested cohort study. The setting was twenty-seven primary schools in the city of Hannover, Germany. Habitual snoring and sleep problems were assessed in primary school children using an extended version of Gozal's sleep-disordered breathing questionnaire (n = 1144). Approximately 1 year late

Control of non-REM sleep by ventrolateral medulla glutamatergic neurons projecting to the preoptic area.

Understanding the neural mechanisms underlying sleep state transitions is a fundamental goal of neurobiology and important for the development of new treatments for insomnia and other sleep disorders. Yet, brain circuits controlling this process remain poorly understood. Here we identify a population of sleep-active glutamatergic neurons in the ventrolateral medulla (VLM) that project to the preoptic area (POA), a prominent sleep-promoting region, in mice. Microendoscopic calcium imaging demonstrate that these VLM glutamatergic neurons display increased activity during the transitions from wak

Pelvic movements as rhythmic motor manifestation associated with restless legs syndrome.

Video-polysomnographic monitoring of a patient with a 4-year history of an unpleasant restless sensation originating in his lower abdomen showed stereotyped, repetitive, rhythmic pelvic body movements resembling coital behaviour at the wake-sleep transition. We discuss the association between restless legs syndrome and rhythmic movement disorder as a particular manifestation of a spectrum of rhythmic sleep-related movement disorders.

Parasomnias Occurring in Non-Rapid Eye Movement Sleep.

PURPOSE OF REVIEW: This article discusses the clinical manifestations, diagnosis and differential diagnosis, pathophysiology, and management of parasomnias occurring in non-rapid eye movement (REM) sleep. RECENT FINDINGS: Disorders of arousal are characterized by dissociated sleep, with wake and sleep phenomena intermingling, and local sleep, in which different areas of the brain exist simultaneously in different states of wakefulness or sleep. The frequency of arousals from slow-wave sleep with delta or mixed-frequency activity has a high sensitivity but relatively low specificity for the di

REM sleep disorder following general anesthesia in rats.

Postoperative sleep disorders, particularly the REM sleep disorder, may have a significant deleterious impact on postoperative outcomes and may contribute to the genesis of certain delayed postoperative complications. We have followed the effect of distinct anesthesia regimens (ketamine/diazepam vs. pentobarbital) over 6days following the induction of a stable anesthetized state in adult male Wistar rats, chronically instrumented for sleep recording. In order to compare the effect of both anesthetics in the physiological controls vs. the rats with impaired pedunculopontine tegmental nucleus (P

Behavioral state instability in orexin knock-out mice.

Narcolepsy is caused by a lack of orexin (hypocretin), but the physiologic process that underlies the sleepiness of narcolepsy is unknown. Using orexin knock-out (KO) mice as a model of narcolepsy, we critically tested the three leading hypotheses: poor circadian control of sleep and wakefulness, inadequate activation of arousal regions, or abnormal sleep homeostasis. Compared with wild-type (WT) littermates, orexin KO mice had essentially normal amounts of sleep and wake, but wake and non-rapid eye movement (NREM) bouts were very brief, with many more transitions between all behavioral states

Assessing the severity and improving the understanding of sleep-related breathing disorders in heart failure patients.

In this manuscript we present an overview of novel signal processing techniques developed by our group to reduce scoring time in the assessment of the severity of sleep-related breathing disorders in heart failure patients and to detect sleep/wake fluctuations during periodic breathing. Besides describing these methods, we present the results of validation experiments. Our work shows that novel signal processing techniques can reduce costs and resources needed to screen the patients and can provide relevant information for better understanding the role of wake/sleep transitions in the developm

The Future of Parasomnias.

Parasomnias are abnormal behaviours or mental experiences during sleep or the sleep-wake transition. As disorders of arousal (DOA) or REM sleep behaviour disorder (RBD) can be difficult to capture in the sleep laboratory and may need to be diagnosed in large communities, new home diagnostic devices are being developed, including actigraphy, EEG headbands, as well as 2D infrared and 3D time of flight home cameras (often with automatic analysis). Traditional video-polysomnographic diagnostic criteria for RBD and DOA are becoming more accurate, and deep learning methods are beginning to accuratel

Dyssomnias, parasomnias, and sleep disorders associated with medical and psychiatric diseases.

Sleep disorders can be intrinsic, as are insomnia or narcolepsy, or can be accounted for by external factors, such as noise, altitude, drug or alcohol abuse, or shift work. The arousal disorders, common in children, are usually benign and disappear by puberty. Sleep-wake transition disorders such as sleep starts are benign as well, and may occur at any age. The parasomnias comprise different entities such as nightmares, REM-sleep behavior disorder, sleep enuresis, and bruxism. Diagnosis and treatment often require a multidisciplinary approach. Virtually every psychiatric, neurologic, or medica

["Sleep attacks" in Parkinson patients. A side effect of nonergoline dopamine agonists or a class effect of dopamine agonists?].

Recently, sudden "sleep attacks" have been described in parkinsonian patients taking the nonergoline dopamine agonists pramipexole and ropinirole. Due to this possible side effect, patients must be instructed not to drive vehicles and to refrain from other activities carrying the risk of self-injury. However, the very existence of sleep attacks remains controversial in sleep medicine, since a gradual transition from wakefulness to sleep is normally observed. Accordingly, sudden onset of sleep, e.g., in narcolepsy or sleep apnea syndrome, is usually associated with excessive daytime sleepiness.

Deep profiling of nocturnal hypokinesia in multiple system atrophy and its clinical implications.

STUDY OBJECTIVES: Patients with multiple system atrophy (MSA) spend more time supine overnight. Assessing nocturnal hypokinesia (NH) in MSA via video polysomnography (VPSG) is limited in metrics and analysis depth. This study deconstructs NH by sleep-wake phases, develops new VPSG-based body transition parameters, and explores their utility in understanding NH's pathogenesis and therapeutic implications in MSA. METHODS: We retrospectively reviewed VPSG data of 150 participants - 58 MSA, 42 obstructive sleep apnea (OSA), and 50 controls. We analyzed the proportion of sleep in each body positio
