HYPOTHESIS: Pilonidal disease is a common chronic disorder of the sacrococcygeal region affecting young people. Although many surgical methods have been suggested, an ideal method is still lacking because of significant morbidity and high recurrence rates. However, the crossed triangular flaps technique, which involves excision and primary closure, with less morbidity and low recurrence rates, appears to meet the criteria for an ideal method.

A 52-year-old man had uneventful bilateral myopic laser in situ keratomileusis (LASIK) with planned monovision. Three months later, the left eye was retreated to improve the near vision. On postoperative day 1, the uncorrected distance visual acuity (UDVA) was 0.95 and the patient reported mild ocular pain. Stage 2 diffuse lamellar keratitis was observed. Despite hourly prednisolone instillation, the UDVA decreased to 0.1 and central flap necrosis syndrome was diagnosed. The flap was lifted and stromal bed irrigation performed. One month later, the UDVA was 0.05 with a hyperopic shift of +3.25

The prepuce free flap was used in 10 oral and oropharyngeal reconstructions. During the course of this study, various modifications took place. Residual penile skin necrosis and skin island necrosis early in the series led to modification of flap design. This solved the donor-site problem by placing the skin island more proximally, to consist of the outer layer of the prepuce and an equidimensional area of penile skin proximal to the prepuce. The Authors report a case of laryngeal stenosis initially treated through cervicotomy with the application of an Eliachar prosthesis and finally resolved, after recurrence, with two Z-shaped mucosa flaps. The Authors feel that this method is worthy of note because: its is simple to perform; provides excellent flap vitality; makes a stent unnecessary; and, above all, provides positive results.

The infrahyoid myocutaneous flap technique was described by Wang in 1986, the skin pad being orientated vertically. Its blood supply comes from the superior thyroid artery. This flap consists of the sternohyoid muscle, the sternothyroid muscle, and the superior belly of the omohyoid muscle. The harvesting of a horizontal skin flap does not modify its reliability and avoids additional scars. The donor site anatomy and flap vascularization are briefly described as well as the flap features and harvesting technique.
