This report describes a case of Streptococcus intermedius meningitis with an unusually gradual clinical course in an elderly patient with cancer. Such a subacute-to-chronic presentation has not been reported previously. Although S. intermedius meningitis typically involves intracranial suppuration, systemic imaging has revealed no abscesses or other infectious foci. The identification of this pathogen can be challenging in some clinical settings. In this patient, S. intermedius bacteremia was confirmed, whereas cerebrospinal fluid cultures were negative. However, a 16S rRNA gene analysis succe

Streptococcus intermedius, a gram-positive, beta-hemolytic, catalase-negative member of the Streptococcus milleri group, is a commensal organism within the human body that is known for its propensity to cause severe bacterial infections. This case report presents two distinct cases involving culture-positive S. intermedius infections. The first case involves a 53-year-old man diagnosed with new liver, lung, and brain lesions in the setting of suspected renal cell carcinoma. In the second case, an 84-year-old woman was found to have S. intermedius bacteremia with concurrent culture-positive liv

Brainstem abscess is an uncommon occurrence in itself and an abscess in the medulla oblongata is even rarer. Although these abscesses have established association with head and neck infections, we report the first known case of medullary abscess that was found to be strongly associated with dental procedure. Important point to note is that a simple procedure like tooth extraction can predispose spread of infective seeding to distant sites. High clinical index of suspicion and early diagnosis are essential, as if left untreated, the brainstem abscess is almost always fatal otherwise.

BACKGROUND: Purulent pericarditis is rare but rapidly fatal without urgent drainage and targeted antibiotics; malignancy-related effusions should not be presumed malignant when an anatomic breach is possible.
CASE PRESENTATION: A 67-year-old male with esophageal squamous cell carcinoma presented with pleuritic chest pain. Electrocardiogram showed diffuse ST elevation; transthoracic echocardiography revealed a large effusion with fibrinous strands and mild right-ventricle diastolic collapse. Computed tomography angiography demonstrated tumor erosion into the left main bronchus. The patient rec

Ventriculitis commonly arises as a complication of various central nervous system conditions. The causes of ventriculitis include both iatrogenic conditions, such as catheter-related infections, and non-iatrogenic conditions, such as community-acquired bacterial meningitis. The incidence of pyogenic ventriculitis associated with community-acquired bacterial meningitis remains unclear. Additionally, the optimal treatment strategy for pyogenic ventriculitis secondary to community-acquired bacterial meningitis remains uncertain. A 47-year-old man presented with headache, fever, and impaired consc

Brain abscess is a life-threatening infection that can occur secondary to contiguous or hematogenous spread. Several underlying conditions can lead to brain abscesses, such as dental infection, otitis media, sinusitis, and immunosuppression. Esophageal perforation leading to brain abscesses is extremely rare. We report a rare case of a 32-year-old man who presented to the emergency department with progressive headaches and upper-extremity weakness. Upon further evaluation, computed tomography (CT) revealed multiple brain abscesses secondary to Streptococcus intermedius infection. The patient e

Streptococcus intermedius is a gram-positive commensal associated with suppurative infections. Pediatric pulmonary abscesses due to S. intermedius are rare, and outcomes in children with severe neurological comorbidities are poorly characterized. A 5-year-old boy with refractory epilepsy and global developmental delay who presented with a right upper lobe abscess, pneumonitis, and pleural effusion. Targeted next-generation sequencing (tNGS) of bronchoalveolar lavage fluid (BALF), drainage pus culture, and tNGS of drainage pus all confirmed the presence of S. intermedius. To our knowledge, this

BACKGROUND: Cerebellar abscesses are rare but potentially life-threatening infections that require prompt diagnosis and management. Streptococcus intermedius is a recognized cause of brain abscesses, particularly in individuals with underlying conditions that compromise immune function. Sarcoidosis, a systemic granulomatous disease, may predispose patients to opportunistic infections due to immune dysregulation. To our knowledge, this is the first reported case of a cerebellar abscess caused by S. intermedius as the initial manifestation of sarcoidosis.

