[Sphenopalatine ganglion block : a promising approach for headache and supratentorial intracranial surgery ?].

We aimed to determine whether sphenopalatine ganglion block (SPGB) had a place as a treatment for headache, as well as its place as an anterior component of scalp block for supratentorial intracranial surgery. Using the keywords «sphenopalatine ganglion», «sphenopalatine ganglion block», «pain», «surgery» and «craniotomy» in PubMed and Google Scholar, we identified 14 relevant articles. The methods used to perform the block and the treatment of control groups differed between studies. The main outcomes studied were reduction in pain scores and hemodynamic variations during supratentorial intra

Transnasal sphenopalatine ganglion block for post-dural puncture headache and associated tinnitus.

Lumbar epidurals are frequently inserted for women in labour as they provide excellent analgesia. One of the more common procedural complications is post-dural puncture headache which can be associated with auditory symptoms such as hearing loss and tinnitus and can be treated with an epidural blood patch. Sphenopalatine ganglion blocks have also been used to treat post-dural puncture headache but have not been previously shown to resolve the associated tinnitus. We report a case where postural neck pain and tinnitus from an accidental dural puncture during lumbar epidural insertion for labour

[Self-applied sphenopalatine ganglion block for postdural puncture headache: four case reports].

BACKGROUND AND OBJECTIVES: The Sphenopalatine Ganglion Block (SGB) is an effective, low-risk treatment option for Postdural Puncture Headache (PDPH) refractory to conservative management. CASE REPORT: This report presents four complex cases of patients with headache related to low cerebrospinal fluid pressure. Three of them were successfully treated with the application of local anesthetic topical drops through the nasal cavity. CONCLUSION: The novel approach described in this report has minimal risks of discomfort or injury to the nasal mucosa. It is quick to apply and can be administered b

Sphenopalatine ganglion block in primary headaches: An American Headache Society member survey.

BACKGROUND: The sphenopalatine ganglion (SPG), in the pterygopalatine fossa, is a known current and historical target for therapeutic intervention in headache disorders because of its role in cranial autonomics and vasodilation. There remains an overall lack of well-established SPG treatment protocols, particularly with the advent of newer commercial devices. METHODS: A 22 multiple-choice question survey was created to evaluate clinical practice patterns with SPG block and sent to members of the American Headache Society (AHS). Questions focused on determining indications, preferred applicato

Use of the Sphenopalatine Ganglion Block to Treat Migraine Headaches in the Emergency Department.

Headaches are a common presenting complaint to the emergency department. Amongst the common non-life-threatening headaches, migraine headaches tend to be one of the more severe. Commonly, migraines are treated with the so-called "migraine cocktail." At our facility, this cocktail is usually IV fluids, metoclopramide, diphenhydramine, and ketorolac. The patient is then left in a quiet, dark room for two to three hours, then reassessed for symptom improvement or if more medications are required. Recently we have begun to employ the sphenopalatine ganglion (SPG) block as a rapid, and easy-to-admi

Sphenopalatine Ganglion Nerve Block for the Treatment of Migraine Headaches in the Pediatric Population.

BACKGROUND: Persistent headaches and migraines are common in pediatrics with various treatment options. The sphenopalatine ganglion (SPG) has been identified as communicating with the parasympathetic autonomic nervous system and pain receptors. In adults, SPG block is an established treatment but there is no published literature in pediatrics. OBJECTIVES: The purpose of this study is to analyze the SPG block in pediatrics. STUDY DESIGN: Retrospective, single-center study. SETTING: This study was conducted at Phoenix Children's Hospital in Phoenix, Arizona. METHODS: A comprehensive review o

Sphenopalatine ganglion block: a safe and easy method for the management of orofacial pain.

The sphenopalatine ganglion (SPG) block is a safe, easy method for the control of acute or chronic pain in any pain management office. It takes only a few moments to implement, and the patient can be safely taught to effectively perform this pain control procedure at home with good expectations and results. Indications for the SPG blocks include pain of musculoskeletal origin, vascular origin and neurogenic origin. It has been used effectively in the management of temporomandibular joint (TMJ) pain, cluster headaches, tic douloureux, dysmenorrhea, trigeminal neuralgia, bronchospasm and chronic

Sphenopalatine Ganglion Block: Treatment of Migraine and Trigeminal Neuralgia Associated With Multiple Sclerosis.

