Bridging Public Health and Health Care for Systems-Level Change: The NYC Coalition to End Racism in Clinical Algorithms, 2021-2023.

The use of race-based clinical algorithms contributes to population-level racial health inequities. To address these inequities, the New York City Department of Health and Mental Hygiene convened 10 health systems in the Coalition to End Racism in Clinical Algorithms (CERCA). Our mixed-methods evaluation showed that health systems implemented algorithm change and provider education and that CERCA members perceived the coalition as a catalyst for systems-level change. CERCA is a replicable model for collaboration between public health and medicine to advance health equity and antiracist policy.

An Ethical Framework for Addressing the Differential Impact of Systemic Racism and Inequities on Type 1 and Type 2 Diabetes Mellitus in Youth in the United States.

BACKGROUND: Inequities in the clinical care and health outcomes of youth and young adults (YYAs) with type 1 diabetes mellitus (T1D) and type 2 diabetes mellitus (T2D) are well-established. Systemic and institutional racism and barriers, as well as implicit biases underlie these inequities in the USA. SUMMARY: This article offers a broad overview and analysis of disparities in clinical care and outcomes among YYAs with T1D and T2D, framed within an ethical context. We argue that achieving ethical care requires centering assessments of patient and family needs within the realities of their liv

The Embedded Influence of Systemic Racism on Interpersonal Violence, Alcohol and Substance Misuse, and Reproductive Health Among American Indian and Alaska Native Communities: Great Plains Metropolitan Area, 2018-2024.

Objectives. To use syndemic theory and system dynamics to describe how systemic racism shapes interrelationships among interpersonal violence, alcohol and substance misuse, and reproductive health among American Indian and Alaska Native communities. Methods. We conducted a community-based system dynamics study in a Great Plains metropolitan area. Using purposive and key informant sampling, we interviewed 110 participants (2018-2024) to develop causal loop diagrams. We aggregated 19 diagrams into a synthesized model, refined it through 5 additional group sessions (n = 28), and applied rigorousl

(Re)imagining the Future of Genetic Counseling: A Reflexive Qualitative Analysis of Sociopolitical Power, Cultural Safety, Systemic Racism, and Comparative Practice in the United Kingdom, Aotearoa New Zealand and, Australia.

Genetic counseling is undergoing a rapid transformation as genomic medicine becomes embedded within mainstream healthcare systems. At the same time, the profession is being challenged to respond to systemic racism, colonial legacies, technological change, and evolving expectations regarding equity and justice. Historically, genetic counseling emerged within twentieth-century medical genetics and was influenced by political, social, scientific, and medical forces that included eugenic ideology, values, and practices. The profession has since evolved substantially toward psychosocial, patient-ce

Violence, policing, and systemic racism as structural barriers to substance use treatment amongst women sex workers who use drugs: Findings of a community-based cohort in Vancouver, Canada (2010-2019).

BACKGROUND: Despite a high prevalence of substance use among women sex workers (SWs), rigorous social epidemiologic data on substance use treatment experiences among SWs remains limited. Given these gaps and the disproportionate burden of criminalization borne by Indigenous SWs, we evaluated (1) structural correlates of unsuccessful attempts to access substance use treatment; and (2) the interaction between policing and Indigenous ancestry on unsuccessful attempts to access treatment among SWs who use drugs. METHODS: Prospective data were from an open community-based cohort of women SWs (2010

Linking racism and homonegativity to healthcare system distrust among young men of color who have sex with men: Evidence from the Healthy Young Men's (HYM) study.

BACKGROUND: Experiences with racism predict healthcare system distrust among people of color, but Black and Latino young men who have sex with men (YMSM) also experience overlapping forms of stigma associated with their sexual identities and behaviors (i.e., homonegativity and internalized homonegativity). These forms of minority stress may drive healthcare system distrust among Black and Latino YMSM but have received far less attention. OBJECTIVES: To examine the associations of racism, homonegativity, and internalized homonegativity with healthcare system distrust among a community sample o

Exposure to Racism and Adverse Pregnancy Outcomes for Black Women: A Systematic Review and Meta-Analysis.

