INTRODUCTION: Chronic pain after spinal surgery (CPSS), formerly known as failed back surgery syndrome, encompasses a variety of highly incapacitating chronic pain syndromes emerging after spinal surgery. The intractability of CPSS makes objective parameters that could aid classification and treatment essential. In this study, we investigated the use of cerebral diffusion-weighted magnetic resonance imaging.
METHODS: Cerebral 3T diffusion-weighted (DW-) MRI data from adult CPSS patients were assessed and compared with those of healthy controls matched by age and gender. Only imaging data with

Substance P, acting through neurokinin I receptors, is involved in the processing of nociceptive information in the spinal cord. Sensitization of spinothalamic tract neurons occurs to low-intensity stimuli following capsaicin injection. The current study tested the effects of the novel neurokinin I receptor antagonist, SDZ NKT 343, on the sensitization of spinothalamic tract cells by capsaicin in monkeys. Spinothalamic tract cells from the lumbar enlargement with receptive fields in the hindpaw were isolated and recorded before and after intradermal injection of capsaicin. The background activ

The responses of 25 spinothalamic tract (STT) neurons to mechanical and thermal stimulation of the skin, as well as to a battery of iontophoretically applied excitatory amino acids (EAAs), were tested before and then during microdialysis of substance P (SP) or neurokinin A (NKA) into the dorsal horn of anesthetized monkeys. Neither peptide had significant effects on the background activity or the responses to mechanical or thermal stimulation of the skin. However, each peptide produced significant increases in the responses to iontophoretic application of one or more EAAs. In addition, followi

BACKGROUND: The primary objective of neurosurgical intervention for central nervous system tumors is a balance between optimal tumor resection and minimal postoperative neurological impairment. Intraoperative neurophysiological monitoring is essential in safeguarding somatosensory tracts during surgery. Virtually the only tool for continuous monitoring of sensory pathways is registration of peripheral somatosensory evoked potentials (SSEPs). However, this method has some limitations.
OBJECTIVE: To increase the quality of intraoperative SSEP monitoring in surgical treatment of brainstem and sp

1. Spinothalamic tract (STT) neurons in the C3-T6 spinal segments were studied for their responses to stimulation of phrenic and cardiopulmonary spinal afferent fibers. A total of 142 STT neurons were studied in 44 anesthetized, paralyzed monkeys (Macaca fascicularis). All neurons were antidromically activated from the ventroposterolateral nucleus and/or medial thalamus. 2. Electrical stimulation of phrenic afferent fibers (PHR) excited 43/58 (74%), inhibited 2/58 (3%), and did not affect 13/58 (13%) of cervical STT neurons. Neurons with excitatory somatic fields confined to the proximal limb

The surgical management of brainstem tumors and cavernous malformations represents one of the most demanding challenges in neurosurgery, given the critical concentration of eloquent neural structures in this region. These include nuclei, cranial nerve roots and their corticonuclear tracts, corticospinal and spinothalamic pathways. Intraoperative injury to these critical structures may result in permanent neurological deficits, including progressive sensory deterioration during the postoperative course. Therefore, continuous monitoring of the functional state of the spinothalamic tracts during

AIMS: A patient developed delayed-onset central pain due to degeneration of the spinothalamic tract resulting from degeneration of the ischemic transcallosal fibers, in the aftermath of a corpus callosum (CC) hemorrhage. The detection and diagnosis of these lesions using diffusion tensor tractography are described.
METHODS: A 59-yr-old man underwent conservative management for hemorrhages in the CC and lateral ventricle that resulted from hitting his head against a wall while falling. He began to feel pain in his right upper and lower extremities approximately 1½ yrs after the initial injury.

Two patients are reported on who experienced loss of pain and temperature sensation in the entire contralateral hemibody but sparing the ispsilateral face (pure sensory stroke pattern) related to acute lateral medullary infarction. In both patients MRI showed a notch-like retro-olivary ischaemic lesion in the ventromedial tegmentum with preservation of the far lateral medulla. The mediolateral lesion involved the crossed lateral spinothalamic tract and the ventral trigeminothalamic tract, corresponding to sensory loss in the contralateral face, arm, and upper trunk. The ventrolateral extension

BACKGROUND: Although the pathogenesis of central poststroke pain (CPSP) remains obscure, interactions between the spinothalamic tract and lemniscal sensory tract may be involved in its pathogenic mechanism.
CASE DESCRIPTION: A patient developed lateral medullary infarction, which produced contralateral spinothalamic sensory deficits and subsequent CPSP. The CPSP gradually improved until the development of ipsilateral medial medullary infarction 26 months later, which was associated with mild hemiparesis and lemniscal sensory deficits. The ipsilateral medial medullary infarction immediately ag

Monoamine systems have been shown to be an important part of an endogenous analgesic system of the central nervous system. Some aspects of the anatomical basis of monoamine modulation of nociceptive input were investigated in these studies. Two sites examined where monoamine systems are known to impinge on the pain transmission system included the grey matter of the somatosensory thalamus and the spinal cord. In particular, the connections of noradrenergic systems with these regions were emphasized. In the ventral posterolateral nucleus of the thalamus the presence of a sparse innervation by b

