Visual vertigo: symptom assessment, spatial orientation and postural control.

Certain patients with balance disorders report a 'visual vertigo' in which their symptoms are provoked or aggravated by specific visual contexts (e.g. supermarkets, driving or movement of objects). In order to determine the causes of visual vertigo (VV), we assessed symptoms, anxiety and the influence of disorienting visual stimuli in 21 such patients. In 17 out of 21 patients, a peripheral vestibular disorder was diagnosed. Sixteen bilateral labyrinthine-defective subjects (LDS) and 25 normal subjects served as controls. Questionnaire assessment showed that the levels of trait anxiety and chi

Validating the incontinence symptom severity index: a self-assessment instrument for voiding symptom severity in women.

PURPOSE: Most voiding symptom self-assessment instruments assess either symptom bother or effect on quality of life. The Incontinence Symptom Severity Index is an instrument for self-assessment of severity of female urinary storage and voiding symptoms, rather than symptom bother or effects of symptoms on quality of life. We assessed the validity of the Incontinence Symptom Severity Index for female voiding symptom self-assessment. MATERIALS AND METHODS: The Incontinence Symptom Severity Index assesses 8 symptom domains, including emptying, urgency, urge incontinence, nocturia, daytime freque

Testing the feasibility of using the Edmonton Symptom Assessment System (ESAS) to assess caregiver symptom burden

ABSTRACTObjective:Caregiver symptom assessment is not part of regular clinical cancer care. The ESAS (Edmonton Symptom Assessment System) is a multidimensional tool regularly used to measure symptom burden in patients but not caregivers. The objectives of the present study were to determine the feasibility of the ESAS in caregiver completion (defined as ≥ 9 of 12 items) and determine its concurrent validity with the Zarit Burden Interview–12 (ZBI–12).Method:We conducted a prospective study on 90 patient–primary caregiver dyads seen in an outpatient supportive care center in a cancer center. Assessment of reflux symptom severity: methodological options and their attributes.

Despite major advances in our understanding of reflux disease, the management of this disorder still presents many challenges. Reduction of heartburn is the most readily apparent objective for the patient with reflux disease. Thus the ability to measure heartburn accurately is of fundamental importance to clinical research in reflux disease. Here, the available data on the assessment of reflux symptoms--predominantly heartburn--in clinical trials of symptomatic reflux disease are examined.

The Acute Cystitis Symptom Score for Patient-Reported Outcome Assessment.

OBJECTIVES: The Acute Cystitis Symptom Score (ACSS) was developed as a self-reporting questionnaire for clinical diagnosis of acute uncomplicated cystitis (AUC) and symptomatic changes in female patients. The aim of the present study was to assess the utility of the different domains of ACSS including the 'dynamics' domain after therapy. The applicability of these domains in assessing changes in symptoms, as a function of time, in this population was investigated. MATERIALS AND METHODS: During the validation process of the ACSS in Uzbek and Russian languages, a subgroup of patients (n = 63) r

Topographical assessment of symptom resolution following open carpal tunnel release.

PURPOSE: Patients with carpal tunnel syndrome commonly present with paresthesias and pain extending into the small finger and proximal to the hand. This study was conducted to assess the ability of carpal tunnel release to relieve symptoms outside of the median nerve distribution. METHODS: We enrolled 34 consecutive surgical patients (40 wrists) diagnosed exclusively with carpal tunnel syndrome based on the history, physical examination, and electrodiagnostic studies. Preoperative symptoms were categorized as pain, numbness, tingling, or "burning and electrical shocks." The presence or absenc

Validating the Minimal Clinically Important Difference for the Edmonton Symptom Assessment Scale (ESAS) Total Symptom Distress Score (TSDS).

BACKGROUND: The Edmonton Symptom Assessment Scale (ESAS) Total Symptom Distress Score (TSDS) is widely used in palliative care to assess symptom burden. While minimal clinically important difference (MCID) values have been established by Hui and colleagues' validation across different patient populations is essential. OBJECTIVE: To validate the MCID values for the ESAS TSDS using data from two randomized controlled trials of medicinal cannabis in advanced cancer. METHODS: We analyzed data from 186 patients with incurable cancer. The Patient Global Impression (PGI) scale served as the anchor,

Prospective Study of Use of Edmonton Symptom Assessment Scale Versus Routine Symptom Management During Weekly Radiation Treatment Visits.

