Streptococcus constellatus: a rare causative agent of pyogenic liver abscess.

A 26-year-old man who was previously well presented to the emergency in septic shock. He had a preceding history of fever, right upper abdominal pain and jaundice. On examination, there was tenderness over the right hypochondrium and epigastrium, without features of generalised peritonitis. His blood tests were suggestive of sepsis with deranged liver function tests. CT scan of the abdomen showed multiples abscesses in various segments of the liver and a thrombus in the inferior venacava, without any other intraabdominal focus of infection. The abscess was aspirated under sonographic guidance,

Genetic and functional characterization of the natural transformation system in Streptococcus constellatus.

Streptococcus constellatus is an opportunistic pathogen frequently associated with abscess formation in various body sites. While the species has been shown to acquire exogenous DNA through natural transformation, functional analyses of its underlying mechanisms and optimized genetic editing protocols remain limited. Thus, our aim was to characterize the natural transformation system in S. constellatus and investigate environmental factors coordinating its activation. In addition, we sought to develop an optimized protocol for genome editing. Genomic analysis revealed that 73% of analyzed stra

A Rare Case of Primary Purulent Pericarditis Caused by Streptococcus constellatus.

Background: Bacteria-caused acute pericarditis is a very rare entity. It is usually associated with an underlying infection or compromised immune system. Primary purulent pericarditis in a previously healthy individual is highly unexpected; therefore, it is likely to have a delayed diagnosis and poor outcomes. Case: We report a case of an adult immunocompetent patient with primary bacterial pericarditis caused by a member of the commensal oral flora Streptococcus constellatus. The patient presented with septic shock and cardiac tamponade, and was further complicated with constrictive pericardi

Intra-abdominal abscess caused by Streptococcus constellatus in an immunocompetent patient.

Streptococcus constellatus is a Gram-positive commensal bacterium of the oropharyngeal, gastrointestinal and urogenital flora. It can cause abscesses in different parts of the body, especially in immunocompromised patients. We present the clinical case of a 33-year-old female patient with a previous history of one anastomosis gastric bypass, among others. The patient was hospitalized two years after surgery for peritonitis secondary to a perforation in the biliopancreatic loop, underwent laparoscopic surgery and was hospitalised for two weeks. Fifteen days after being discharged from the hospi

Empyema caused by Streptococcus constellatus: a case report and literature review.

BACKGROUND: Streptococcus constellatus is a member of Streptococcus anginosus group (SAG) that tends to cause pyogenic infections in various sites. However, Streptococcus constellatus is easily ignored by routine clinical laboratory tests for its prolonged anaerobic culture environment. CASE PRESENTATION: A 71-year-old man was admitted to our hospital due to productive cough, fever, chest pain and shortness of breath for 3 weeks. Chest computed tomography showed patchy opacities and right-sided pleural effusion, so a chest tube was inserted and purulent and hemorrhagic fluid was aspirated. Th

Management of refractory periorbital abscesses caused by Streptococcus constellatus: case report.

BACKGROUND: Streptococcus constellatus is recognized for its propensity for aggressive abscess formation; however, its refractory nature in periorbital infections remains underemphasized in the literature. This study aims to present two cases of treatment-resistant periorbital abscesses and delineate a successful multidisciplinary management protocol, complemented by a review of previously reported cases to provide a comprehensive clinical perspective. CASE PRESENTATION: The first case involves a 25-year-old male who presented with a three-day history of a maxillary dental abscess followed by

Streptococcus constellatus Peritonitis and Subsequent Septic Shock following Intrauterine Device Removal.

BACKGROUND: Previous reports have described cases of abscess formation by Streptococcus constellatus involving the oral cavity, gastrointestinal tract, and septic thrombophlebitis of the right ovarian vein with subsequent bacteremia and septic shock. Ascending infection from the genital tract to the fallopian tubes resulting in peritonitis from Streptococcus constellatus is a rare clinical circumstance where there is minimal information in the literature to guide its diagnosis, management, and expected prognosis. CASE: A 36-year-old G3P0111 developed a tubo-ovarian abscess two weeks after int

Fecal Signatures of Streptococcus anginosus and Streptococcus constellatus for Noninvasive Screening and Early Warning of Gastric Cancer.