Research suggests that stress due to racism may underlie the disproportionately high rates of adverse pregnancy outcomes experienced by Black women in the US. Study objectives: (1) Identify forms of systemic racism affecting pregnancy outcomes and (2) increase understanding about the role of racism in adverse pregnancy outcomes for Black women. A systematic review was conducted to explore the relationship between systemic racism and pregnancy outcomes for Black women. Incorporating Measures of Structural Racism into Population Studies of Reproductive Health in the United States: A Narrative Review.

Purpose: Black women in the United States face poor outcomes across reproductive health measures-from pregnancy outcomes to gynecologic cancers. Racial health inequities are attributable to systemic racism, but few population studies of reproductive health outcomes integrate upstream measures of systemic racism, and those who do are limited to maternal and infant health outcomes. Advances in understanding and intervening on the pathway from racism to reproductive health outcomes are limited by a paucity of methodological guidance toward this end. We aim to fill this gap by identifying quantita

Structural racism: A concept analysis.

BACKGROUND: Despite the broad agreement that structural racism is problematic, there remains significant confusion as to what structural racism means and how to research it. PURPOSE: Perform a comprehensive concept analysis of structural racism and propose an operational definition. METHODS: Walker and Avant's eight-step, iterative method was used for conducting the concept analysis. DISCUSSION: Structural racism has five defining attributes: oppressive racial ideologies, dynamic state, inverse-related influence, temporality, and a false sense of "racial equity." Structural racism has six a

William A. Hinton (1883-1959): Diagnosing and Confronting Racism in the Medical Profession.

Racism impacts every aspect of medicine, including the careers and lives of Black physicians. The story of William Augustus Hinton (1883-1959), who invented the Hinton Test for syphilis before becoming the first African American full professor at Harvard University in 1949, offers an instructive perspective on the intersection of interpersonal and systemic racism, and personal determination, just over our historical horizon. Yet there are sobering and instructive lessons throughout this history. Hinton had to navigate prejudice throughout his career. Indeed, while there is much to be inspired

Indigenous excellence: confronting racism and complicity in peer review.

Racism in peer review is a common, structurally embedded, and systemic problem. However, it is often framed as isolated acts of individual-level bias. Racism in peer review manifests across interconnected societal, system, and process levels, shaping who participates in knowledge production, what is deemed credible evidence, and whose work is published. This Indigenous-led Perspective examines racism in peer review through a three-layer framework: (1) societal, (2) system, and (3) process (decision-making and interpersonal), situating peer review as one domain within the broader problem of ant

Online Racism, Smoking Motives, and Loneliness Among Black Emerging Adults: The Role of Rumination and Vigilance.

OBJECTIVES: Racism is an important social determinant that disproportionately increases the risk of smoking among Black emerging adults. In today's digital era, Black emerging adults encounter online racism, including online racial discrimination (direct and vicarious) and exposure to racist content on systemic racism and race-related hate crimes. Yet, little is known about the relationship between online racism and smoking motives among Black emerging adults. Thus, we examined whether the associated anticipatory psychosocial costs (rumination, vigilance) would help explain the link between on

The Longitudinal Association between Gendered Racism and Disease Activity among Black Women Living with Lupus.

Gendered racism, rooted in the interlocking injustices of racism and sexism, is a contributor to poor mental health; its association with physical health outcomes, however, is not well understood. This study investigates whether gendered racism is associated with disease activity among Black women living with systemic lupus erythematosus (SLE). Data are from the Vascular Aging, Inflammation, and Stress in African American Women's Health Research Study (VISTA; 2017-2023). Analyses were restricted to Black women with SLE (N = 201). Results revealed more frequent gendered racism experiences were

Latent Patterns of Violence and Racism on Mental Health and Coping Among Black Women.