PURPOSE: During radiotherapy (RT), patient symptoms are evaluated and managed weekly during physician on-treatment visits (OTVs). The Edmonton Symptom Assessment Scale (ESAS) is a 9-symptom validated self-assessment tool for reporting common symptoms in patients with cancer. We hypothesized that implementation and physician review of ESAS during weekly OTVs may result in betterment of symptom severity during RT for certain modifiable domains. METHODS: As an institutional quality improvement project, patients were partitioned into 2 groups: (1) 85 patients completing weekly ESAS (preinterventi

Structured Symptom Assessment in Dermato-Oncology Patients-A Prospective Observational Study of the Usability of a Symptom Questionnaire.

BACKGROUND: A focused anamnesis of symptoms is essential in medical practice, especially in oncology, where new or increasing symptoms often indicate disease progression or therapy side effects. However, there are only a few studies in dermato-oncology that address the question of how symptoms should ideally be recorded. There are no validated questionnaires specifically tailored to the needs of dermato-oncology. PATIENTS AND METHODS: Over a one-year period, all patients at the skin cancer centre were given a standardised questionnaire to record relevant symptoms before consultation. This was

Culture and assessment of manic symptoms.

Cultural background may influence the perception of psychiatric symptoms. We examined the effects of cultural biases on the identification of manic symptoms using the Young Mania Rating Scale. Two video interviews, each with an American person with mania, were shown to psychiatrists from three countries (US, UK and India). Total scores on the scale differed significantly between the US and UK (P<0.001) and between India and UK (P<0.001) rater groups. Overall, differences between India and US rater groups were less marked (P=0.28). These differences suggest that cultural biases influence the in

A questionnaire for the assessment of biliary symptoms.

OBJECTIVES: Gallstone disease is common and causes high health care costs, but a measure of symptomatic biliary disease for outcome studies is lacking. We aimed to develop a reproducible, valid, discriminative, disease-specific measure of biliary symptoms. METHODS: We created the self-report Biliary Symptoms Questionnaire (BSQ) by combining possible biliary symptoms with validated items for gastroesophageal reflux disease (GERD), irritable bowel syndrome (IBS), and other disorders. We developed the final version through an iterative process and assessed reproducibility by the test-retest meth

Depressive and Anxiety Symptom Assessment in Adults with Polycystic Ovarian Syndrome.

BACKGROUND: Polycystic ovarian syndrome (PCOS) is an endocrinopathic disorder commonly affecting women in the reproductive age. These women have a possibility of developing depression and anxiety due to biochemical changes, concerns regarding physical appearance, and social pressure from infertility. Thus, the connection between PCOS, anxiety, and depression has a possible impact on patients' quality of life. This study is aimed at assessing depression and anxiety symptoms among PCOS patients and their association with different socioeconomic aspects. METHODS: A cross-sectional study was cond

Exercise-induced bronchoconstriction in athletes - A qualitative assessment of symptom perception.

BACKGROUND: A poor relationship between perceived respiratory symptoms and objective evidence of exercise-induced bronchoconstriction (EIB) in athletes is often reported; however, the reasons for this disconnect remain unclear. The primary aim of this study was to utilise a qualitative-analytical approach to compare respiratory symptoms in athletes with and without objectively confirmed EIB. METHODS: Endurance athletes who had previously undergone bronchoprovocation test screening for EIB were divided into sub-groups, based on the presence or absence of EIB ± heightened self-report of dyspnoe

[Symptom validity assessment and forensic evaluation of cognitive and mental disorders].

Continuing from two previous reviews in this journal (Fortschr Neurol Psychiatr 2002; 70: 126 - 138; 2007; 75: 140 - 154), modern methods and research on symptom validity assessment are presented, with special emphasis on civil forensic examinations. In spite of continuing resistance from many professionals, symptom validity assessment is gradually gaining ground in German-speaking countries, but there is a developmental lag in European countries as a whole, as compared to developments in North America. Major research lines are presented, and questionnaire measures for the detection of a negat
