Relationship of personality factors and some social habits to cardiovascular risk in submariners.

Correlational and contingency analytical techniques were used to investigate the relationship between subtest scores on the Minnesota Multiphasic Personality Inventory (MMPI) and coronary heart disease (CHD) risk factors in 1000 submariners. Groups of personality traits, both enhancing and protecting against risk of heart disease, were identified in these subjects. Personality trait patterns tending to be least associated with cardiovascular risk are tentatively identified by the three MMPI scales: psychasthenia, schizophrenia, and social introversion. On the other hand, the scales most strong

Barodontalgia and barotrauma in the human teeth: findings in navy divers, frogmen, and submariners of the Federal Republic of Germany.

From 2,580 submariners, divers, and frogmen, 13,618 individual findings were evaluated from a total of about 50,000 dental findings of the Nautical Medical Institute of the German Navy, Kiel, West Germany. Trend analysis and statistical significance tests showed that the teeth of navy divers and frogmen had deteriorated very much more within a period of 9 years after their first dental examination than those of submariners. This negative tendency was verified through additional direct comparison between 37 navy divers and 37 submariners whose dental states were observed during a period of 10 y

A preliminary study on submariners with xerostomia after a 3-month deployment.

To observe the clinical manifestations and salivary secretion of xerostomia patients in submariners who engaged in a three-month deployment. The general conditions and clinical examination of the 136 submariners were evaluated, by which the patients with xerostomia were screened out and their clinical manifestations were recorded. Besides, the flow rate of unstimulated saliva and stimulated saliva was measured and calculated. Subsequently, the related factors of xerostomia were quantitatively classified and statistically analyzed. In all the involved submariners, 42 were diagnosed to have xero

Identifying Predictors of Pressurized Submarine Escape Training (PSET) Attrition.

Pressurized Submarine Escape Training (PSET) physically prepares submariners to safely escape a submarine at depth. Failure to complete PSET is not a submarine service disqualification. Serious medical incidents are rare, but the safety record tradeoff has been low throughput. From 2009 to 2015, only 34% of students screened completed PSET. Students may be medically screened out of the training altogether (disqualified), or dropout during the physical training (attrite). Training records from 12,122 U.S. Navy students were used to identify factors contributing to training disqualification and

Dysbaric osteonecrosis of the femoral diametaphysis.

A 28-year-old submarine officer was involved in a dysbaric exposure at 155 feet for 21 hours. Subsequent to rescue, he developed a painful osteonecrosis involving over two-thirds of the femoral shaft and distal metaphysis. Pain is totally atypical for a bony injury of this distribution but because of failure to improve after 12 months, he underwent a series of three operations: (1) core biopsy, (2) intramedullary reaming and intramedullary rod fixation of the femur, and (3) bone grafting to a metaphyseal defect in the distal femur. Serial MRI scans were used to follow the progression of his os

An Approach to Reduce the Incidence of Submarine Medical Evacuations by Identifying Individual Risk Characteristics: Preliminary Findings.

INTRODUCTION: Military medical evacuations (MEDEVACs) are resource intensive and can disrupt operations and decrease readiness. Medical evacuations are a concern for the submarine force because of the limited medical resources onboard, the impact of manpower loss on smaller crews, and the compromise of operational stealth. Although some medical emergencies cannot be avoided, some MEDEVACs may be preventable. However, there is limited knowledge of the underlying causes and risk factors associated with submarine MEDEVACs. This work describes an approach to identify individual characteristics ass

Evaluation of a contingency blood donor program on U.S. Navy submarines.

OBJECTIVE: Because the role of submarine warfare is shifting from strategic deterrence to littoral force projection, submarine providers will be required to manage combat trauma. Currently, submarine providers have only crystalloid available for the reversal of hemorrhagic shock. A proposal program to provide blood products on submarines was evaluated. METHODS: Existing military emergency blood transfusion protocols were reviewed. The restrictions of donation/transfusion onboard submarines were considered. RESULTS: A protocol to provide screening for, implementation of, and a reporting syste

Is there still a benefit to operate appendiceal abscess on board French nuclear submarines?

