REM sleep behavior disorder and synucleinopathies.

Rapid Eye Movement (REM) Sleep Behavior Disorder (RBD) is a somnipathy characterized by a physiological failure of skeletal muscle atonia during REM sleep. Such a brainstem pathway disruption, which directly affects the pontomedullary reticular formation, disrupts the descending inhibitory pathways that normally cause muscle paralysis. Patients then are much more likely to act out their dreams and will have complex, often violent dream-enactment behaviors that may cause injury and disrupt sleep continuity. Almost two decades of research have shown that RBD is potentially a crucial early indica

Enteric Pathologic Manifestations of Alpha-Synucleinopathies.

BACKGROUND: Gastrointestinal (GI) symptoms are common in Parkinson disease (PD), often preceding neurological manifestations; however, early diagnostic utility of GI biopsies remains controversial. Studies suggest aberrant deposition of alpha-synuclein (α-syn) follows step-wise progression in central nervous system though histologic interpretation of normal and aberrant staining patterns have shown variable results. This study examines whether GI α-syn mRNA expression combined with standard α-syn immunohistochemical staining enhance the role of GI biopsy in PD. MATERIALS AND METHODS: Four gro

Neuropathology of autonomic dysfunction in synucleinopathies.

The synucleinopathies-Parkinson's disease, dementia with Lewy bodies, multiple system atrophy, and pure autonomic failure-result from distinct patterns of abnormal α-synuclein aggregation throughout the nervous system. Autonomic dysfunction in these disorders results from variable involvement of the central and peripheral autonomic networks. The major pathologic hallmark of Parkinson's disease and dementia with Lewy bodies is Lewy bodies and Lewy neurites; of multiple system atrophy, oligodendroglial cytoplasmic inclusions; and of pure autonomic failure, peripheral neuronal cytoplasmic inclusi

Glucocerebrosidase is shaking up the synucleinopathies.

The lysosomal enzyme glucocerebrosidase, encoded by the glucocerebrosidase gene, is involved in the breakdown of glucocerebroside into glucose and ceramide. Lysosomal build-up of the substrate glucocerebroside occurs in cells of the reticulo-endothelial system in patients with Gaucher disease, a rare lysosomal storage disorder caused by the recessively inherited deficiency of glucocerebrosidase. Gaucher disease has a broad clinical phenotypic spectrum, divided into non-neuronopathic and neuronopathic forms. Like many monogenic diseases, the correlation between clinical manifestations and molec

Progressive supranuclear palsy phenotype mimicking synucleinopathies.

BACKGROUND: Atypical parkinsonian syndromes are currently divided into two groups based on their pathological appearance: synucleinopathies and tauopathies. Based on recent clinico-pathological studies it is increasingly clear, that some pathological characteristics are shared by both groups. STUDY OBJECTIVE: To describe two pathologically proven cases of tauopathy manifesting in vivo in two typical synucleinopathy phenotypes: multiple system atrophy and dementia with Lewy bodies. PATIENTS AND METHODS: There were 67-year-old woman with a phenotype of multiple system atrophy and a 70-year-old

The Glymphatic System in α-Synucleinopathies.

α-Synucleinopathies-including Parkinson's disease (PD), dementia with Lewy bodies (DLB), and multiple system atrophy (MSA)-are neurodegenerative disorders characterized by the pathological accumulation of misfolded α-synuclein. While current treatments remain largely symptomatic, emerging evidence suggests that impaired clearance of toxic proteins may underlie disease progression. The glymphatic system, a brain-wide network facilitating cerebrospinal fluid (CSF)-mediated waste removal through perivascular pathways and astrocytic aquaporin-4 (AQP4) channels, has gained attention as a key regula

Positron emission tomography tracers for synucleinopathies.

