INTRODUCTION: The superficial musculoaponeurotic system (SMAS) is a controversial functional fibro-adipose layer that connects the mimic muscles to the skin and is involved in a variety of facial mimic expressions. The presence of muscle fibers within SMAS fibrous septa is hypothetical. The present study analyzed SMAS fibrous septa composition for the existence of striated muscle cells.
METHODS: Histological serial sections of the sample borders (n=107) of 19 in sano-resected and diagnosed cutaneous tumors of the midfacial region were investigated. Immunohistochemical (actin and myosin) and h

BACKGROUND: The exact anatomical entity behind the term superficial musculoaponeurotic system (SMAS) remains poorly understood. The different interpretations of the term SMAS by anatomists, surgeons, and histologists have caused confusion. This article aims to provide clarity regarding this term and the relevant anatomy.
METHODS: A literature review was conducted to uncover the variety of descriptions of the term SMAS. A feasibility study, followed by a conclusive series of standardized layered dissections complemented by histologic analysis and sheet plastination, was performed on 50 cadaver

The superficial musculoaponeurotic system (SMAS) was advocated by Mitz and Peyronie in 1976. The concept of this superficial fascia was established by surgical findings of facelift surgery and is familiar to plastic surgeons and anatomists. However, detailed characteristics of this fascia are still not widely known among head and neck surgeons. Moreover, the SMAS is generally located at the parotid and cheek regions and divides facial fat into superficial and deep layers. The SMAS connects to the superficial temporal fascia cranially and to the platysma caudally. The frontal muscle and the per

As a facial plastic surgeon gains experience, further improvements can be gained with more aggressive surgery, but complications begin to occur when more aggressive measures are undertaken. Therefore, the ideal technique is one that maximizes rejuvenation while minimizing adverse effects. The senior author has found that the aggressive techniques in the region of the neck have improved dramatically the overall initial and long-term results for the neck portion of the rhytidectomy. More aggressive treatment of the midface during the modified deep plane rhytidectomy does not necessarily improve

This article addresses several facelift challenges involving anatomic conditions, including platysma banding, endomorphic facial habitus, and midface hypoplasia. In addition, patient counseling and conveying realistic expectations about limitations of facelift alone, with and without adjunctive procedures, are presented. In addition, a few technical modifications of the facelift procedure contributing to more uniform success and longevity are discussed.

The focus of modern face-lifting has shifted from isolated superficial musculoaponeurotic system (SMAS) manipulation to providing necessary volume restoration and overall facial shaping. Volume deflation is a major component of facial aging but cannot be corrected solely by rhytidectomy. This article presents a detailed overview of facial fat compartment anatomy and examines its role in facial rejuvenation. A good understanding of facial fat compartment anatomy cannot be overemphasized. In combination with methodical preoperative visual analysis, this allows surgeons to target the specific are

We introduce the expanded polytetrafluoroethylene (ePTFE) superficial musculoaponeurotic system extension graft (SEG) as an effective method of lower face-lift and an alternative to the minimally invasive lower face-lift and thread-lift. During a 4.5-year period, 450 lower face-lifts with ePTFE SEG were performed. The procedure consisted of a primary ePTFE grafting operation followed by a secondary ePTFE SEG traction operation 3 to 4 weeks later; the skin flap was then excised and redraped to adjust the postauricular sulcus. No major complications resulted from surgery, but minor complications

A standard bayonet-shaped incision with no reconstruction of the parotid bed is usually used for removal of parotid tumours. Its disadvantages are often an obvious cervical scar, a conspicuous shallow contour around the angle of the mandible, and gustatory sweating. We have studied 17 patients with benign parotid tumours, who were treated by superficial parotidectomy with a combination of a face-lift approach and a superficial musculoaponeurotic system (SMAS) advancement flap. The patients were followed up every 6 months for 3 years. All patients were satisfied with the cosmetic outcome, and t

BACKGROUND: A clear understanding of the superficial musculoaponeurotic system (SMAS) perfusion during extended SMAS rhytidectomy is helpful for surgical planning and for elucidating the structural characteristics of the SMAS.
OBJECTIVES: The aim of this study was to determine the vascularity of the SMAS during extended SMAS rhytidectomy, as assessed using indocyanine green angiography (ICGA), and to discuss the clinical relevance of the SMAS perfusion pattern for surgical practice.
METHODS: A total of 20 patients who underwent extended SMAS rhytidectomy were evaluated intraoperatively. The

BACKGROUND: Although artificial intelligence (AI) is implemented in diagnostic medicine, few modalities exist to evaluate aesthetic outcomes. The literature demonstrates AI's ability to predict patient age mainly from frontal views, despite the importance of profile views in age perception. It is also uncertain how the predominant facelift techniques (deep plane versus SMAS) and adjunct procedures affect AI estimation. We aimed to investigate AI's ability to estimate age pre- and postoperatively using frontal and profile views, comparing outcomes across techniques and adjuncts.

OBJECTIVE: To investigate the outcomes of superficial musculoaponeurotic system (SMAS) flap and classic techniques in superficial parotidectomy in terms of Frey's syndrome (FS) and cosmetic satisfaction.
METHODS: In this study, a retrospective chart review of patients that underwent superficial parotidectomy was performed. These patients were divided into two subgroups: group 1 included patients in which the SMAS flap was harvested and group 2 comprised the remaining patients on whom classic superficial parotidectomy was performed. All the patients were evaluated clinically and with Minor's s

BACKGROUND: Facial aging is a multifactorial process that results from deflation and descent of facial soft tissue. Modern deep layer face-lift techniques aim to restore a youthful facial shape by using the superficial musculoaponeurotic system (SMAS) to reposition descended facial fat into areas of deflation within the lateral cheek and malar eminence. Although there are many advocates of various SMAS techniques and numerous descriptions of methods to manipulate and fixate repositioned facial fat, there is no consensus in the literature as to a "best" SMAS technique. The authors' investigatio

BACKGROUND: The superficial musculoaponeurotic system (SMAS) is an important structure to understand for face-lift surgeries. However, the detailed distribution and imaging findings of the SMAS have not been established yet.
OBJECTIVES: The aim of this study was to establish the accurate distribution and computed tomography (CT) imaging findings of the SMAS based on cadaveric studies.
METHODS: This study was designed in a 3-step procedure. First, 7 cadaver faces were utilized to understand the distribution and histology of the SMAS. Second, the SMAS findings from the cadaveric studies were c

BACKGROUND: The superficial musculoaponeurotic system (SMAS) is an anatomical structure involved in the facial ageing process. We aimed to evaluate the SMAS laxity using multi-detector row computed tomography (MDCT) as a diagnostic tool, and to analyse the relationship between SMAS laxity and age.
METHODS: Fifty-seven female subjects (aged 21-57 years) were enrolled in the study. The subjects were divided into two age groups: one below the mean age (Group Y, 30 subjects) and one over it (Group O, 27 subjects). The Posture Change (PC)-induced SMAS Laxity Index was measured using reconstructed