Black women are underrepresented in interpersonal violence research and studies often overlook the impact of racism on mental health. The current study used person-centered analytic methods to identify latent profiles of Black women's experiences of child maltreatment, adult sexual assault (ASA), racism, and gendered racism. Differences in depression, alcohol use, and discrimination coping strategies were examined across profiles. Data from 425 Black women in the United States revealed a three-profile solution. Profile 1 (Lower Adversity; n = 267) had the lowest scores on all measures. Profile

Giving while grieving: Racism-related stress and psychological resilience in Black/African American registered nurses.

BACKGROUND: Studies have found race-related stress psychologically and physiologically harms members of stigmatized racial groups. However, the stressor is racism, not race. PURPOSE: This study examined the relationship between racism-related stress and psychological resilience in Black/African American nurses. METHOD: This study used a cross-sectional, quantitative, correlational design with two instruments, an investigator-developed demographic questionnaire and a convenience sample. FINDINGS: Participants perceived they have low psychological resilience in stressful situations. With raci

"Because I Was a Criminal and Drug Addict.": Experiences of Anti-Black Gendered Racism and Reproductive Injustice Among Black Pregnant and Postpartum Women with a Substance Use Disorder and Incarceration and Family Policing Histories.

Racism pervades the US criminal legal and family policing systems, particularly impacting cases involving women with a history of a substance use disorder (SUD). Laws criminalizing SUD during pregnancy disproportionately harm Black women, as do family policing policies around family separation. Discrimination within intersecting systems may deter Black pregnant women with a SUD from seeking evidence-based pregnancy and substance use care. This convergent parallel mixed-methods study aimed to illuminate how systemic oppression influenced the lived experiences of Black mothers with a SUD, facing

Stigma as a facilitator of the 2022 mpox outbreak.

PURPOSE OF REVIEW: This review aims at exploring how stigma played a role in the naming of mpox, its classification, and how anti-LGBT+ bias and systemic racism all played roles in facilitating the outbreak. RECENT FINDINGS: The naming of mpox is in flux with recent efforts to change back to monkeypox in the United States, which could exacerbate stigma and discrimination. Anti-LGBT+ bias affected access to mpox testing, treatment, and prevention, but mpox also served as a facilitator of homophobic rhetoric. Systemic racism and colonialism both affect the global response to mpox and could furt

Preliminary Evidence for the Sequentially Mediated Effect of Racism-Related Stress on Pain Sensitivity Through Sleep Disturbance and Corticolimbic Opioid Receptor Function.

Sleep disturbance predicts worse pain outcomes. Because sleep disturbance inequitably impacts Black adults - with racism as the upstream cause - understanding how racism-related stress impacts pain through sleep might help minimize racialized pain inequities. This preliminary study examined sequential mediation of the effect of racism-related stress on experimental pain through sleep disturbance and corticolimbic μOR function in pain-free non-Hispanic Black (NHB) and White (NHW) adults. Participants completed questionnaires, actigraphy, positron emission tomography, and sensory testing. We rep

"Drug Use with Racism…The Reason I Wanted to Do This Study": Perceptions of Race and Racism's Impact on Drug Use among Black Americans Using Opioids.

Background: Several studies link racism with drug use disparities among systemically marginalized populations. However, few invite Black Americans to discuss how they perceive racism's impact on their drug use. Objectives: To examine qualitative accounts from N=40 Black adults reporting non-medical prescription opioid use on their experiences of racism and drug use. Results: A deductive structural tabular thematic analysis informed by Jones's (2000) levels of racism resulted in two themes: (a) Experiences of Racism (subthemes: Denying Experiences of Racism and Endorsing Experiences of Racism)

Racism, Not Race: A Physician Perspective on Anti-Black Racism in America.

In this commentary, which responds to the article "Anti-Black Racism as a Chronic Condition," by Nneka Sederstrom and Tamika Lasege, I draw on my experience as a physician who cares for a largely Black patient population. Physicians are trained to "first do no harm" and strive to treat patients to the best of their abilities. However well intentioned, many of us fall short of this goal and witness the consequences of health inequities that disproportionally impact the lives of Black patients. Recent years have brought increased acknowledgment and understanding of social determinants of health,