Synucleinopathies, such as Parkinson's disease, dementia with Lewy bodies, and multiple system atrophy, are characterized by the aggregation of α-synuclein. Variations in brain distribution allow for differentiation among these diseases and facilitate precise clinical diagnosis. However, distinguishing between synucleinopathies and Parkinsonism with tauopathies poses a challenge, significantly impacting clinical drug development. Therefore, molecular imaging is crucial for synucleinopathies, particularly for clinical diagnosis, assessment of drug efficacy, and disease surveillance. In recent y

Evolving insights into erythrocytes in synucleinopathies.

Synucleinopathies, including Parkinson's disease (PD), multiple system atrophy (MSA), and dementia with Lewy bodies (DLB), are characterized by neuronal loss accompanied by α-synuclein (α-syn) accumulation in the brain. While research conventionally focused on brain pathology, there is growing interest in peripheral alterations. Erythrocytes, which are rich in α-syn, have emerged as a compelling site for synucleinopathies-related alterations. Erythrocyte-derived extracellular vesicles (EVs), containing pathological α-syn species, can traverse the blood-brain barrier (BBB) under certain conditi

Neurogenic Orthostatic Hypotension. Lessons From Synucleinopathies.

Maintenance of upright blood pressure critically depends on the autonomic nervous system and its failure leads to neurogenic orthostatic hypotension (NOH). The most severe cases are seen in neurodegenerative disorders caused by abnormal α-synuclein deposits: multiple system atrophy (MSA), Parkinson's disease, Lewy body dementia, and pure autonomic failure (PAF). The development of novel treatments for NOH derives from research in these disorders. We provide a brief review of their underlying pathophysiology relevant to understand the rationale behind treatment options for NOH. The goal of trea

Neuropathology and molecular diagnosis of Synucleinopathies.

Synucleinopathies are clinically and pathologically heterogeneous disorders characterized by pathologic aggregates of α-synuclein in neurons and glia, in the form of Lewy bodies, Lewy neurites, neuronal cytoplasmic inclusions, and glial cytoplasmic inclusions. Synucleinopathies can be divided into two major disease entities: Lewy body disease and multiple system atrophy (MSA). Common clinical presentations of Lewy body disease are Parkinson's disease (PD), PD with dementia, and dementia with Lewy bodies (DLB), while MSA has two major clinical subtypes, MSA with predominant cerebellar ataxia an

Alpha-synuclein seed amplification assays differentiate synucleinopathies.

INTRODUCTION: Synucleinopathies are characterized by the misfolding and aggregation of α-synuclein (α-Syn) into pathogenic strains that seed Lewy-body pathology in neurons and/or glial cytoplasmic inclusions in oligodendrocytes. α-syn seed-amplification assays (α-Syn SAAs) detect as little as 20 femtogram of synthetic α-Syn pre-formed fibrils (PFFs) or analogous synthetic aggregates in biospecimens, offering high sensitivity and specificity for synucleinopathies. AREAS COVERED: We review how distinct α-Syn strains propagate in Parkinson's disease (PD), dementia with Lewy bodies (DLB), and mul

PET tracer development for imaging α-synucleinopathies.

Abnormal α-synuclein aggregation is a key neuropathological hallmark of α-synucleinopathies, such as Parkinson's disease (PD), multiple system atrophy (MSA), and several other neurological disorders, and closely contributes to pathogenesis. The primary characteristics of α-synucleinopathies are selective targeted neurodegeneration and the accumulation of Lewy pathologies. Specifically, α-synuclein positron emission tomography (PET) radiotracers target the fibrillar forms of the protein, thus enhancing early diagnosis and the evaluation of treatment effectiveness for various α-synucleinopathies

Evaluation of Valsalva Maneuver in Alpha-Synucleinopathies.