Streptococcus intermedius is a Gram-positive coccus usually found in the normal digestive or respiratory flora of humans and in some animal species, including rabbits. In conditions of immunosuppression, it can cause serious infections that can be difficult to treat or can even lead to death if not treated properly. S. intermedius-induced infections must be taken seriously, and proper treatment needs to be provided as soon as the patient is diagnosed, because otherwise, these infections can evolve in such a dramatic way as to result in the death of the patient. This study reports the case of a

INTRODUCTION: Streptococcus intermedius is an opportunistic pathogen associated with prolonged hospital stays and high mortality rates in adults. However, little is currently known about the clinical features of Streptococcus intermedius infection in children.
METHODS: This retrospective case series study included 40 children diagnosed with Streptococcus intermedius, confirmed through bacterial cultures or high-throughput sequencing. Antibiotic resistance was assessed through susceptibility testing. The site and clinical manifestations were evaluated for all patients.
RESULTS: The common inf

Subdural empyema is a collection of pus in the subdural space between the dura mater and the arachnoid. It carries very high morbidity and mortality as it can spread anywhere in the brain; however, the risk can be mitigated with appropriate surgical and medical intervention. Being protected by the skull, cranial infections are usually preceded by a significant risk factor, either an external invader such as skull fractures secondary to trauma, penetrating injury, prior surgery, or, more commonly, in more than 50% of cases, due to spread of an internal infection such as ear or sinus infections.

This is a case report of pyogenic sacroiliitis in a pediatric patient caused by Streptococcus intermedius. The patient is a 16-year-old boy who presented to an emergency department with sudden onset of back pain radiating to the left lower extremity. The diagnosis was confounded by the presence of isthmic spondylolisthesis. Plain radiography demonstrated mild isthmic spondylolisthesis but no radiographic signs of tumor, trauma, infection, arthritis, or other developmental problems. The C-reactive protein level was 23 mg/L. Over the next 24 hours, the patient developed fever, and the C-reactive

An 85-year-old woman with a history of rheumatoid arthritis fell due to unsteadiness and visited our emergency room due to head injury. The patient had minor head trauma and lip and oral cavity injuries, and she presented with fever. Laboratory tests showed high inflammatory marker levels. Moreover, there were indicative of urinary tract infection. Thus, the patient was admitted to our hospital. Blood cultures performed upon admission revealed the presence of Streptococcus intermedius, and contrast-enhanced computed tomography scan showed solitary right external jugular vein thrombosis and mul

PURPOSE: To report a woman with congenital rubella syndrome of delayed-onset, bleb-related endophthalmitis induced by Streptococcus intermedius.
METHODS: Interventional case report.
RESULTS: A 33-year-old woman who underwent bilateral trabeculectomy with adjunctive mitomycin C, 11 years earlier presented with redness and blurred vision in her right eye. She had received a topical antimicrobial agent because of bleb leak before presentation. She underwent anterior chamber tap and complete vitrectomy with intravitreal antibiotics. Cultures from the vitreous aspirate grew gram-positive cocci. T

Streptococcus intermedius is an inhabitant of the oral cavity and gastrointestinal tract, known to cause deep-seated abscesses. Thereby, we present a previously healthy adolescent with esophageal perforation (EP) and secondary mediastinal and brain abscesses due to Streptococcus intermedius. EP is a potentially life-threatening condition that requires a prompt diagnosis.

Streptococcus intermedius, an oral commensal and a cause of systemic pyogenic disease, expresses fimbriae. To identify the gene(s) encoding these fimbriae, we used a serum raised against purified fimbriae to screen libaries of recombinant lambda phages. The cloned gene cluster encoding S. intermedius fimbriae, (saliva-mediated aggregation and adherence-associated fimbriae), contained 4 ORFs, i.e. a putative ribonulease (Saf1), a putative adhesin (Saf2), the main pilus subunit (Saf3) and a sortase C (SrtC). Escherichia coli strains harboring recombinant phages and plasmids carrying the saf3 gen

Organ abscesses are a rare and life-threatening complication mostly of hematogenously disseminated infections. We report a case of brain and liver abscesses. Identification of the lesions was made by contrast-enhanced computed tomography (CT) and magnetic resonance imaging (MRI), respectively. An oral examination comprised an oral focus of infection. Streptococcus intermedius was isolated from oral smear, liver and ventricular drainage, and blood sample. After the commencement of antibiotic therapy, drainage of abscesses and oral rehabilitation, complete recovery was noted.
