PURPOSE: Preclinical research provides evidence for the complement system as a potential common pathway in Stargardt disease (STGD1) and age-related macular degeneration (AMD) leading to retinal pigment epithelium (RPE) loss. However, systemic complement activation has not yet been assessed in STGD1 patients. We conducted a cross-sectional case-control study to assess systemic complement activation in STGD1 patients and its association with disease severity.
METHODS: Systemic concentrations of complement component C3 and its degradation product C3d were compared between 80 STGD1 patients and

Stargardt disease (STGD1) is an inherited juvenile macular degeneration caused by mutations in the ATP-binding cassette subfamily A member 4 gene (ABCA4), a flippase located in the photoreceptor outer segment. ABCA4 mutations cause the buildup of a toxic byproduct in the retina called lipofuscin resulting in the death of photoreceptors by ferroptosis. Mammalian disease models (e.g., mouse Abca4-/-) have been established, but do not replicate all the disease symptoms in human STGD1 patients. It is challenging to model a cone degeneration disease like STGD1 using mice. Here we suggest zebrafish

BACKGROUND: The fundus aspect of Stargardt's disease is characterized by progressive atrophic macular changes often associated with fundus flavimaculatus flecks. Other fundus changes have been described such as pallor of the optic disc, attenuated retinal vessels, cicatricial chorioretinitis, retinal pigment hyperplasia, subretinal neovascularization, or subretinal fibrosis. Three cases of Stargardt's disease and presumed subretinal hyperlipofuscinosis or subretinal fibrosis are presented.
METHODS: The clinical history, and the ophthalmoscopic and fluoroangiographic characteristics of these t

PURPOSE: Primary visual cortex (PVC) contains a retinotopic map in which the central visual field (CVF) is highly magnified compared to the peripheral field. Several studies have used functional magnetic resonance imaging (fMRI) in patients with macular degeneration to assess the reorganization of visual processing in relationship with the development of extrafoveal preferred retinal locus (PRL). We evaluated the functional response in PVC and its correlation with retinal parameters in patients with Stargardt disease due to ABCA4 mutations (STGD1).

IMPORTANCE: New outcome measures for treatment trials for Stargardt disease type 1 (STGD1) and other macular diseases are needed. Microperimetry allows mapping of light sensitivity of the macula and provides topographic information on visual function beyond visual acuity.
OBJECTIVE: To measure and analyze retinal light sensitivity of the macula in STGD1 using fundus-controlled perimetry (microperimetry).
DESIGN, SETTING, AND PARTICIPANTS: This was a multicenter prospective cohort study. A total of 199 patients and 326 eyes with molecularly confirmed (ABCA4) STGD1 underwent testing with the N

Reported growth rates (GR) of atrophic lesions in Stargardt disease (STGD1) vary widely. In the present study, we report the longitudinal natural history of patients with confirmed biallelic ABCA4 mutations from five genotype groups: c.6079C>T, c.[2588G>C;5603A>T], c.3113C>T, c.5882G>A and c.5603A>T. Fundus autofluorescence (AF) 30° × 30° images were manually segmented for boundaries of definitely decreased autofluorescence (DDAF). The primary outcome was the effective radius GR across five genotype groups. The age of DDAF formation in each eye was calculated using the x-intercept of the DDAF

UNLABELLED: Patients with Stargardt's disease often show a significantly reduced ability to read despite fairly good visual acuity. We evaluated whether fundus perimetry with simultaneous observation of the point of fixation can help to explain these difficulties.
METHODS: A total of 40 eyes in 21 patients with Stargardt's disease were examined by means of automatic fundus threshold perimetry and special fixation tasks using a scanning laser ophthalmoscope (SLO).
RESULTS: During fundus perimetry 19 eyes showed a movement of the mean fixation point towards the top, and five to the right of th

Autosomal recessive Stargardt disease type 1 (STGD-1) is a common inherited retinal disorder caused by mutations in the ABCA4 gene. This case series highlights the role of multimodal imaging (MMI): fundus autofluorescence, optical coherence tomography, optical coherence tomography angiography and fluorescein angiography in diagnosing and staging STGD-1 in three patients with varying disease severities. Imaging revealed characteristic retinal changes, including flecks and atrophic lesions, which were correlated with genotype and clinical presentation. Genotype-phenotype correlation demonstrated

We report on the case of a 19-year-old male with Stargardt disease (STGD1) who presented with a five-year history of progressive vision loss, accompanied by the recent onset of alternating exotropia. This patient initially sought care due to difficulties with near vision and tended to focus on distant objects when looking to the right. He was found to have a best-corrected visual acuity of 20/200 in both eyes. A comprehensive evaluation, including multimodal imaging and multifocal electroretinogram, was performed. Genetic testing confirmed the diagnosis of STGD1, identifying homozygous mutatio

ABCA4-associated retinopathies (Stargardt disease) are the most common inherited macular dystrophy and a leading cause of early onset central vision loss. Biallelic pathogenic variants in the ABCA4 gene cause impaired clearance of retinoid byproducts, leading to toxic bisretinoid accumulation, retinal pigment epithelium dysfunction, and progressive photoreceptor degeneration. Clinical presentation and disease progression are highly heterogeneous, largely influenced by genotype, age at onset, and environmental modifiers. Current management remains supportive and includes low-vision rehabilitati

The retina is the light-sensitive layer of the human eye. The macula forms the central part of the retina. The character of light responsiveness is attributed to the presence of photoreceptor cells here. Stargardt's disease is the most common cause of hereditary macular dystrophy. It is linked to disease-causing sequence variations/mutations in the ABCA4 gene on chromosome 1p21-p13, which destroys rod and cone cells within the retina. The disc membranes of rod and cone outer segments include an ATP-binding cassette transport protein encoded by the ABCA4 gene. All trans-retinal conjugates are t

Stargardt disease is a progressive hereditary retinal disease which is currently incurable. Although ABCA4 has been identified as a major causative gene, patients with genetic mutations in other genes (PRPH2, ELOVL4, and PROM1) display similar clinical phenotypes as Stargardt disease. Recent advances in genetic and molecular studies have greatly enhanced our understanding of the disease and have led to identification of targetable pathophysiological progresses and new molecular clinical applications. Here we summarize the current understanding of the pathophysiology and potential therapies for