BACKGROUND: Magnetic resonance imaging can provide a preoperative diagnosis of pontine glioma, but the findings sometimes do not correspond with the clinical symptoms. We describe 2 cases of pontine gliomas who did not present with motor and sensory disturbance.
CASE REPORT: Three-dimensional anisotropy contrast (3DAC) imaging was used to assess the neuronal tracts in 2 patients with pontine gliomas. Conventional MR imaging depicted markedly abnormal findings of abnormally high or heterogeneous signal intensity in the pons in 2 cases. In contrast, 3DAC imaging obviously showed the corticospin

The midbrain represents the uppermost portion of the brainstem, containing numerous important nuclei and white matter tracts, most of which are involved in motor control, as well as the auditory and visual pathways. Notable midbrain nuclei include the superior and inferior colliculus nuclei, red nucleus, substantia nigra, oculomotor nuclear complex, and trochlear nucleus. In addition, white matter tracts include the brachium conjunctivum, medial and lateral lemniscus, spinothalamic tracts, and the fiber tracts within the cerebral peduncles. Although neurologically vital, many of these small mi

The ability to localize the three spinal tracts (corticospinal tract, spinothalamic tract, and dorsal [posterior] columns) involved in incomplete spinal cord syndromes at cross-sectional imaging and knowledge of the classic clinical manifestations of the various syndromes enable optimized imaging evaluation and provide clinicians with information that aids in diagnosis and treatment. The requisite knowledge for localizing these tracts is outlined. The authors review the spinal cord anatomy, blood supply, and course of these tracts and describe the various associated syndromes: specifically, do

The purpose of this study was to examine an excised human brainstem with high-resolution MR imaging. The MR images were correlated with myelin-stained histologic specimens and standard anatomic descriptions. Structures visualized included nuclei and/or fibers of all the cranial nerves, with the exception of the trochlear nerve. The topography of the brainstem is discussed. Major tracts such as the corticospinal spinothalamic, spinocerebellar, medial and lateral lemniscus, and other ascending and descending tracts and intrinsic brainstem nuclei such as the olivary complex, pontine nuclei, red n

OBJECTIVES: Focal spinal cord lesions occur across a variety of neurological diseases including non-traumatic cervical myelopathies which can lead to neuropathic pain. Currently, the degree to which signs and symptoms of neuropathic pain correlate with functional impairments and/or anatomical deficits remains unclear. This study aimed to identify structural and functional determinants associated with neuropathic pain in individuals with focal spinal lesions.
METHODS: Individuals fulfilling the diagnostic criteria of neuropathic pain were identified among individuals with cervical myelopathy p

Anterior cord syndrome (ACS) occurs as a result of ischemia in the territory of the anterior spinal artery (ASA). Although spinal cord strokes are rare, the ASA is the most commonly affected vessel in the spinal cord. The typical presentation of an ASA stroke is paraparesis or paraplegia, bilateral loss of pain and temperature sensation, and fecal or urinary incontinence; the underlying neural structures responsible for these symptoms include the corticospinal tracts and anterior horns, anterolateral spinothalamic tracts, and lateral horns, respectively. ACS is a feared complication of aortic

OBJECTIVES: Restricted sensory syndrome provides an excellent chance for the understanding of neuroanatomic correlation. The trigeminal nerve has been shown to convey buccal sensory impulse to central compartment. However, the pathway between cortex and the trigeminal sensory nucleus remains largely unknown.
METHOD: A patient presented with cheirobuccopedal syndrome, or objective sensory impairment confined to the left intraoral cheek, hand and foot, was reported.
RESULTS: Decrease in pinprick pain and fine touch sensation was detected at the left intraoral cheek, hand and foot. A recent inf

OBJECTIVE: Degenerative cervical myelopathy (DCM) is a centromedullary spinal cord disorder mainly affecting crossing fibers. While contact heat evoked potentials (CHEPs) are sensitive in detecting DCM by testing spinothalamic integrity, somatosensory evoked potentials (dSSEPs) show unaffected dorsal column conduction. Intra-epidermal electrically evoked potentials (IEEPs) have unknown spinal propagation after noxious stimulation. We investigated (1) the spinothalamic tract propagation and (2) the discriminative power in detecting spinal pathology of IEEPs compared to CHEPs and dSSEPs in DCM.

During pathological examinations of amyotrophic lateral sclerosis (ALS) patients, we noticed that bundles of dense small myelinated fibers were relatively well preserved in the anterior marginal areas of the cervical cords in a few ALS patients with diffuse myelin pallor. Moreover, these fibers were strongly immunostained by anti-parvalbumin antiserum. These parvalbumin-positive fiber bundles were also seen in other ALS and non-ALS patients, and they were continuously traced up to the spinothalamic tract areas of the medulla oblongata. Therefore, it is suggested that these fiber bundles belong
