Epidural abscess caused by Streptococcus milleri in a pregnant woman.

BACKGROUND: Bacteria in the Streptococcus milleri group (S. anginosus, S. constellatus, and S. intermedius) are associated with bacteremia and abscess formation. While most reports of Streptococcus milleri group (SMG) infection occur in patients with underlying medical conditions, SMG infections during pregnancy have been documented. However, SMG infections in pregnant women are associated with either neonatal or maternal puerperal sepsis. Albeit rare, S. milleri spinal-epidural abscess in pregnancy has been reported, always as a complication of spinal-epidural anesthesia. We report a case of

Isolation of Streptococcus milleri from clinical specimens.

During a period of 26 months, 80 strains of Streptococcus milleri were isolated from 80 patients with clinical evidence of infection. More than a third of the isolates were from lesions related to the gastro-intestinal tract and a quarter from abscesses of various other sites. The haemolytic activity and group antigenic profile of these isolates are discussed. In addition, the clinical significance of S. milleri isolated from various sites is evaluated.

Polymicrobial odontogenic periorbital and orbital necrotizing fasciitis (PONF): A case report.

PURPOSE: To present a case of periorbital and orbital necrotizing fasciitis (PONF) from an odontogenic source with a distinct microbiologic profile and highlight the need for emergent multidisciplinary management. OBSERVATIONS: A 39-year-old man presented with periorbital swelling, pain, and erythema following facial trauma. Imaging revealed peri-dental collections, accompanying maxillary sinusitis, and pre- and post-septal involvement. Immediate surgical debridement of necrotic tissue along with broad-spectrum antibiotics were pursued for management. Cultures grew multiple organisms, most no

Streptococcus sinensis may react with Lancefield group F antiserum.

Lancefield group F streptococci have been found almost exclusively as members of the 'Streptococcus milleri' group, although they have been reported very occasionally in some other streptococcal species. Among 302 patients with bacteraemia caused by viridans streptococci over a 6-year period, three cases were caused by Streptococcus sinensis (type strain HKU4T, HKU5 and HKU6). All three patients had infective endocarditis complicating their underlying chronic rheumatic heart diseases. Gene sequencing showed no base differences between the 16S rRNA gene sequences of HKU5 and HKU6 and that of HK

[Clinical significance of Streptococcus milleri isolated from various suppurative lesions].

Seventy-one strains of Streptococcus milleri were isolated from 68 patients admitted to Tamana Central Hospital. (1985.2-1989.1) S. milleri was isolated from the following specimens: sputum (15 strains), urine (10), ascites (9), postoperative wound (9), pleural effusate (6), bile (5), genital discharge (3), aspirated fluid from lung abscess (2) and others (12). In 12 patients (18%), S. milleri was isolated in pure culture and in 56 patients (82%) as part of a mixed flora. Other bacteria isolated in association with S. milleri were obligate anaerobes (41%), Gram-negative rods (40%), Gram-positi

[Bacteriological and clinical studies in 23 cases of thoracic empyema--the role of oral streptococci and anaerobes].

The bacteriology of empyema fluid and the clinical background of 23 cases from July 1987 through July 1992 were studied. Nineteen cases were male and 4 female, with a mean age of 59.6 years (range; 33 to 84 y.o.). There were 15 cases of community-acquired infection and 8 of nosocomial infection. Acute pneumonia and/or lung abscess developed into empyema in 19 cases. Sixteen cases had associated predisposing conditions, such as diabetes, chronic bronchitis, disorders with dysphagia, and excess alcohol intake. Forty-one strains were isolated from empyema in 22 cases. The predominant organisms, i

High-resolution CT findings in Streptococcus milleri pulmonary infection.

AIM: To assess pulmonary high-resolution computed tomography (CT) findings in patients with acute Streptococcus milleri pulmonary infection. MATERIALS AND METHODS: Sixty consecutive patients with acute S. milleri pneumonia who had undergone high-resolution CT chest examinations between January 2004 and March 2010 were retrospectively identified. Twenty-seven patients with concurrent infections were excluded. The final study group comprised 33 patients (25 men, 8 women; aged 20-88 years, mean 63.1 years) with S. milleri infection. The patients' clinical findings were assessed. Parenchymal abno

An uncommon case of septic cortical osteitis.

A 39-year-old healthy female patient with an unremarkable medical history complained about a rapidly progressive pain for a period of 1 month at the level of the anterior part of the right leg that was not relieved by NSAIDs. She mentioned only that she resumed her high level sport training 6 months before the onset of the symptoms. There was no specific history of trauma, fever, or other distinctive symptoms. On clinical examination, the right pretibial skin was inflamed and extremely painful. Walking became difficult the last 2 weeks. No skin injury was observed on the right leg and foot. Th

Nonclostridial gas gangrene due to Streptococcus anginosus in a diabetic patient.

Streptococcus anginosus was recently identified as a distinct species from the other members of Streptococcus milleri group (Streptococcus constellatus, Streptococcus intermedius). We report a rare case of nonclostridial gas gangrene caused by S. anginosus. A 62-year-old diabetic woman was admitted with gas gangrene of the perineal area. She had been taking her oral hypoglycemia medication regularly for 10 years, but the diabetes was inadequately controlled. She was treated with surgical debridement of the necrotic tissue, insulin injection, and antibiotic therapy, and had a satisfactory clini

"Streptococcus milleri" endocarditis caused by Streptococcus anginosus.

