Thoracoscopic thymectomy in a previous sternotomy.

A video assisted thoracoscopic surgery (VATS) thymectomy is a viable alternative to conventional open thymectomy in myasthenia gravis (MG). A previous operation in the same anatomical region is generally considered a relative contraindication to any minimally invasive approach in the same area. Few cases of VATS on previously operated chests have been reported. We report a case of a VATS thymectomy in a patient who had undergone two previous sternotomies for cardiac disease.

Mucormycosis of a median sternotomy wound.

Mucormycosis is an unusual complication of cardiothoracic surgery. The fungi may infect the sternotomy wound causing a progressive gangrene or seed the implanted prosthetic valve or graft resulting in endocarditis or graft failure. There have been six previous reports of mucormycosis following cardiothoracic surgical procedures. Four cases involved prosthetic devices, the remaining two are examples of sternal wound mucormycosis acquired from the use of contaminated elasticized bandages. The first case of sternal wound mucormycosis not associated with elasticized bandages is reported here. The

Comparison Between Mini-Sternotomy and Full Sternotomy for Aortic Valve Replacement: A 10-Year Retrospective Study.

Introduction Aortic valve replacement (AVR) is a mainstay treatment for moderate to severe aortic valve stenosis. This retrospective study aimed to compare the clinical outcomes of mini-sternotomy and conventional sternotomy. Methodology This 10-year retrospective study compared the clinical outcomes of mini-sternotomy and full sternotomy. Patient-related outcomes include sternal wound dehiscence, operative time, length of hospital stay, and Intensive Care Unit (ICU) stay, whereas intraoperative parameters such as cardiopulmonary bypass (CPB) time and Aortic Cross Clamp time (ACCt) were compar

Sternal reconstruction after post-sternotomy mediastinitis.

BACKGROUND: Deep sternal wound complications are uncommon after cardiac surgery. They comprise sternal dehiscence, deep sternal wound infections and mediastinitis, which will be treated as varying expressions of a singular pathology for reasons explained in the text. METHODOLOGY AND REVIEW: This article reviews the definition, prevalence, risk factors, prevention, diagnosis, microbiology and management of deep sternal wound infections and mediastinitis after cardiac surgery. The role of negative pressure wound therapy and initial and delayed surgical management is discussed with special empha

Bilateral thoracoscopic extended thymectomy versus sternotomy.

BACKGROUND: Complete open surgical resection is the standard treatment for thymoma and myasthenia gravis. We evaluated the feasibility of bilateral video-assisted thoracoscopic extended thymectomy, and compared it to surgery via sternotomy. METHODS: From 2011 to 2014, 43 patients undergoing thymectomy were divided into 2 groups: 23 underwent video-assisted thoracoscopic extended thymectomy, and 20 had thymectomy via sternotomy. The primary outcomes were postoperative pain score (visual analog scale) at 6, 12, 24, 48, and 72 h, and 1-month postoperatively, and morphine consumption in the first

Post-sternotomy internal mammary arteriovenous fistula.

Arteriovenous fistula from the mammary artery is a rare complication following cardiac surgery. The fistula usually develops within the first 2 weeks after surgery and is initially asymptomatic. Typically, a continuous machinery murmur is heard along the parasternal border of the chest wall. A patient with an arteriovenous fistula between the right internal mammary artery and mammary vein following a combined aortic valve and coronary bypass operation is described. A transthoracic colour Doppler scan led to the diagnosis of the fistula. Because of potential late complications endovascular embo

Subclinical brachial plexopathy following median sternotomy.

In order to detect subclinical cases of brachial plexus injury following open heart surgery through median sternotomy, 15 patients were included in this prospective study. Physical and electrophysiologic examinations were performed preoperatively and postoperatively. The axillary F-loop latency was used to diagnose the brachial plexopathy. No clinical symptoms were found in this group postoperatively. Of the 15 patients, 13 (87%) were found to have a postoperative subclinical brachial plexopathy, at least on one side, and in eight of the patients, it was present on both sides.

Internal mammary arteriovenous fistula after sternotomy.