Appendicular abscess occurred in 14.2% of patients presenting acute appendicitis. Management of these patients remains controversial, ranging from an emergency appendectomy to a nonoperative treatment. On board French nuclear submarines, the usual treatment for all cases of appendiceal masses, including both appendicitis and appendiceal abscess, is an appendectomy. In the past 5 years, the introduction of ultrasonography (US) on board has enabled the diagnosis of appendiceal abscess with a high rate of accuracy, and the latest studies show that nonoperative treatment is an alternative approach

Subject of the study was spreading of mycoses in the troops based on the Kola Peninsula. Examined were the military serving on atomic submarines, maintenance crews of deactivated atomic submarines awaiting disposal, and coastal units. Spreading of skin lesions among the submariners was not same as among the coastal military. Signs of clinical mycoses were observed in 41.2% of submariners of the active unit, in 66.9% of the maintenance crews, and in 38.1% of the coastal military. Infection agents were fungi g. Candida (albicans, guilliermondii, krusei, pseudotropicalis), Epidermophytia plicarum

Diabetic Ketoacidosis Reported from U.S. Navy Submarines, 2021-2024.

BACKGROUND: Type 1 diabetes mellitus (T1DM) is a complex systemic condition that poses unique challenges for diagnosis and management in military environments, particularly on submarines where access to standard medical resources is limited. CASE SERIES: This case series examines three instances of a first diagnosis of T1DM among active duty male U.S. Navy submariners, highlighting the challenges of managing this condition in austere military environments. Case 1 involves a 27-yr-old experiencing gastrointestinal symptoms during deployment and found to have diabetic ketoacidosis (DKA) complic

A Bosphorus submarine passage and the reinvention of neurosurgery.

One of the major themes characterizing the emergence of modern neurosurgery has been the concept of technology transfer and the application of a broad spectrum of revolutionary elements of technology from both physical and biological science. These transference applications are now apparent in modern neurosurgery as it is practiced on all continents of the globe. More than 3 decades ago, these ideas that now have come to fruition were in states of formulation. This article describes and further documents one such fertile cauldron of ideas and practical realities--the United States Navy Nuclear

[Maximal pulmonary ventilation and the forced expiratory rate under hyperbarism].

In divers, breathing with artificial gas mixtures of 14.3 g/l density, the maximal lung ventilation and the maximal velocity of forced expiration decrease along with an increase in the mixture density. The decrease of these parameters is unrelated to nitrogen anesthesia or exhaustion of respiratory muscles. The findings suggest that the value of both these parameters is only limited by expiratory dynamic compression of respiratory pathways.

Interdependence of decompression sickness and plasma enzymes on dive profile and vitamin B-6 status.

The mortality rate due to decompression injury was found to be significantly greater in a rat population with a mild vitamin B-6 deficiency, compared to an adequately fed control group, when subjected to a bends-producing N2-O2 dive. Relative post-dive changes in lactate dehydrogenase, creatine phosphokinase, and transaminase levels in plasma do not appear to be sufficiently different to allow a ready distinction in the degree of susceptibility of one nutritionally defined population from the other.

[Effects of submarine engine room steady noise on the compound action potential tuning curves and its relation to cochlear pathology in guinea pigs].

Compound Action Potential Tuning Curves (CAP-TC) for tone pip of 2k, 4 kHz were examined in 8 guinea pigs before and after exposure to noise with main energy centered in the range of 0.25-4.0 kHz. CAP-TC was measured with the pure tone simultaneous masking profiles. AP was evoked by tone pip with an intensity of 10 dB above threshold. Masker level producing 40% reduction in AP amplitude was used. Relations between changes in CAP-TC and AP threshold shifts and the pathology of the stereocilia of hair cells were evaluated by surface preparation and SEM observation in 13 ears. After noise exposur

Clinical utility of dipstick urinalysis in assessing fitness to dive in military divers, submariners, and hyperbaric personnel.

INTRODUCTION: Routine dipstick urinalysis is part of many dive medical assessment protocols. However, this has a significant chance of producing false-positive or false-negative results in asymptomatic and healthy individuals. Studies evaluating the value of urinalysis in dive medical assessments are limited. METHODS: All results from urinalysis as part of dive medical assessments of divers, submarines, and hyperbaric personnel of the Royal Netherlands Navy from 2013 to 2023 were included in this study. Additionally, any information regarding additional testing, referral, or test results conc