BACKGROUND & AIMS: Most patients with gastric cancer (GCa) are diagnosed at an advanced stage. We aimed to investigate novel fecal signatures for clinical application in early diagnosis of GCa. METHODS: This was an observational study that included 1043 patients from 10 hospitals in China. In the discovery cohort, 16S ribosomal RNA gene analysis was performed in paired samples (tissues and feces) from patients with GCa and chronic gastritis (ChG) to determine differential abundant microbes. Their relative abundances were detected using quantitative real-time polymerase chain reaction to test

Acute glomerulonephritis associated with streptococcus pyogenes with concomitant spread of streptococcus constellatus in four rural families.

We studied history, renal histopathology and microbiology of an epidemic of acute glomerulonephritis associated with throat infections and uncommon culture results in four neighbour families. A 40-year-old man (index patient) was referred to a university hospital for dialysis and kidney biopsy due to a suspected acute glomerulonephritis. An acute tonsillitis had preceded the condition. Penicillin treatment had been started four days before the discovery of renal failure. Throat swabs were positive for beta-hemolytic streptococci, group C (GCS). GCS were also found in throat cultures from his w

Streptococcus constellatus Tubo-ovarian Abscess in a Non-Sexually Active Adolescent Female.

Tubo-ovarian abscess (TOA) in non-sexually active female adolescents is a rare presentation to the pediatric emergency department. In the following case, bilateral TOA secondary to Streptococcus constellatus was diagnosed in a 13-year-old virginal female. The patient was seen 4 months before presentation for interventional radiology-guided drainage and antibiotic treatment for an intra-abdominal abscess due to suspected appendiceal rupture. Exploratory laparotomy on the most recent presentation demonstrated an appendix with inflammation and serositis on pathology report, a concern for chronic

Emphysematous Esophagogastritis: a rare and life-threatening presentation of Streptococcus constellatus infection.

Emphysematous esophagogastritis (EEG) is a rare and potentially fatal infection of the upper gastrointestinal tract, marked by gas within the walls of the esophagus and stomach. It is typically associated with gas-forming organisms such as K. pneumoniae, P. aeruginosa, S. aureus, and S. anginosus; however, we report a unique case caused by Streptococcus constellatus, a species known for its aggressive tissue-invasive behavior but rarely implicated in EEG. A previously healthy adult presented with acute epigastric pain, fever, and septic shock. Computed tomography demonstrated diffuse intramura

Streptococcus constellatus Empyema: A Case Highlighting the Need for Timely Surgical Intervention.

This case report explores the intricate challenges of diagnosing and managing empyema caused by Streptococcus constellatus, particularly in patients with predisposing factors such as alcohol abuse and underlying respiratory conditions. We present a 34-year-old male patient with a medical history of hypertension, peripheral neuropathy, and alcohol abuse who developed empyema. Despite an initial presentation at another facility with symptoms mimicking a myocardial infarction and unremarkable chest X-ray results, his condition worsened, leading to a subsequent emergency department visit. The pati

An empyema caused by Streptococcus constellatus in an older immunocompetent patient: Case report.

RATIONALE: Empyema caused by Streptococcus constellatus is rare in patients without underlying diseases. However, the importance of the Streptococcus anginosus group, which consists of S constellatus, S anginosus, and Streptococcus intermedius, as causative organisms of empyema has been increasing. PATIENT CONCERNS: A 78-year-old man initially presented with dyspnea and chills for 4 days. He had no medical history. DIAGNOSIS: Chest X-ray and chest computed tomography showed a large and multiloculated pleural effusion with an air bubble on the right side. Cultivation of the pleural effusion u

Herpes zoster in the ophthalmic branch of the trigeminal ganglia obscuring cavernous sinus thrombosis due to Streptococcus constellatus subsp. constellatus - A case report.

BACKGROUND: Herpes zoster ophthalmicus (HZO) is an infectious disease that results from the reactivation of latent varicella zoster virus in the ophthalmic branch of the trigeminal ganglia. HZO manifests with herpes zoster-like symptoms such as rash with or without signs of ocular involvement. Cavernous sinus thrombosis (CST) is a life-threatening condition accompanied by signs and symptoms involving the eyes and the cranial nerves. CASE: We report a case of septic cavernous sinus thrombosis (caused by Streptococcus constellatus subsp. constellatus) which was masked by the simultaneous occurr
