A case of nontraumatic gastric rupture in an adult is reported. Rupture occurred after distention of the stomach with blood from a bleeding gastric ulcer.

Spontaneous rupture of the stomach may occur in cases of abnormal distension and reduced emptying capacity, either by normal peristalsis or by vomiting, of the stomach. We present a case of spontaneous rupture of the stomach in which the dilatation was due to apple fermentation and gas formation in the stomach. Peristalsis was inadequate for stomach emptying due to impaction of apple skins in the duodenum. The inability to vomit was due to a Nissen fundoplication performed 18 months earlier.

Two cases of impending gastric rupture, associated with malrotation and midgut volvulus, are reported. The diverticulous mucosal protrusions were characteristically revealed as a box-shaped gastric shadow in the plain X-ray and upper GI series. Two cases were operatively treated with only the seromuscular suture of the defect without excision of the mucosal protrusion. Midgut volvulus and malrotation were reduced. Post-operative course was uneventful.

When infants with tracheoesophageal fistula require ventilatory assistance, gastric distention may ultimately result in perforation and pneumoperitoneum. This report describes four neonates with esophageal atresia and tracheoesophageal fistula who experienced such a complication.

Pseudocysts of the pancreas are not rare, but spontaneous perforation and/or fistulization occurs in fewer than 3% of these pseudocysts. Perforation into the free peritoneal cavity, stomach, duodenum, colon, portal vein, pleural cavity and through the abdominal wall has been reported. Traumatic diaphragmatic rupture is uncommon in blunt or penetrating abdominal trauma. Diaphragmatic injury associated with pericardial tear is even rarer. A 50s man visited our hospital because of abdominal pain that initiated 1 day prior. An approximately 4.0×5.0×4.5c m tumor, which was in contact with the greater curvature of the gastric body, was detected on contrast-enhanced computed tomography. An unusual case of simultaneous multifocal rupture of the oesophagus, stomach and diaphragm after blunt trauma is described. The characteristic computed tomography scans (with intraluminal contrast application) of the ruptured oesophagus are shown, and successful management is presented.

The cases of two preschool-age children who suffered spontaneous gastric ruptures are reported herein. The first was a 2-year-old girl with tetralogy of Fallot, transferred to our hospital due to shock.
