Acoustic stapedius reflex function in man revisited.

OBJECTIVE: The objective of this study was to examine the role of the acoustic stapedius reflex in the protection of speech recognition from the upward spread of masking arising from low-frequency background noise. DESIGN: Speech recognition scores were measured for nine control participants (19-34 years) and six patients with transected stapedius tendons poststapedotomy (39-57 years) as a function of the amplitude of a low-frequency masker, presented at nominal signal to noise ratios of +5 dB, -5 dB, and -15 dB. All participants had pure-tone hearing thresholds in the normal range. Continuou

[Isolated stapedius tendon ossification: a case report].

In this study, a case with bilateral isolated stapedius tendon ossification was reported, since it is a rare clinical condition. A 46-year-old female patient was admitted to our clinic with the complaint of bilateral hearing loss. Tympanic membrane was bilateral normal in color and appearance on otomicroscopic examination. Pure tone air bone gap was between 20-25 dB at 1 kHz and 4 kHz on audiogram bilaterally. Middle ear pressure was normal and stapedius reflex was negative bilaterally. Preoperative diagnosis was otosclerosis. Right exploratory tympanotomy was performed. Isolated stapedius ten

Diverse synaptic terminals on rat stapedius motoneurons.

Stapedius motoneurons (SMN) mediate the contraction of the stapedius muscle, which protects the inner ear from injury and reduces the masking effects of background noise. A variety of inputs to SMNs are known to exist, but their terminal ultrastructure has not been investigated. We characterized the synaptic terminals on retrogradely labeled SMNs found just ventromedial to the facial motor nucleus. About 80% of the terminals contained round synaptic vesicles. One type (Sm Rnd) had small, round vesicles filling the terminal with occasional dense core vesicles and formed an asymmetric synapse. S

Absence of both stapedius tendon and muscle.

During surgery for otosclerosis, it is common for the surgeon to cut the stapedius tendon. The absence of the stapedius muscle with its tendon is uncommon. In this study, we present a case of the absence of the unilateral stapedius tendon and muscle. During dissections of adult temporal bones, the absence of the stapedius tendon and muscle was found in one case. The tympanic cavity was explored with the help of a surgical microscope. The pyramidal process was not developed. A possible ontogenetic explanation was provided. In the presented case, the cause of the anomaly may be failure of the em

Abnormalities of the stapedius reflex in patients with vertigo.

Analysis of the latency, amplitude, and rise characteristics of the stapedius reflex as demonstrated on a strip recorder revealed abnormalities in patients with vestibular neuronitis and other vertigo syndromes. The findings in vestibular neuronitis, acute labyrinthitis, perilymphatic fistulas, Meniere's disease, cerebrovascular insufficiency, and metabolic vertigo are described. Alterations in the stapedius reflex included increase in latency, decrease in amplitude, stepping of the rise, and absence of the reflex. This study suggests that absence of the reflex can be associated with vertigo a

Stapedius reflex threshold in cochlear implant patients.

The contralateral stapedius reflex elicited by sinusoidal electrostimulation via cochlear implant was investigated in 12 patients. Contraction of the middle ear muscle was monitored by time-resolved synchronized digital sampling of the impedance change in the contralateral ear. The reflex was detected in all patients in whom the uncomfortable loudness level (UCLL) could be reached and who had intact middle ear function. Generally, the reflex threshold is located in the upper part of the dynamic range between the most comfortable loudness (MCL) and the UCLL. The demonstrated method may provide

Stapedius reflex decay in normal hearing subjects.

The fatigue of the stapedius reflex was studied in 100 normal ears. Stimulus frequencies 500 and 1000 Hz were used, and the decrement of the stapedius reflex at 5, 10, and 15 dB above the reflex threshold was estimated. No decrement at all (less than 10% decay 10 sec after the onset of a stimulus 10 dB above the reflex threshold) was present in a majority of the ears. Between 27 and 58% of the ears showed reflex decay depending on variations of different factors such as stimulus frequency, intensity of the stimulus, and the method to register the reflex. Reflex decay was more often seen at 1000

Preservation of the stapedius tendon in laser stapes surgery.

OBJECTIVES/HYPOTHESIS: The stapedius tendon is routinely transected during stapes surgery. The objective of this study was to evaluate the technique of stapedial tendon preservation during stapes surgery and to compare results of these cases with cases where the stapedial tendon was not preserved. STUDY DESIGN: Retrospective study. METHODS: Four groups of patients were evaluated. Two groups had undergone stapes surgery with preservation of the stapedial tendon. One of these groups underwent a laser stapedotomy minus prosthesis (laser STAMP) procedure, while the other group had a prosthesis i

Detection of hearing impairment with the acoustic stapedius reflex.

