[Imaging of demyelinating and neoplastic diseases of the spinal cord].

The clinical symptoms of myelopathy are variable and non-specific. Demyelinating as well as neoplastic spinal cord diseases can cause paresthesia, progressive sensomotoric deficits and bowel and bladder dysfunction. Imaging of the spine, especially with magnetic resonance imaging (MRI), is an essential component in the diagnostic assessment of myelopathy and makes a substantial contribution to achieving the correct diagnosis. Although intramedullary neoplasms are far less common than demyelinating spinal cord diseases, radiologists should be familiar with the three most common entities, astroc

Intradural spinal cord lesions: Gd-DTPA-enhanced MR imaging.

Twenty-five patients with suspected spinal cord neoplasms were studied with high-field-strength magnetic resonance (MR) imaging (1.5 T) before and after administration of gadolinium diethylenetriamine-pentaacetic acid (DTPA) (gadopentetate dimeglumine). Five patients had enhancing, nonneoplastic lesions, including spinal dural arteriovenous fistulas (AVFs), cord infarction, and chronic arachnoiditis. Fifteen patients had proved spinal cord neoplasms, 13 intramedullary and two extramedullary. Four of the intramedullary tumors were detected only after Gd-DTPA administration; in five others, cont

[Magnetic resonance imaging in spinal cord diseases: 127 cases].

Acute or chronic spinal cord syndromes with or without evidence of cord compression are a common diagnostic problem in neurology. Magnetic resonance imaging (MRI) was performed on 127 patients to assess the sensitivity of this examination in detecting local spinal cord lesions. The most common location noticed in this study were the cervico-thoracic (40.9%) and cervical regions (33.8%). A total spine examination accounted for 14.9% of the examinations performed, and the lumbar spine was evaluated in 10.2%. An increased signal corresponding to plaques of multiple sclerosis was the most frequent

Solitary fibrous tumour of cervical spinal cord.

Solitary fibrous tumors (SFTs) are rare mesenchymal neoplasms of fibroblastic origin. They commonly arise from visceral pleura, but also arise from nonserosal sites such as meninges, central nervous system parenchyma, and spinal cord. In the spinal cord, SFTs commonly arise from the thoracic spinal cord, followed by cervical spinal cord, lumbar spinal cord, and sacrum. Histologically, SFTs can be similar to hemangiopericytoma, schwannoma, fibrous meningioma, fibroma, gliofibroma, and ependymoma. Immunohistochemistry (IHC) plays an important role in differentiating SFTs from other identical tum

Non-enhancing pilocytic astrocytoma of the spinal cord.

Pilocytic astrocytomas are among the most common intramedullary spinal cord tumors in the pediatric age group. The presence of contrast enhancement is a major factor used to distinguish these tumors from other spinal cord lesions. We present a case of histologically proved non-enhancing intramedullary spinal cord pilocytic astrocytoma in a 12-year-old girl. This case represents an exception to the conventional wisdom that pediatric spinal neoplasms enhance with administration of intravenous contrast material.

A Pain in the Neck - A Case of Intramedullary Spinal Ependymoma.

Spinal cord tumors are sporadic and account for around 2%-4% of central nervous system neoplasms. Ependymoma is one of the most common spinal cord neoplasms and can present with different neurological signs and symptoms. They commonly present with neck or back pain and associated neurological involvement, with sensory symptoms usually antedating the motor symptoms. We now present a rare case of spinal ependymoma in a 26-year-old female who presented with isolated neck pain and the absence of other neurological symptoms. Attention to peculiar characteristics like isolated neck pain might be an

Primary germinoma of the spinal cord: a case report with 28-year follow-up and review of the literature.

Germ cell neoplasms occur in extra-gonadal midline locations of the retroperitoneum, mediastinum, pineal gland, areas of the suprasellar cistern, and rarely in the spinal cord. We recently reviewed a case of an unresectable lumbar spinal cord tumor in a 16-year-old female previously diagnosed as "metastatic poorly differentiated carcinoma." An extensive evaluation for a primary neoplasm at that time was unsuccessful and the patient was treated with local radiation therapy. Recently, additional histochemical and immunocytochemical studies were performed on the archival formalin-fixed paraffin-e

Neonatal Spinal Cord Injury.

Neonatal spinal cord injury is rare, and diagnosis is often delayed. The causes of spinal cord injury are quite broad including mechanical trauma, ischemia in the prenatal or perinatal period, iatrogenic causes, or other etiologies such as spinal abnormalities or neoplasms. The perinatal history and specific physical examination findings can suggest a diagnosis of neonatal spinal cord injury, which warrants confirmation by magnetic resonance imaging. Management involves supportive care and physical and occupational therapy. Early recognition is paramount to determine the specific cause, manage

How I do it: cervical hemangioblastoma resection. Surgical technique and complication avoidance.

BACKGROUND: Spinal cord hemangioblastomas are benign, highly vascular neoplasms that affect the brain and, rarely, the spinal cord. They can be solitary or as part of von Hippel-Lindau syndrome. Radiosurgery is not a suitable treatment option. Endovascular embolization can only be adjunct to surgery. METHOD: We present a detailed approach to resection of a spinal cord hemangioblastoma. A video demonstrates the microsurgical technique and discusses complication avoidance. CONCLUSION: The pitfalls to consider are preservation of normal spinal cord vessels, protection of the pia-arachnoid cleav

Spinal meningeal oligodendrogliomatosis in two boxer dogs.

