Computer-assisted surgery in the frontal and maxillary sinus.

We report the intraoperative use of bent probes for 3-dimensional computer-assisted navigation in ENT surgery with the ISG Viewing Wand. Probe calibration is standardized and the new probes comply with the overall application accuracy of the navigation system. The bent probes permit an easy access to the remotest locations of the maxillary and the frontal sinus and are beneficial for application in paranasal sinus surgery. We present our clinical experiences with these probes.

Computer-assisted surgical navigation in revision endoscopic sinus surgery.

The anatomical variations of surgical landmarks associated with revision endoscopic sinus surgery (ESS) represent a significant challenge even to the most experienced surgeon. Multiple studies have demonstrated higher rates of both minor and major complications associated with revision cases. In the past decade, the availability of multiple computer-assisted surgical navigation systems has resulted in its widespread adoption as a tool in ESS.Computer-aided surgery (CAS) allows for triplanar anatomic localization, which may improve spatial orientation and help avoid trauma to vital structures,

Stereo reconstruction from microscopic images for computer-assisted ophthalmic surgery.

PURPOSE: This work presents a novel platform for stereo reconstruction in anterior segment ophthalmic surgery to enable enhanced scene understanding, especially depth perception, for advanced computer-assisted eye surgery by effectively addressing the lack of texture and corneal distortions artifacts in the surgical scene. METHODS: The proposed platform for stereo reconstruction uses a two-step approach: generating a sparse 3D point cloud from microscopic images, deriving a dense 3D representation by fitting surfaces onto the point cloud, and considering geometrical priors of the eye anatomy.

Computer-assisted surgery and planning in percutaneous pelvic screw fixation.

Percutaneous pelvic screwing (PPS) enables fixation of traumatic or atraumatic fractures with little or no displacement, or displaced but reduced fractures, and preventive fixation of primary or secondary tumoral lesions. It is a relatively recent technique, and indications are evolving with progress in pre- and intra-operative imaging. Morbidity is lower than with open surgery. PPS is classically performed under fluoroscopy; computer-assisted surgery is of great interest, enabling analysis of safe bone corridors. Planning is based on image processing tools included in CT DICOM viewer packages

[Computer-assisted surgery: automated screw placement in the vertebral pedicle].

AIM OF THE STUDY: Previous studies of conventional surgical procedures have shown a significant rate of incorrect pedicle screw placement ranging from 10 to 40%. Transpedicle screw insertion may cause three types of complications: neurologic, vascular and mechanical. The aim of this prospective study is to improve the reliability of pedicle screwing with computer assistance. MATERIAL AND METHODS: A new and original technique that combines preoperative computed tomography imaging with intraoperative passive navigation has been used to perform 48 pedicle screwings in the thoracolumbar region. [Curved osteotomy technique for intertrochanteric varus surgery--an experimental and computer-assisted surgery].

PURPOSE: In conventional intertrochanteric varus osteotomy, shortening of the leg and insufficiency of the abductor muscles occur frequently. To avoid these disadvantages, a curved osteotomy is presented. METHOD: 189 intertrochanteric barrel vault and wedge osteotomies were compared. By means of digitized preoperative radiographs the varus osteotomies were planned with a computer program optimising postoperative leg length, bone contact area at the osteotomy site, femoral offset, and abductor muscle length. RESULTS: The average leg shortening was 3.9 mm (range: 0-5 mm) for barrel vault osteo

Advances and innovations in computer-assisted head and neck oncologic surgery.

Over the past years, computer-assisted surgery has gained more importance in craniomaxillofacial surgery, especially in primary and secondary treatment of head and neck malignancies. The basis for oncologic treatment of the head and neck region requires detailed planning using computed tomography, cone-beam computed tomography, or magnetic resonance imaging in combination with computer-assisted, infrared-based navigation system. These techniques allow a preplanned image-guided path to the tumor region for taking biopsies, resection, or reconstruction. The aim of this work was to show the advan

Computer assisted localizer for planning of surgery and intra-operative orientation.

There is discrepancy between the exact representation of anatomical structures and tumours in the CT or MRI scan and the more or less accurate intra-operative localisation methods based mostly upon landmarks of the skull and extracerebral space and visible abnormalities of the cerebral surface. To overcome these problems of exact intra-operative localisation a Computer Assisted Localizer (CAL) is presented which allows precise intra-operative orientation without these aids. It consists of a mechanically articulated arm with six degrees of freedom with a high precision digital incremental and a
