Cerebrospinal fluid (CSF) is thought to facilitate brain waste clearance and immune surveillance, yet its compartmentalization remains unclear. Previous work, using invasive dynamic intrathecal MRI contrast imaging, identified a perivascular subarachnoid space (PVSAS) that enhances along the major cerebral arteries with a 'donut'-like appearance. These findings suggest that the PVSAS may be separated from the surrounding broader subarachnoid space (SAS) by a semipermeable perivascular membrane. To investigate if the PVSAS could be observed non-invasively in healthy controls, we used a magnetic

CASE REPORT: A 33-year-old woman was admitted for evaluation of vertigo. An unruptured fusiform aneurysm of left distal posterior inferior cerebellar artery was found and presumed to be the cause of her vertigo. The aneurysm was clipped using three clips and then the vertigo was relieved. However, she developed low back pain 53 months after the operation. On plain radiographic examination, a right-angled clip was found at the sacral level. Cerebral angiography revealed two clips remaining at the previous location and previous aneurysm was no longer visualized. Her symptom of back pain was reso

Lipomatous hamartomas are rare disorders affecting the central nervous system. In our report, two observations of this disorder are presented. Both are interhemispheric in location and are associated with a complete agenesis of the corpus callosum, while having different histological structures. In our first patient, the intracranial formation caused refractory seizures, was partially surgically removed, and a biopsy was performed. Light microscopic examination disclosed the presence of a highly vascularized mature adipose tissue with numerous calcifications. The second case was an incidental

OBJECTIVE: To describe the anatomic variations of the subarachnoid space (SAS) in the facial nerve canal from human temporal bone specimens.
BACKGROUND: The SAS in the facial canal usually is assumed to terminate in the petrosal segment of the canal. This temporal bone study indicates that in 12% of temporal bones the SAS extends laterally into or beyond the tympanic segment of the facial canal.
METHODS: Histologic sections through the petrosal and tympanic segments of the facial canal were examined by light microscopy in 1 fetal (30 weeks) and 163 adult human temporal bones. The arachnoidal

Measurements of the subarachnoid space at myelography were carried out in three groups of patients, one group with stenosis of the lumbar spinal canal, one group without stenosis, and one intermediate group. Although spinal canal stenosis is most prominent at disc levels, the results demonstrate decreased sagittal diameter at mid-vertebral body level as well. The mean transverse diameters did not differ in three groups. This points towards congenital or developmental factors as the basic abnormality in the majority of patients with stenosis of the lumbar spinal canal.

OBJECTIVE: The aim of this study was to characterize ventricular measurements in children diagnosed with enlargement of the subarachnoid spaces (ESS) to determine ventricular morphology.
METHODS: Children diagnosed with ESS were retrospectively identified between 2015 and 2023. Inclusion required a craniocortical width > 5 mm on neuroimaging. Demographic data and developmental outcomes were collected. Referrals to therapy services, including speech and language therapy, occupational therapy, or physiotherapy, were recorded. Ventricular size was quantified using the Evans Index (EI), bicaudate

The central nervous system is lined by meninges, classically known as dura, arachnoid, and pia mater. We show the existence of a fourth meningeal layer that compartmentalizes the subarachnoid space in the mouse and human brain, designated the subarachnoid lymphatic-like membrane (SLYM). SLYM is morpho- and immunophenotypically similar to the mesothelial membrane lining of peripheral organs and body cavities, and it encases blood vessels and harbors immune cells. Functionally, the close apposition of SLYM with the endothelial lining of the meningeal venous sinus permits direct exchange of small

The purpose of this article is to describe, with 5 clinical cases, the physiological communications between the inner ear and the subarachnoid spaces (SAS) and present the imaging features with regard to. Therefore we briefly illustrate abnormal communications between SAS and perilymphatic fluids in certain cochlear and internal acoustic meatus (IAM) malformations and their consequences. Imaging features may depict diffusion pathway of bacterial meningitis to membranous labyrinth via the cochlear aqueduct or via the IAM. Rarely, in some patients referred for cochleovestibular symptoms, imaging

At our hospital, the initial surgical treatment of Rathke's cleft cysts is simple drainage and wall biopsy by opening the cyst. If intraoperative cerebrospinal fluid leakage occurs, subarachnoid space cisternostomy is added to prevent reaccumulation of cyst contents. We aimed to determine whether the addition of cisternostomy to simple drainage decreases postoperative reaccumulation and reoperation rates. Rathke's cleft cysts initially operated on between January 2011 and December 2021, with postoperative follow-up of more than 1 year, were retrospectively reviewed. The postoperative course wa

Seven children with rapid enlargement of the head and other clinical features resembling hydrocephalus are described. All children remained with relatively large heads, developed slowly and all but one had various degrees of mental and motor handicaps. The EEG was disturbed in all cases; two children developed seizures. Increased intracranial pressure was present. In all patients the subarachnoid space was markedly widened while the ventricular system was normal or minimally dilated. The characteristics of this syndrome, the features distinguishing it from similar conditions as well as its pos

The purpose of this series was to describe sonographic features of an isolated widened fetal subarachnoid space with a thin cerebral mantle and possible associations. Between January 2004 and December 2013, fetuses with a prenatal diagnosis of a widened subarachnoid space were prospectively recruited and followed. Histories of medical and familial diseases, as well as other demographic data such as drug exposure and lifestyles, were assessed and prospectively recorded. The women were investigated for possible associated factors. Ten pregnant women were recruited. Their fetuses showed various d

INTRODUCTION AND IMPORTANCE: Intracranial dermoid cysts (IDC) are defined as rare, slow-growing cystic congenital neoplasms. Rupture of an intracranial dermoid cyst occurs rarely and most often spontaneously and results in potentially serious symptoms.
CASE PRESENTATION: A39-year-old female, with mechanical prosthetic heart valve presented with history of headache for 10 months and generalized tonicoclonic seizures. On the admission, the patient had a normal neurological and cranial nerve exam. The authors performed a computed tomography of the brain, The MRI could not be performed because of

OBJECTIVE: Benign enlargement of the subarachnoid space (BESS) is the most common cause of macrocephaly in infants. This study aimed to evaluate the neurodevelopmental outcomes in infants with BESS.
MATERIALS & METHODS: In this follow-up study, all records of infants diagnosed with BESS in 2012-2016 were assessed. A clinical follow-up examination was carried out at 6, 12, 18, and 24 months of age to assess the macrocephaly outcomes. Denver Developmental Screening Test-II (DDST-II) was used for evaluating the psychomotor development of infants at 24 months of age. All data were entered in SPSS

PURPOSE: Explore in vivo whether there is direct communication between the cerebrospinal fluid (CSF) and extravascular compartment of human visual pathway structures.
METHODS: A prospective and observational study included 10 subjects who underwent intrathecal gadolinium-enhanced magnetic resonance imaging (MRI) for suspected CSF circulation disorder, but with a negative result and with no known ophthalmic diseases. After precontrast T1-weighted MRI, 0.5 mL of gadobutrol (Gadovist, 1.0 mmol/mL) was injected intrathecally. Gadobutrol distributes in the extravascular space, and served as a CSF

Enlargement of the subarachnoid spaces is occasionally encountered during neuroimaging of children. This enlargement is generally regarded as a nonpathologic process that resolves uneventfully. However, there are several genetic disorders in which enlargement of the subarachnoid spaces can be an early sign, or the feature of an associated syndrome, that may aid in the underlying diagnosis. Recognizing subarachnoid space enlargement in these circumstances requires an understanding of the normal physiology of the subarachnoid space at different time points in a child's neurodevelopment.