Unlike other viridans streptococci, members of the "Streptococcus milleri group" are often associated with abscess formation, but are only rare causes of infective endocarditis. Although it has been shown that almost all S. intermedius isolates and most S. constellatus isolates, but only 19% of S. anginosus isolates, were associated with abscess formation, no report has addressed the relative importance of the 3 species of the "S. milleri group" in infective endocarditis. During a 5-year period (April 1997 through March 2002), 6 cases of "S. milleri" endocarditis (out of 377 cases of infective

Prospective study of Streptococcus milleri bacteremia.

In a prospective study of bacteremia caused by organisms of the Streptococcus milleri group a total of 32 adult patients were observed over a seven-year period. These patients accounted for 1.6% of all patients diagnosed as having significant bacteremia and 17% of all cases of streptococcal (nonpneumococcal) bacteremia diagnosed during the study period. Only five patients had polymicrobic bacteremia. In 31 cases, a presumed origin of infection was identified, generally oral or gastrointestinal disease. There were only six cases of nosocomial acquisition. The most common presenting symptom was

A Rare Case of Myonecrosis with Soft-Tissue Emphysema in a Diabetic Foot Caused by Streptococcus anginosus Isolated in Pure Culture: A Case Study.

Streptococcus anginosus (SAG) is a known human pathogen and member of the Streptococcus milleri group. SAG is a known bacterial cause of soft-tissue abscesses and bacteremia and is an increasingly prevalent pathogen in infections in patients with cystic fibrosis. We describe a rare case of SAG as an infectious agent in a case of nonclostridial myonecrosis with soft-tissue emphysema. This is the only case found in the literature of SAG cultured as a pure isolate in this type of infection and was associated with a prolonged course of treatment in an otherwise healthy patient.

Streptococcal meningitis in adult patients: current epidemiology and clinical spectrum.

Streptococci other than Streptococcus pneumoniae are a rare cause of bacterial meningitis in adults. We report 29 cases of streptococcal meningitis (1977-1997). The patients comprised 19 men and 10 women, with a mean age +/- standard deviation of 47 +/- 18 years. Nine cases were secondary to neurosurgical procedures, seven to brain abscess, five to cerebrospinal fluid pericranial fistula, and three to endocarditis. Causative microorganisms included the following: viridans group streptococci, 20 cases; anaerobic streptococci, 3; Streptococcus agalactiae, 3; Streptococcus bovis, 2; and Streptoco

Investigation and management of adult periorbital and orbital cellulitis.

BACKGROUND: Orbital cellulitis has important complications. Despite this, there are few studies in the literature of large groups of cases of this condition. METHODS: We performed a retrospective case analysis of all patients admitted with periorbital and orbital cellulitis between 2002 and 2004. RESULTS: A total of 27 cases were included in the study. Of these, 25 had undergone a computed tomography scan, 19 of which had revealed significant sinus disease; 10 had had a microbiology result, with the most common organism being Streptococcus milleri; 20 had had a white cell count taken, with r

Group C streptococci in human infection: a study of 308 isolates with clinical correlations.

A collection of 308 clinical isolates of beta-haemolytic Lancefield group C streptococci was assembled from laboratories in England, Nigeria and New Zealand. Of these, 276 isolates were Streptococcus equisimilis, 23 S. milleri and nine S. zooepidemicus. Isolates of S. equisimilis in the African collection, though few, gave higher rates of lactose and raffinose fermentation, aesculin hydrolysis and positive alpha-galactosidase reactions than those from elsewhere. Erythromycin resistance was found in 1.9% of the English isolates of S. equisimilis. Strains from superficial infections accounted fo

Images of the month: Cavernous sinus venous thrombosis secondary to Streptococcus milleri maxillary sinusitis: An unusual cause of diplopia and headache.

Cavernous sinus venous thrombosis is an uncommon condition associated with high mortality rates if not recognised early. Symptoms include headache, visual loss, ophthalmoplegia, altered consciousness, proptosis and periorbital oedema. High-quality imaging is critical in early diagnosis and successful management. Primary infection (such as sinusitis) and possible complications (including meningitis) should be considered as potential aetiologies of cavernous sinus venous thrombosis, especially in those with a preceding history of localised infection. We present a case of a 50-year-old man with a

Parafalcine empyema, a tricky infectious cause of headache: a case report.

Headache caused by subdural empyema is usually associated with fever and symptoms and/or clinical signs of meningeal irritation and increased intracranial pressure. We describe a patient with headache with absence of these signs or symptoms of meningeal irritation or intracranial pressure, who turned out to have a parafalcine subduralempyema. A 28-year-old man had headache for 2 weeks, which had started with visual symptoms with duration of 5 minutes. Two days later, he developed fever. During these 2 weeks, he had recurrence of visual symptoms for 4 times, with duration of several minutes.Neu

[Bacteremia caused by Streptococcus salivarius and S. Milleri, and colonic carcinoma].

There is a well-known association between colon carcinoma and bacteraemia with Streptococcus bovis biotype I. There are also associations, less well known, with other streptococci of the viridans group. In three patients, a man of 44, a woman of 53 and one of 52 years old, colorectal carcinomas were diagnosed in association with bacteraemia with S. salivarius, S. milleri, and S. salivarius respectively.