Parkinson's disease (PD) and multiple system atrophy (MSA) are neurodegenerative diseases that not only cause motor symptoms but also impair the autonomic nervous system. This study aims to characterize the response to Valsalva maneuver (VM) in individuals affected by PD and MSA by analyzing the systolic arterial blood pressure (SBP) and heart rate (HR) signals. During the maneuver SBP and HR signals are highly nonstationary, therefore we decided to adopt a multi-level uniform phase empirical mode decomposition (UPEMD), a modification of empirical mode decomposition (EMD), together with standa

Fluid markers of synapse degeneration in synucleinopathies.

The abnormal accumulation of α-synuclein in the brain is a common feature of Parkinson's disease (PD), PD dementia (PDD), dementia with Lewy bodies (DLB) and multiple system atrophy (MSA), and synucleinopathies that present with overlapping but distinct clinical symptoms that include motor and cognitive deficits. Synapse degeneration is the crucial neuropathological event in these synucleinopathies and the neuropathological correlate of connectome dysfunction. The cognitive and motor deficits resulting from the connectome dysfunction are currently measured by scalar systems that are limited in

Neurons and Glia Interplay in α-Synucleinopathies.

Accumulation of the neuronal presynaptic protein alpha-synuclein within proteinaceous inclusions represents the key histophathological hallmark of a spectrum of neurodegenerative disorders, referred to by the umbrella term a-synucleinopathies. Even though alpha-synuclein is expressed predominantly in neurons, pathological aggregates of the protein are also found in the glial cells of the brain. In Parkinson's disease and dementia with Lewy bodies, alpha-synuclein accumulates mainly in neurons forming the Lewy bodies and Lewy neurites, whereas in multiple system atrophy, the protein aggregates

Immunotherapies in Huntington's disease and α-Synucleinopathies.

Modulation of immune activation using immunotherapy has attracted considerable attention for many years as a potential therapeutic intervention for several inflammation-associated neurodegenerative diseases. However, the efficacy of single-target immunotherapy intervention has shown limited or no efficacy in alleviating disease burden and restoring functional capacity. Marked immune system activation and neuroinflammation are important features and prodromal signs in polyQ repeat disorders and α-synucleinopathies. This review describes the current status and future directions of immunotherapie

Survival in synucleinopathies: A prospective cohort study.

OBJECTIVES: Parkinson disease (PD), multiple system atrophy (MSA), and pure autonomic failure (PAF) involve cytoplasmic deposition of α-synuclein and are considered to be synucleinopathies. Approximately 40% of patients with PD, most patients with MSA, and all patients with PAF have neurogenic orthostatic hypotension (OH). This study compared long-term survival in these synucleinopathies. METHODS: In this prospective cohort study, survival data were obtained for 97.6% of 206 referred patients evaluated between 1994 and 2014 (47 PD + OH, 54 PD no OH, 15 cerebellar MSA [MSA-C], 57 parkinsonian

The Pharmacology of Visual Hallucinations in Synucleinopathies.

Visual hallucinations (VH) are commonly found in the course of synucleinopathies like Parkinson's disease and dementia with Lewy bodies. The incidence of VH in these conditions is so high that the absence of VH in the course of the disease should raise questions about the diagnosis. VH may take the form of early and simple phenomena or appear with late and complex presentations that include hallucinatory production and delusions. VH are an unmet treatment need. The review analyzes the past and recent hypotheses that are related to the underlying mechanisms of VH and then discusses their pharma

Pure Autonomic Failure-A Localized Alpha Synucleinopathy with a Potential for Conversion to More Extensive Alpha Synucleinopathies.

Pure autonomic failure (PAF) is an alpha synucleinopathy with predominant involvement of the autonomic ganglia and peripheral nerves. The hallmark clinical feature is orthostatic hypotension. However, genitourinary, sudomotor, and cardiac involvement is also common. Many patients also develop supine hypertension. Almost a quarter of patients can phenoconvert or evolve into Parkinson's disease, multiple system atrophy, and Lewy body dementia in the future. Early severe bladder involvement, higher supine noradrenaline level, early motor involvement, and dream enactment behavior increase the risk