Head and facial pain are a burden to many people both directly and indirectly. This is manifested not only as a personal burden but also as a financial one in the form of sick leaves from work and loss of workplace productivity. These costs stem from emergency department visits, hospitalizations, preventative treatments, and medical management. Medical management of migraine headaches and other causes of facial pain often proves insufficient, and sphenopalatine ganglion block (SPGB) provides an innovative, adjunctive outpatient treatment option with excellent results in alleviating symptoms. Sphenopalatine Ganglion Block for Headache Treatment After an Incidental Durotomy: A Case Report.

Incidental durotomy is the most common intraoperative complication of spine surgeries. Our main goal is to report a case of a postoperative postdural puncture headache following an incidental durotomy successfully managed with a sphenopalatine ganglion block. A 75-year-old woman, American Society of Anesthesiologists physical status II, proposed for a lumbar interbody fusion. During surgery, an incidental durotomy with cerebrospinal fluid leak occurred, being repaired with muscle and DuraSeal® Dural Sealant System. In the recovery room, 1 hour after the end of the surgery, the patient develope

Sphenopalatine ganglion block ... a new treatment for burning mouth syndrome?: a case report.

Burning mouth syndrome is a poorly understood entity for which current treatment modalities fail to provide effective relieve. Branches of the maxillary and mandibular nerves are responsible for the innervation of the affected area. These are also the nerves involved in trigeminal neuralgia, an entity where sphenopalatine block has proved to be effective. We present a case of a patient with burning mouth syndrome in whom a bilateral sphenopalatine ganglion block was successfully performed for pain treatment. It is an easy and safe technique that can be a valuable treatment option for these pat

Anatomically and physiologically based guidelines for use of the sphenopalatine ganglion block versus the stellate ganglion block to reduce atypical facial pain.

This literature review is designed to develop guidelines needed for the use of a sphenopalatine ganglion block versus a stellate ganglion block to reduce atypical facial pain. We have reviewed the basic anatomy of both ganglia and the physiological responses usually associated with each, and have given an opinion on appropriate use of these therapeutic modalities.

Effect of Scalp Block vs Sphenopalatine Ganglion Block with Posterior Occipital Nerve Block on Hemodynamic Response Following Skull Pin Application.

BACKGROUND: Sphenopalatine ganglion (SPG) block combined with occipital nerve block can attenuate the hemodynamic response to the painful stimulus of skull pin application. MATERIALS AND METHODS: About 60 patients, aged 18-65 years, were randomly assigned to two groups. All patients were classified as American Society of Anesthesiologists (ASA) grades I and II, had a Glasgow Coma Scale (GCS) score of 15/15, and were scheduled for elective craniotomy. Group S was given a scalp block with 0.25% bupivacaine, while group SPG was given a bilateral transnasal SPG block with 0.5% bupivacaine, along

The Efficacy of Stellate Ganglion and Sphenopalatine Ganglion Blocks in Management of a Rare Case of Glossodynia: Burning Mouth Syndrome.

Burning mouth syndrome (BMS) is a debilitating intraoral burning or dysesthesia sensation without any evident causative lesions on clinical examination and investigations. The etiopathogenesis of BMS remains uncertain, with proposed mechanisms including psychological, peripheral neuropathic, and central neuropathological factors. Management of BMS is generally symptomatic and often met with poor response. Published data on the successful pain management interventions are inadequate. This report describes the successful management of a 43-year-old patient with severe glossodynia, diagnosed as B

Sphenopalatine Ganglion Block for Postdural Puncture Headache in the Obstetric Population: A Narrative Review.

Postdural puncture headache (PDPH) frequently complicates neuraxial anesthesia, delaying patient recovery and reducing quality of life after surgery. Epidural blood patch (EBP), the conventional standard of care, is effective but not without risks. Therefore, safer, less invasive alternatives are of increasing interest. The sphenopalatine ganglion block (SPGB) is a minimally invasive regional analgesic technique that has drawn attention in recent years for managing PDPH, largely because it is non‑opioid and generally well tolerated. In this narrative review, we summarize current knowledge on t
