Living on the extreme margin: social exclusion of the transgender population (hijra) in Bangladesh.

The transgender people (hijra), who claim to be neither male nor female, are socially excluded in Bangladesh. This paper describes social exclusion of hijra [The term is used in this abstract both in singular and plural sense] focusing on the pathway between exclusion and sexual health. In an ethnographic study, 50 in-depth interviews with hijra, 20 key-informant interviews, and 10 focus-group discussions (FGDs), along with extensive field observations, were conducted. The findings revealed that hijra are located at the extreme margin of exclusion having no sociopolitical space where a hijra c

Emotional Distress in a Marginalized Population as a Function of Household-Level Social Determinants of Health.

Low-income, underrepresented communities of color are disproportionally affected by emotional distress. Little is known about malleable, household-level determinants of emotional distress, addressable by feasible, stigma-neutral interventions. The present study addressed this knowledge gap by analyzing secondary data from a cross-sectional community needs assessment survey in a marginalized urban community (N = 677). Relying on dominance analyses, authors found that, on average, the largest household-level contributions to respondents' emotional distress included exposures to fellow household

The prevalence of latent tuberculosis infection in social marginal groups. An experience conducted in Tuscany.

Tuberculosis is encountering a renewal attention due to emergent causes, linked primarily to migration from high-endemic countries of many subjects who present important risk factors as poverty, homelessness, drug abuse, HIV infection. The purpose of the present study is to evaluate the spread of Mycobacterium tuberculosis in social groups traditionally at higher risk of developing the disease and often difficult to detect. Tuberculin screening with Mantoux intradermal reaction has been carried out on 2014 subjects with one or more risk conditions. We have considered the following risk conditi

Factor structure and use of the hospital anxiety and depression scale in the homeless and socially marginalized.

The homeless have been identified as a group at risk of significant anxiety and depression and screening is therefore highly desirable to identify those needing psychological and psychiatric support. However, the psychometric properties of routine screening instruments have yet to be established in this group. This study sought to determine the psychometric properties of the widely used Hospital Anxiety and Depression Scale in this group. Using a cross-sectional design, 314 clients presenting at homelessness units (shelters) supported by The Salvation Army were administered the HADS. Confirmat

Too young to die: social inequalities and infant mortality in marginalized Roma communities in Slovakia.

BACKGROUND: In Slovakia, several tens of thousands of persons live in extremely poor living conditions in segregated Roma settlements. We can hardly find populations with such a short life expectancy and a high risk of death in infancy anywhere in Europe. The inadequate infrastructure, catastrophic housing conditions, deteriorated environmental quality, high unemployment and dependence on social transfers in combination with social and geographical segregation or negative behavioral aspects markedly affect their health status and mortality rates. Despite this, only little attention has hithert

Punitive school discipline as a mechanism of structural marginalization with implications for health inequity: A systematic review of quantitative studies in the health and social sciences literature.

Punitive school discipline deploys surveillance, exclusion, and corporal punishment to deter or account for perceived student misbehavior. Yet, education and legal scholarship suggests it fails to achieve stated goals and exacerbates harm. Furthermore, it is disproportionately imposed upon Black, Latinx, Native/Indigenous, LGBTQIA, and disabled students, concentrating its harms among marginalized young people. Its implications for health, however, are less clear. Using public health theories of sociostructural embodiment, we propose a framework characterizing pathways linking societal ideologi

Associations between deprived life circumstances, wellbeing and self-rated health in a socially marginalized population.

BACKGROUND: Previous studies of self-rated health among socially marginalized people provide insufficient understandings of what influences their self-rated health. This study aimed to examine how disadvantaged life circumstances (homelessness, substance abuse, poverty) and general well-being were associated with poor self-rated health among the socially marginalized. METHODS: In a nationwide survey in Denmark, 1348 users of shelters, drop-in centres, treatment centres and social psychiatric centres answered a self-administered questionnaire. We analysed data using logistic regression. RESUL

Social and structural factors associated with substance use within the support network of adults living in precarious housing in a socially marginalized neighborhood of Vancouver, Canada.