A patient who had undergone coronary artery bypass surgery was found to have a continuous murmur two weeks postoperatively. The diagnosis of internal mammary arteriovenous fistula was made by arteriography, which ruled out a more serious complication involving a saphenous vein to coronary artery anastomosis. The fistula was approached through the intercostal spaces above and below and ligated without difficulty. These are rare occurrences, usually asymptomatic, and are treated by ligation or ligation and excision. The etiology was thought to be closure of the median sternotomy incision with pa

Upper extremity neuropathies following median sternotomy.

The status of 958 patients who underwent median sternotomy between January, 1978, and May, 1981, was analyzed. Fifty-four patients had an upper extremity neuropathy. Among 38 patients who underwent further evaluation, motor and sensory nerve conduction studies localized the injury to the level of the elbow in 13, to the brachial plexus in 10, and to both locations in 6. Ninety-two percent of these 38 patients were asymptomatic 3 months after operation.

Brachial plexus injury following median sternotomy.

Brachial plexus injuries are annoyingly common after median sternotomies and vary from those causing minor symptoms to those producing major disability. We compared two groups of patients operated upon with the arms either abducted to a 90 degree angle or at the sides and found no difference in the incidence of brachial plexus injury. However, the finding of Horner's syndrome in one patient and the finding in another (at autopsy) of brachial plexus penetration by a fractured first rib caused us to question traction on the brachial plexus as the correct pathogenesis of the injury. A concomitant

Long-term outcomes of mini-sternotomy versus conventional sternotomy for aortic valve replacement: a randomized controlled trial.

OBJECTIVES: Aortic valve replacement (AVR) for severe symptomatic aortic stenosis is one of the most common cardiac surgical procedures with excellent long-term outcomes. Multiple previous studies have compared short-term outcomes of AVR with mini-sternotomy versus AVR with conventional sternotomy. We have previously reported the results of the randomized MAVRIC trial, which aimed to evaluate early postoperative morbidity among patients undergoing mini-sternotomy and conventional sternotomy AVR. We now report the long-term all-cause mortality, reoperation, MACE outcomes and echocardiographic d

Brachial plexus injury following median sternotomy.

OBJECTIVES: Brachial plexus injury is a rare complication after median sternotomy. We investigated that injury to the brachial plexus was retrospectively assessed in the results of three patients who underwent median sternotomy for open heart surgery. MATERIALS AND METHODS: All patients were placed in the hands-up position after right internal jugular vein cannulation, and the internal mammary artery was prepared for all of those. Nerve conduction measurements and electromyography were performed besides neurological examination. RESULTS: Brachial plexus injury was detected in three cases (0.

Minimally aortic valve surgery avoiding sternotomy.

OBJECTIVES: Minimally invasive valve surgery, although still in its pioneer era, can open new horizons in cardiac surgery. Following that trend we started aortic valve surgery through a right anterior minithoracotomy using a novel approach under 3-D video-assistance in most of the cases. METHODS: Aortic valve replacement avoiding sternotomy through a right thoracotomy was performed in seven patients (14% were female, average age: 58 years) There were five aortic stenoses and two aortic insufficiencies. In four patients we employed central aortic and right atrium cannulation and in three, femo

Upper rib fractures following median sternotomy.

First and second rib fractures occurred in 11 (16%) of 69 patients undergoing median sternotomy. Although 6 patients had no symptoms related to the rib fractures, 5 patients had postoperative chest, shoulder, and arm pain suggestive of angina pectoris or postpericardiotomy syndrome. The correct diagnosis of pain related to postoperative upper rib fracture may be made by direct visualization of the fracture on supine anteroposterior radiographs, elicitation of pain by palpation of the rib or motion of the upper extremity, lack of response to nitroglycerin, and negative electrocardiogram and car

Management of recalcitrant median sternotomy wounds.

During a 7 year period, 38 consecutive patients (31 male and seven female) had repair of an infected sternotomy wound. Their mean age was 56 years (range 13 to 78 years). Sternotomy was performed for cardiac disease in 34 patients. Staphylococcus aureus was the most commonly cultured organism. All patients required sternal debridement, and 17 had resection. Reconstruction was with muscle flaps in 37 patients, omental transposition in one, and both in four. Sixty-seven muscle transpositions were performed (63 pectoralis major, three rectus abdominis, and one latissimus dorsi). The mean number o