Two Boxer dogs developed progressive ataxia in association with a neoplastic infiltration of the spinal leptomeninges. In the first dog, the leptomeningeal neoplasm encompassed the entire cord and the ventral aspect of the brainstem and extended bilaterally into the piriform lobes. In the second, the neoplasm surrounded the C1-C3 segments of the spinal cord and the brainstem without involvement of the brain or spinal cord parenchyma. In both dogs, the neoplastic cells had variably distinct cell borders, clear to eosinophilic cytoplasm, and a round to ovoid hyperchromatic nucleus. Neoplastic ce

Spinal cord biopsy: a review of 38 cases.

OBJECTIVE: Neurosurgeons are frequently asked to evaluate patients for spinal cord biopsies when preoperative magnetic resonance imaging studies demonstrate nonspecific features. These lesions often appear unresectable, but surgeons must decide whether a biopsy is warranted. To determine the best approach to these cases, we evaluated the clinicopathological findings for patients with unknown spinal cord lesions who underwent spinal cord biopsies. METHODS: Thirty-eight consecutive patients who underwent spinal cord biopsies at the Mayo Clinic (Rochester, MN) between August 1988 and July 1998 w

Pediatric Spinal Vascular Abnormalities: Overview, Diagnosis, and Management.

Hemangioblastomas are true benign vascular neoplasms arising from pluripotent mesenchymal stem cells that give rise to vascular endothelial cells and are most commonly found in the cerebellum, spinal cord, brainstem, and retina. These tumors may be isolated sporadic lesions or may be associated with hereditary genetic factors in the case of von Hippel-Lindau (VHL) syndrome. Spinal cord haemangioblastomas constitute 1.1% to 2.4% of all central nervous system tumors105, with the majority being single tumors that present in the fourth decade of life 106. In the pediatric population, sporadic spin

Brown-Sequard syndrome associated with Horner syndrome following cervical disc herniation.

INTRODUCTION: Brown-Sequard syndrome (BSS) has been reported in patients with various spinal pathologies, including spinal traumatic injuries, spinal cord neoplasms, epidural hematomas and spinal cord ischemia. Pure BSS caused by cervical disc herniation is very rare. CASE PRESENTATION: We report a rare case of cervical disc herniation presenting as BSS associated with Horner syndrome (HS), which has not been reported up to now. A prompt diagnosis by magnetic resonance imaging (MRI), followed by spinal cord decompression was performed. A postoperative rapid improvement of the neurological def

Imaging diagnosis--spinal cord histiocytic sarcoma in a dog.

A 12-year-old mixed breed dog was presented for evaluation of progressive paraparesis and ataxia. Magnetic resonance (MR) imaging was performed and identified multifocal intradural spinal cord mass lesions. The lesions were hyperintense in T2-weighted sequences, isointense to mildly hyperintense in T1-weighted sequences with strong contrast enhancement of the intradural lesions and spinal cord meninges. Spinal cord neoplasia was suspected. A diagnosis of intramedullary spinal cord histiocytic sarcoma, confined to the central nervous system, was confirmed histopathologically. Spinal cord histio

Imaging of intramedullary tumours of the spinal cord.

Intramedullary tumours (IMTs) are the least common neoplasms of the spinal canal. The majority of them are ependymomas and astrocytomas, the third commonest is haemangioblastoma, while other tumours of the spinal cord are relatively rare. This review presents on update on the imaging of spinal cord tumours. Magnetic resonance imaging (MRI) is the imaging method of choice in diagnosing IMTs, with other modalities playing a supplementary role. The authors discuss the MRI protocol in IMTs including advanced techniques and present the imaging features of particular tumours. The differentiation of

Primary tumors of the spine.

Spinal tumors consist of a large spectrum of various histologic entities. Multiple spinal lesions frequently represent known metastatic disease or lymphoproliferative disease. In solitary lesions primary neoplasms of the spine should be considered. Primary spinal tumors may arise from the spinal cord, the surrounding leptomeninges, or the extradural soft tissues and bony structures. A wide variety of benign neoplasms can involve the spine including enostosis, osteoid osteoma, osteoblastoma, aneurysmal bone cyst, giant cell tumor, and osteochondroma. Common malignant primary neoplasms are chord

[Isolated cervical spinal cord hemangioblastoma--case report].

Hemangioblastomas are rare tumors which account for 0.9-2.1% of central nervous system neoplasms. The most common site of hemangioblastomas is the cerebellum, while they are rarely located in spinal cord, cerebrum and brain stem. Hemangioblastomas occur as a sporadic entity, and as a manifestation of von Hippel-Lindau syndrome. A 33-year old patient with isolated cervical spinal cord hemangioblastoma is presented.

Spinal Infections.

Spinal cord infections can present with a wide variety of imaging findings, depending on the pathogen and the host's immune status. Infectious myelitis can have a characteristic distribution of lesions within the spinal cord, which refine the differential disease. Some spinal infections do not show typical imaging features, and many noninfectious may mimic spinal infections with similar MR imaging findings. Infectious arachnoiditis and meningitis must be differentiated from neoplasms. Spondylitis has many mimickers and requires careful interpretations of images, clinical findings, and follow-u

Chronic Pain Secondary to a Cervical Intramedullary Ependymoma: A Case Report.

Intramedullary spinal cord tumors (IMSCT) are a rare subset of neoplasms classified based on anatomical location. The most common presenting symptom is pain; however, the high prevalence of back pain in the general public secondary to common causes including degenerative disc disease or osteoarthritis, makes diagnosing spinal cord tumors a challenge. We present a case of a 43-year-old male with a cervical intramedullary ependymoma to discuss the clinical presentation, diagnosis, and treatment of these spinal tumors.
