The subclavian steal syndrome is characterized by a subclavian artery stenosis located proximal to the origin of the vertebral artery. In this case, the subclavian artery steals reverse-flow blood from the vertebrobasilar artery circulation to supply the arm during exertion, resulting in vertebrobasilar insufficiency. As the vertebrobasilar arterial system feeds both the peripheral and central auditory and vestibular systems, in subclavian steal syndrome, neurotological symptoms are expected because of the vertebrobasilar insufficiency. In this report, we describe three patients suffering from…

The Subclavian Steal Syndrome is a condition that results from the stenosis or occlusion of the subclavian artery proximal to the origin of the vertebral artery. The blockage causes the reversal of the normal direction of blood flow in the vertebral artery which is termed the "steal", because it steals blood from the cerebral circulation. Blood is drawn from the contralateral vertebral, basilar or carotid artery regions into the low-pressure ipsilateral upper limb vessels. Although a relatively uncommon disease, it represents a condition which mimics many conditions often treated by chiropract…

The clinical findings in this series of 22 cases of subclavian steal syndrome showed a predominance of men (13:9), a mean age of 51 years, and a predominance of left subclavian artery obstruction. There was a blood pressure differential of more than 20 mm Hg and diminished pulses ipsilaterally in most. Main indications for surgical repair were transient neurologic symptoms. Twenty-one of our patients had surgical repair of the subclavian steal syndrome; six had concomitant carotid artery surgery. The most common surgical procedure performed consisted of a carotid subclavian bypass graft. One p…

Anaesthesia in two patients with subclavian steal syndrome is described. Both patients showed an interesting response to hyperventilation. The mechanism of this response and its implications for the anaesthetist are discussed.

Proximal occlusions of the subclavian and innominate arteries may create a cerebrovascular insufficiency syndrome. The syndrome is generally characterized by neurologic symptoms of the vertebral-basilar arterial distribution. Blood is siphoned along the vertebral artery because of the low pressures distal to the occlusion. The clinical syndrome is best confirmed by a thoracic arch aortogram. Treatment is reserved for symptomatic patients. An extra-thoracic revascularization of the distal subclavian artery is the preferred operation.

OBJECTIVES: To evaluate the safety and feasibility of subclavian artery stenting for coronary-subclavian steal syndrome (CSSS).
BACKGROUND: CSSS is a rare cause of myocardial ischemia due to ipsilateral subclavian artery stenosis in patients who have undergone coronary artery bypass graft. However, current knowledge of the optimal therapy for CSSS is limited.
METHODS: The clinical data of 37 patients (33 male; mean age 65 ± 6 years) with CSSS who had undergone subclavian artery stenting between April 2007 and December 2015 were analyzed.
RESULTS: The time elapsed between bypass surgery and …

The clinical benefits of using the left internal mammary artery (LIMA) to bypass the left anterior descending artery are well established making it the most frequently used conduit for coronary artery bypass surgery (CABG). Coronary subclavian steal syndrome (CSSS) occurs during left arm exertion when (1) the LIMA is used during bypass surgery and (2) there is a high grade (≥75%) left subclavian artery stenosis or occlusion proximal to the ostia of the LIMA resulting in "stealing" of the myocardial blood supply via retrograde flow up the LIMA graft to maintain left upper extremity perfusion. A…

Subclavian steal syndrome (SSS) is usually caused by unilateral subclavian artery (SA) occlusion, and bilateral SSS is very rare. Takayasu's arteritis (TA) is a chronic granulomatous form of vasculitis that affects the SA, most commonly in women aged 15-40 years. We report a rare case of bilateral SSS due to TA in a 52-year-old woman, who exhibited severe vertigo. Although her blood pressure was within the normal range and did not differ between her arms, Doppler ultrasonography revealed low antegrade blood flow in the right SA and retrograde flow in the left SA. Computed tomography angiograph…

Eighty-two patients presenting with subclavian steal syndrome (36 men, 46 women; median age, 66.5 years) were studied. All patients underwent clinical and noninvasive evaluation. Diagnosis was based on both a 20 mmHg difference in blood pressure between arms and reversed blood flow in the vertebral artery. Twenty-one patients (25.6%) had a transient ischemic attack or cerebrovascular accident before the study. In 16 patients (19.5%), the anterior circulation was involved and the vertebrobasilar circulation was effected in 5 patients (4.8%). Fifty-five patients were followed for one to six year…

A female patient with subclavian steal syndrome is presented in this paper. She was treated originally in the ORL department because of objective ear sensations and of Menière's disease. Otological symptoms of the steal effect, e.g. dizziness, uni- or bilateral internal ear disorders and tinnitus are discussed in view of literature data and of the case history. In the differential diagnosis of above symptoms the ORL physician should also consider subclavian steal syndrome

An 80-year-old female presented to the emergency department with dizziness, generalized weakness, and exertional dyspnea. Physical examination revealed a significant inter-arm systolic blood pressure discrepancy. Computed tomography angiography (CTA) of the chest demonstrated high-grade stenosis at the origin of the left subclavian artery, correlating with both the clinical symptoms and the hemodynamic findings. This case underscores the diagnostic utility of CTA in ensuring a prompt and precise evaluation of subclavian artery stenosis in patients presenting with inter-arm blood pressure diffe

INTRODUCTION: Vertebrobasilar insufficiency (VBI) is a condition that results from restricted blood flow to the posterior portions of the brain, which are primarily served by the vertebral and basilar arteries. It is the most common cause of vertigo in the elderly and is usually accompanied by impaired vision and sensation. Congenital abnormalities, atherosclerosis, stroke and/or trauma may all lead to decreased vertebrobasilar circulation. A syndrome called Subclavian Steal Syndrome (SSS), which manifests with similar neurological symptoms but with a different pathophysiology, may also cause …

Coronary subclavian steal syndrome is a rare ischemic cause in patients after myocardial revascularization surgery. Subclavian artery stenosis or compression proximal to the internal mammary artery graft is the underlying cause. The authors present a clinical case of a patient with previous history of non-ST elevation myocardial infarction, triple coronary bypass, and effort angina since the surgery, with a positive ischemic test. Coronary angiography revealed a significant stenosis of the left subclavian artery, proximal to the internal mammary graft.
