Interventions to prevent stillbirth.

Stillbirth is one of the most distressing complications of pregnancy and still occurs far too frequently. The rate of stillbirth has been decreasing worldwide but room for improvement remains even in high-income countries. Risk factors for stillbirth have been identified in an effort to detect those women at increased risk. However, risk factors are non-specific and do not identify most stillbirths. Strategies employed to screen the general population such as assessment of fetal activity, fetal growth screening and biomarkers have also been used to identify increased risk for stillbirth. As wi

Maternal obesity and stillbirth.

The current obesity epidemic appears to contribute significantly to adverse fetal outcomes, and in this work we compile up-to-date evidence for the link between maternal obesity and risk of stillbirth. The review revealed a preponderance of evidence showing that the risk of stillbirth is increased among obese mothers with amplified risk estimates as the severity of obesity increases. Changes in interpregnancy body mass index (BMI) influence subsequent fetal survival and obese women that normalize their BMI values experience enhanced fetal survival in future pregnancies. The elevated risk of st

Neuropathology associated with stillbirth.

Neuropathologic findings in stillbirths oftentimes provide insight into the specific mechanisms leading to death. Examination of the brains of stillborn infants may also identify pathophysiologic processes that result in prenatal brain injury in liveborn as well as stillborn infants and that lead to neurologic disorders in liveborn infants, such as cerebral palsy or the sudden infant death syndrome (SIDS). A variety of abnormalities are found in the brains of stillborns, the most common including cerebral white matter necrosis (periventricular leukomalacia) or gliosis, germinal matrix or intra

Prospective risk of stillbirth.

Some fraction of any cohort of fetuses alive at a given gestational age will ultimately die before birth. The residual prospective risk of stillbirth as a function of gestational age was calculated from records of the New York City Department of Health covering 370,051 reported births between 1987-1989, including 2454 stillbirths. In the general population, the prospective risk of stillbirth at 26 weeks was one in 150 and, because the time distributions of live births and stillbirths were not proportionate, the risk changed with gestational age. By 40 weeks' gestation, it was one in 475, risin

Stillbirth after nonstress testing.

The authors' experience with stillbirth within one week of a reactive nonstress test was reviewed to determine the reliability of this test in postdates patients and general high-risk patients. The corrected stillbirth rates were the same in both groups. Thirty-three percent of stillbirths in postdates patients occurred as early as 41.5 weeks' gestation. The cause of stillbirth in postdates patients was not clear even after careful autopsy. In the general high-risk group, 60% of stillbirths were due to an acute event with the remainder due to chronic fetal compromise that was not predicted by

Placental pathology in stillbirth.

Placental assessment following a stillbirth is a crucial investigation. It helps identify possible causes or contributing factors to stillbirth. It can help evaluate the risk of recurrence in future pregnancies and may indicate whether further tests are needed. It also provides guidance for managing subsequent pregnancies. In this review, we discuss practical considerations in placental assessment following stillbirth, including how both pre-analytical and analytical factors can influence the assessment. We outline the changes that occur in the placenta after stillbirth and explain how to dist

Fetal Growth and Stillbirth.

Fetal growth restriction (FGR) is a common clinical manifestation of placental insufficiency. As such, FGR is a risk factor for stillbirth. This association has been demonstrated in numerous studies but is prone to overestimation because of the possibility of prolonged in utero retention before the recognition of the fetal death. Stillbirth risk reduction by optimizing maternal medical conditions and exposures and appropriate antenatal testing and delivery timing are essential to pregnancies affected by FGR. It is important to evaluate stillbirths with FGR with fetal autopsy, placental patholo

Management of stillbirth delivery.

Stillbirth is a common adverse outcome of pregnancy. Management should be individualized based on gestational age, maternal condition, prior uterine surgery, availability of skilled professionals, and maternal desires. This article discusses available data on management by gestational age and prior uterine surgery. Expectant management is a viable option for women and families who desire it and do not have any contraindications. In the second trimester, misoprostol induction and dilatation and evacuation are effective in the evacuation of the uterus. In the third trimester, induction of labor

Psychological effects of stillbirth.

Despite the high prevalence globally, the death of a baby to stillbirth is an often misunderstood and disenfranchised loss. Mothers, fathers, and families struggle to cope with the immediate and long-lasting effects of a baby's death which can last for years and sometimes decades. In addition, providers can be adversely affected by stillbirth, particularly when met with experiential avoidance and a sense of guilt and failure. There is little evidence on intervention efficacy in acute grief following perinatal death; however, there is a growing body of scientific literature on the efficacy of m

Sleep position, fetal growth restriction, and late-pregnancy stillbirth: the Sydney stillbirth study.

OBJECTIVE: To identify potentially modifiable risk factors for late-pregnancy stillbirth. METHODS: This was a population-based matched case-control study of pregnant women at 32 weeks of gestation or greater booked into tertiary maternity hospitals in metropolitan Sydney between January 2006 and December 2011. The case group consisted of women with singleton pregnancies with antepartum fetal death in utero. Women in the control group were matched for booking hospital and expected delivery date with women in the case group. Data collection was performed using a semistructured interview and inc

Maternal cohort, time of stillbirth, and maternal age effects in Italian stillbirth mortality.

STUDY OBJECTIVE: The aim of the study was to investigate factors in stillbirth mortality in Italy. DESIGN: Italian stillbirth data from 1955 to 1979 were analysed using a log-linear Poisson model with arbitrary constraints on the parameters to identify the effects of maternal birth cohort, calendar period of stillbirth and maternal age. SETTING: Stillbirth data for the whole of Italy were obtained from the Italian Central Institute of Statistics for the years under examination. During this period the criteria for defining stillbirth did not change and the quality of birth registration was si

Updating unanswered questions for stillbirth research: refresh of the UK Stillbirth Priority Setting Partnership.

OBJECTIVES: Despite UK targets to reduce stillbirth, there has been comparatively less research focused on stillbirth than on other pregnancy complications. This study aimed to ensure that future research addresses the most important contemporary questions regarding stillbirth by updating the 2015 UK Stillbirth Priority Setting Partnership (PSP), in accordance with the James Lind Alliance (JLA), in collaboration with over 30 professional and stakeholder organizations. METHODS: The Stillbirth PSP was accepted by the JLA for a refreshed list of priorities in June 2024, and a steering group was

Stillbirth: the heart of the matter.

We evaluated 2,083 cases within the Wisconsin Stillbirth Service Program (WiSSP) that had autopsy reports or ultrasound data relevant to the heart. Of these, 167/1,782 (9.4%) stillbirths after 20 weeks and 11/301 (3.7%) miscarriages <20 weeks had congenital heart disease (CHD). Cases were classified by type of heart defect and whether it related to cause of death. Among cardiac anomalies that contributed significantly to fetal death, 125/151 (83%) were associated with underlying conditions or syndromes, nearly half of which were chromosomal. The most common forms of CHD in stillborns were seve

Discovering the cause of stillbirth.

PURPOSE OF REVIEW: Stillbirth remains a major problem worldwide, with disparities both between and within nations. Evaluation and classification is essential for development and evaluation of preventive strategies. RECENT FINDINGS: Recently, several organizations have developed standardized protocols for stillbirth evaluation on the basis of maternal history, foetal examination/autopsy and placental pathology. Evaluation is moving from a search for discrete causes to recognition of contributing factors. Comparison of classification guidelines identifies several with the potential for effectiv