The acoustic stapedius reflex (ASR) is the sound-evoked contraction of the stapedius muscle. The ASR is mediated by a neural network, receiving its afferent input from the VIIIth cranial (auditory) nerve and sending its efferent output to the VIIth cranial (facial) nerve. Several centers in the brain stem comprise the central portion of the reflex pathway. The stapedius muscle in each ear responds to ipsilateral, contralateral, and binaural stimulation. The normal ASR threshold is 90 to 95 dB SPL for tones and 70 to 75 dB SPL for wideband noise. In ears with sensorineural hearing loss, ASR thr

Decay and recovery of the acoustic stapedius reflex in humans.

The time course of decay and recovery of ipsilateral and contralateral stapedius reflex responses to 2 000 Hz pure tone stimulation was studied in 10 normal-hearing subjects. Reflex responses were found to follow a closely similar time course with respect to both decay and recovery in simultaneous bilateral recordings. The similarity is compatible with the assumption that decay and recovery originate in the afferent auditory system, and not in the muscles. Recovery was 50% complete 250 ms after the end of the stimulus and most subjects had reached their initial amplitude after 1-3 s. The indiv

The role of the stapedius reflex in poststimulatory auditory fatigue.

The human ear is very resistant against noise-induced damage in the low frequency range. The aim of the present study was to investigate whether or not the stapedium reflex is of any importance for this resistance. Subjects with peripheral facial palsy (Bell's palsy) including unilateral stapedius muscle paralysis were exposed to several different levels of narrow band noise centered at 0.5 and 2.0 kHz. Temporary threshold shift (TTS) at 0.75 kHz was significantly higher in the affected than in the nonaffected ear after 0.5 kHz noise at and above 110 dB SPL. After the exposure with 2.0 kHz nar

Stapedius tendon reconstruction during stapedotomy: technique and results.

During surgery for otosclerosis, it is common for the surgeon to cut the stapedius tendon. Even without reconstruction of the tendon, the results of this kind of surgery are particularly satisfactory. The stapedial reflex is more important for improving intelligibility of speech in the presence of background noise than for protection against hazardous levels of noise. An intact stapedial reflex improves ones ability to follow speech in the presence of background noise. This report will present a technique to reconstruct the stapedius tendon, along with the results obtained when adding this pro

Electromyography of the stapedius muscle via a retrofacial approach and electrically evoked stapedius reflex during cochlear implant surgery: a prospective bicentric study.

Measuring the electrically evoked stapedius reflex threshold (eSRT) with electromyography (EMG) of the stapedius muscle (SM) may offer an objective approach for cochlear implant (CI) fitting. During unilateral CI surgery in 14 adult patients with single-sided deafness, a retrofacial or anterior approach were used to evaluate the feasibility of SM-EMG during acoustically and electrically elicited stapedius reflex (SR). No adverse events occurred. This neurophysiology-based method was compared with visual detection of the SR. Intraoperatively, the belly of the SM was accessed via the retrofacial

Brainstem mediation of the stapedius muscle reflex in hydranencephaly.

Hydranencephaly is a rare condition in which the cerebral hemispheres are absent at birth and are replaced by membranous sacs in a cerebrospinal fluid-filled cranium. Surviving hydranencephalic patients have a functional brainstem and possible remnants of the cerebral cortex. This case report examines hearing function and the integrity of the brainstem mediated stapedius muscle reflex (SMR) in an adult with hydranencephaly, using middle ear impedance change measures. The brainstem mediated ipsilateral and cross-brainstem contralateral SMRs were elicited in the right ear at normal threshold lev

Stapedius reflex in patients with an inner ear prosthesis.

The stapedius reflex elicited by electrical stimulation was investigated in 21 deaf individuals supplied with an inner ear prosthesis. All patients were implanted with the Vienna cochlear implant using intra or extracochlear electrodes. Analog stimulation with sinus bursts of various durations (25, 50, 100, 300, 500 ms) and frequencies (125, 500, 1000, 2000 Hz) was used. The time course of the contralateral acoustic reflex was monitored using a fast-response impedance meter. The stapedius reflex was observed in about half of the cases. Reflex threshold was observed at a stimulation level close

The stapedius reflex tests in retrocochlear hearing disorders.

The commonly used parameters of the stapedius reflex responses in the diagnosis of sensorineural hearing disorders are the reflex threshold level and the persistence of the reflex response on prolonged stimulation, i.e., the reflex decay test. With the object of establishing the sensitivity of these reflex parameters to identify tumours affecting the eighth nerve in the early stages, a series of 97 such cases were examined. The mean hearing loss of the affected ears was 32 dB; 28 of the cases still had normal thresholds of hearing; 95 of the ears demonstrated elevated reflex thresholds and/or
