On the dysfunctional hemoglobins and cyanosis connection: practical implications for the clinical detection and differentiation of methemoglobinemia and sulfhemoglobinemia.

Methemoglobinemia and sulfhemoglobinemia are potentially life-threatening blood-related disorders characterized by similar symptoms and markedly distinct treatment procedures. In this paper, we investigate the causal relationship between these conditions and the onset of cyanosis, which is typically associated with a purple or bluish skin coloration. More specifically, we perform controlled experiments to elicit cyanotic appearances resulting from different severity levels of these disorders and varying physiological conditions. We note that such experiments cannot be induced in living subject…

Hemolytic anemia and sulfhemoglobinemia due to phenacetin abuse: a case with multivisceral adverse effects.

The authors report a case of recurrent sulfhemoglobinemia resulting from a chronic abuse of phenacetin. Cyanosis was accompanied by hemolytic anemia and numerous features of tissue aging. While phenacetin was found to be the oxidizing drug, no source of sulfur was identified. The origin of sulfhemoglobinemia in man is discussed and the possible role of intraerythrocytic glutathione is emphasized.

Congenital sulfhemoglobin and transient methemoglobinemia secondary to diarrhoea.

Sulfhemoglobinemia and Methemoglobinemia are common causes of cyanosis arising due to accumulation of abnormal heme pigment. We present an unusual case of congenital sulfhemoglobinemia along with diarrhoea induced methemo-globinemia in a neonate. A high index of suspicion and a proper diagnostic approach is needed to determine the true frequency of their occurrence together.

Interpreting sulfhemoglobin and methemoglobin in patients with cyanosis: An overview of patients with M-hemoglobin variants.

INTRODUCTION: Methemoglobin (MetHb) and sulfhemoglobin (SHb) measurements are useful in the evaluation of cyanosis. When one or both values are elevated, additional analysis is important to establish the etiology of the disorder. Methemoglobinemia occurs from acquired or hereditary causes with diverse treatment considerations, while true sulfhemoglobinemia is only acquired and treatment is restricted to toxin removal. Some toxic exposures can result in a dual increase in MetHb and SHb. Hereditary conditions, such as M-Hemoglobin variants (M-Hbs), can result in increased MetHb and/or SHb values…

Phenacetin-induced hemolytic anemia.

The hematological features of phenacetin-induced hemolytic anemia are presented in order to make the physician aware of the abnormalities which suggest the use of an oxidant drug. The presence of "bitten out" red cells is the commonest initial clue to the existence of drug-induced hemolytic anemia. The diagnosis is confirmed by the demonstration of Heinz bodies and sulfhemoglobinemia. Early recognition of this form of drug-abuse may avert the development or progression of analgesic nephropathy.

Biologic activity of hydroxylamine: a review.

Although hydroxylamine, as such, is a product of normal cellular metabolism it is also a potent mutagen in vitro. However, in spite of this … rrosive to the eyes. Acute and chronic exposures to hydroxylamine have caused methemoglobinemia and sulfhemoglobinemia.

Discomforts and laboratory findings in workers exposed to sulfur dioxide.

In a broom manufacturing factory the authors performed microclimatic measurements, measurements of sulfur dioxide concentration and dust content. Workers (n = 190) were polled regarding discomforts characteristic of sulfur dioxide effects. Sulfates were determined in urine of 56 subjects, and methemoglobin and sulfhemoglobin were determined in blood. Sulfates were determined in 43 controls and methemoglobin and sulfhemoglobin were determined in 39 controls. Sulfur dioxide concentration in work environment ranged from 17.1 to 149.4 mg/m3 in winter and from 0 to 0.75 mg/m3 in summer. The exposed

[Fatal calcium polysulfide overdose presenting corrosive chemical injury of esophagus and sulf-hemoglobinemia].

A-79-year-old woman ingested a cup of unknown violet agricultural solution intentionally. She was vomiting and smelt of sulfur. Arterial blood gas showed metabolic acidosis and marked cyanosis regardless of relatively high PaO2, caused by sulfhemoglobinemia. A nasogastric tube could not be inserted because of marked stenosis caused by endoscopically proven severe corrosive chemical injury (burn) of esophagus. Considering the smell and the clinical presentation, we concluded that the causative agent was calcium polysulfide or lime-sulphur solution, a common agricultural product used as a fungic…

Hematological and hemorheological effects of air pollution.

Selected hematological parameters and erythrocyte deformability indexes for 16 young male military students were compared before and after a … emoglobin percentage was increased to 2.37 +/- 0.49% (mean +/- standard error) from 0.51 +/- 0.23%. Sulfhemoglobinemia was present in six subjects after the period of pollution, but it was not present in any student prior to this period. Significant increases in erythrocyte deformability indexes were observed after the period of pollution, i.e., from 1.13 +/- 0.01 t …

Flutamide-induced cyanosis refractory to methylene blue therapy.

Flutamide, an anti-androgen used in prostate cancer therapy, is also a derivative of aniline. Mild, usually asymptomatic, methaemoglobinaemia has been reported. We report a patient receiving flutamide therapy who developed cyanosis, dyspnoea and anaemia, initially attributed to marked methaemoglobinaemia by the CO-Oximeter method. An unsuccessful trial of methylene blue therapy led to the finding of marked sulphaemoglobinaemia. Sulphaemoglobinaemia has not previously been reported with flutamide use. Recognition of this association is important, given the refractoriness of sulphaemoglobinaemia

[A rare cause of cyanosis: Sulphaemoglobinaemia related to thiocolchicoside (Miorel)].

INTRODUCTION: An acquired abnormality of haemoglobin is among the many causes of cyanosis, especially in patients with no identified cardiorespiratory cause. CASE REPORT: A 50-year-old woman, suffering from amyotrophic lateral sclerosis, was hospitalised for dyspnoea. Physical examination revealed cyanosis that persisted despite oxygen therapy. Discordance between the reduced arterial oxygen saturation and normal arterial oxygen tension led to a search for a dyshaemoglobinaemia as a possible cause. Use of co-oxymetry with spectrophotometry revealed sulphaemoglobinaemia. Sulphaemoglobinaemia i