BACKGROUND: The structure of a social network as well as peer behaviours are thought to affect personal substance use. Where substance use may create health risks, understanding the contribution of social networks to substance use may be valuable for the design and implementation of harm reduction or other interventions. We examined the social support network of people living in precarious housing in a socially marginalized neighborhood of Vancouver, and analysed associations between social network structure, personal substance use, and supporters' substance use. METHODS: An ongoing, longitud

Unmet oral health needs and barriers to dental services among socially marginalized youth: a scoping review.

OBJECTIVES: Barriers limiting access to oral health significantly impact dental service utilization among socially marginalized youth, often resulting in unmet needs and poor oral health outcomes. Identifying and understanding these barriers is critical to inform the development of strategies to enhance oral healthcare access for this vulnerable population. This review examines the barriers restricting access to oral healthcare and unmet dental needs among socially marginalized youth worldwide, offering insight to guide the development of targeted interventions. METHODS: A comprehensive searc

High COPD prevalence among socially marginalized hospitalized patients. A pragmatic multicenter prospective cohort study.

BACKGROUND: Tobacco exposure is the main risk factor for chronic obstructive pulmonary disease (COPD), but other risk factors, including low socio-economic status (SES), may also contribute. Danish hospitals have introduced social nurses who support patients with complex health and social conditions. These nurses represent a unique point of contact for identifying hospitalized marginalized individuals who may not have undergone prior diagnostic evaluation for COPD. This study aimed to assess the prevalence of COPD among hospitalized marginalized individuals in contact with social nurses in Den

High rates of Mycobacterium tuberculosis among socially marginalized immigrants in low-incidence area, 1991-2010, Italy.

Migration from low- and middle-income countries to high-income countries increasingly determines the severity of tuberculosis (TB) cases in the adopted country. Socially marginalized groups, about whom little is known, may account for a reservoir of TB among the immigrant populations. We investigated the rates of and risk factors for Mycobacterium tuberculosis transmission, infection, and disease in a cohort of 27,358 socially marginalized immigrants who were systematically screened (1991-2010) in an area of Italy with low TB incidence. Overall TB and latent TB infection prevalence and annual

Digital Media for Social Justice Amidst Artificial Intelligence: Towards Equalizing Marginalized Media Access and Anti-Oppression.

This paper examines the relationship between intersectional marginalization, digital media, and anti-oppressive resistance. Grounded in Merriam's case study framework, the study prioritizes views of participants of their everyday media engagements, with emphasis on resulting community capacity to resist systemic social injustices. Utilizing purposive sampling, a cohort of 14 participants identifying with multiple intersecting social statuses of marginalization - including racialized identities, gender diversity, Indigenous ancestry, socioeconomic marginalization, and lived trauma - participate

"New barefoot Doctors": How social workers navigate HIV care and structural marginality in Yi ethnic minority communities.

Social workers play a critical yet underexamined role in HIV prevention and treatment in China's remote ethnic minority regions. Drawing on ethnographic fieldwork conducted between 2024 and 2025 in two Yi-autonomous mountain townships, this study examines how social workers navigate structural inequality and constraints within HIV care. This study identifies three interrelated agentic practices. First, women's healing workshops provide spaces that empower widowed women living with HIV through relational and cultural forms of support. Second, social workers advance drug assignment by advocating

A cohort study of the long-term outcome of latent tuberculosis infection among socially marginalized people in a low-incidence country.

BACKGROUND: Tuberculosis (TB) prevalence is high among socially marginalized citizens in Denmark, and management of latent TB infection (LTBI) may be part of preventing new cases. Patients with LTBI are offered either preventive treatment (TPT) or follow-up chest x-rays, but knowledge about the long-term outcome in terms of active TB is sparse. METHODS: We performed a retrospective cohort study investigating the long-term outcomes for socially marginalized citizens who were diagnosed with LTBI or who had a positive interferon-gamma release assay (IGRA) but were lost to follow-up. Six-month outcomes among socially marginalized alcohol and drug users attending a drop-in center allowing alcohol consumption.

BACKGROUND: Despite their experience of substance-related harm, few socially marginalized alcohol and other drug (AOD) users access substance use treatment. Thus, identifying alternative approaches for this population is important. This program evaluation documented substance use and health-related quality of life (QoL) following exposure to such an alternative approach: a harm-reduction drop-in center allowing alcohol consumption onsite. METHODS: Participants (N=85) were socially marginalized AOD users (e.g., alcohol, heroin) attending a harm-reduction drop-in center in the French-speaking p
