In rodent models of tardive dyskinesia, dopamine-agonist-induced stereotyped behaviors after neuroleptic administration are used to assess presumed striatal postsynaptic receptor hypersensitivity. Early studies provided evidence that neuroleptic pretreatment did alter dopaminergic receptor sensitivity and hence the threshold for amine-induced stereotyped behavior. Later studies showed a dose effect for several neuroleptics. However, no hypersensitivity was seen with thioridazine, which prevented or reversed haloperidol-induced hypersensitivity. Duration appeared to be a risk factor early in bu

Dopa-responsive dystonia, an autosomal-dominant disorder caused by mutations in the guanosine triphosphate (GTP)-cyclohydrolase I gene, is characterized by severe striatal dopamine depletion. Tardive dyskinesia, on the other hand, has often been associated with striatal dopamine overactivity. This article reports on a 44-year-old man with dopa-responsive dystonia who developed tardive dyskinesia on long-term haloperidol therapy. Nigrostriatal dopamine deficiency may be necessary for the development of tardive dyskinesia.

Tardive dyskinesia (TD) is a common, iatrogenic movement disorder affecting many individuals treated with dopamine-receptor blocking agents (DRBAs). Studying treatment of TD can be complex, as the symptoms can be affected by changes in either dosage or type of DRBA, as well as by the variable natural course of the disease. Historically many pharmacological therapies have been studied in TD, finding varying degrees of treatment success. Most recently, the VMAT2 inhibitors valbenazine and deutetrabenazine were rigorously studied in TD in large, phase III clinical trials, and were shown to be ben

OBJECTIVE: Although tetrabenazine is widely used as an effective treatment for chorea and other hyperkinetic movement disorders, it has been associated with rare instances of parkinsonism and acute dystonic reactions. There have been no prior reports of tardive dyskinesia with this drug.
METHODS: Case report.
RESULTS: Generalized choreiform dyskinesia developed after 10 months of conventional-dosage tetrabenazine treatment, persisting for several weeks after the drug was discontinued.
CONCLUSIONS: As might be anticipated from its pharmacological profile, tetrabenazine has the potential to c

The following communication concerns two schizophrenic patients with Tardive dyskinesia (TD) in whom fluctuations in the severity of the dyskinesias were accompanied by changes in the severity of the seborrheic skin lesions. Since seborrheic dermatitis may be associated with increased plasma melanocyte-stimulating hormone (MSH) level, these observations suggest an association between the severity of TD and increased pituitary MSH release. In addition, TD may be associated with hypothalamic-pituitary dysfunction of MSH autoregulation.

Based on the results of two preliminary studies, we concluded that late-developing persistent drug-induced movement disorders are pharmacologically heterogeneous, and this heterogeneity is seen between individual patients (and groups of patients) as well as within body areas of individual patients; dystonic pathology has a distinct and more consistent response to pharmacologic stimulation than does nondystonic tardive dyskinesia (TD); and, although disturbances in dopamine and acetylcholine appear to be involved in these disorders, they do not in all cases exist in functionally opposite relati

Three patients are reported who developed oral-buccal-lingual movements typical of tardive dyskinesia while being treated with stable doses of haloperidol for Tourette syndrome. In each case signs resolved within a number of weeks after medication was discontinued. One of the patients experienced recurrence of the dyskinesia during a challenge with phenothiazine tranquilizers. In each case the dose of haloperidol was well within the usual therapeutic range. A strong family history of movement disorders, including Tourette syndrome, was present in each family.

We review the results of more than 120 studies of the treatment of tardive dyskinesia with noncatecholaminergic agents. The disorder is thought to arise from dopamine receptor supersensitivity brought on by long term neuroleptic-induced receptor blockade. Ironically, neuroleptics are the most consistently effective treatment of tardive dyskinesia. Nevertheless, it would be desirable to treat it with other compounds. The most intensively studied drugs are the cholinergics, including physostigmine, deanol, choline, and lecithin, but their efficacy has been equivocal. Anticholinergics, opiates, a

Tardive dyskinesia remains a poorly understood consequence of neuroleptic treatment for psychiatric illness. To date, there have been no reports in the literature regarding the presence of dyskinetic symptomatology in first degree relatives or other family members. The following two cases reveal the development of abnormal involuntary movement in brothers with similar mental illness and treatment histories, and suggest the importance of examining neuroleptically treated relatives of an affected individual for dyskinesia and may suggest a genetic factor in vulnerability.

Tardive dyskinesia (TD) is a syndrome of involuntary hyperkinetic movement associated with long-term neuroleptic treatment. The syndrome depends on 1. predisposing factors, especially high age and an unknown constitutional vulnerability, and 2. a postsynaptic dopamine receptor hypersensitivity and/or a cholinergic hypofunction developed during prolonged antidopaminergic (neuroleptic) treatment. TE is easily kept in check by continued and intensified neuroleptic treatment, which on the other hand presumably involves an aggravation of the underlying pathological process. On this account the impo

We have investigated the action of pimozide in tardive dyskinesia induced by prolonged administration of phenothiazines. Improvement was recorded in a double blind study of 18 patients treated with maximum tolerated dosage (mean 18.8 mg/day) for 6 weeks. There was no deterioration in the therapeutic action of pimozide over this time. Parkinsonism and sedation were the main adverse effects. They were corrected by reduction of the dose of pimozide, but often recurred so that further adjustments of dosage were necessary. Our findings support the view that tardive dyskinesia is produced by a distu

Because the symptoms of tardive dyskinesia (TD) have an insidious onset and fluctuating nature, and the risk of TD associated with second-generation antipsychotic (SGA) treatment has been underestimated, it has been challenging for clinicians to make an early and accurate TD diagnosis. More patients are at risk of developing this potentially permanent, disabling condition than ever before because of the widespread use of SGAs; therefore, prevention of TD, if possible, is of utmost importance. Clinicians must use reliable screening tools and diagnostic criteria to assess patients for TD, rule o

Tardive dyskinesia (TD), the choreoathetoid movements of fingers, arms, legs, and trunk and irregular stereotypical movements of the mouth, face, and tongue, has been the scourge of antipsychotic medications since the approval of chlorpromazine. TD tends to occur late in treatment and sometimes remains after discontinuation of the antipsychotic medication. With the recent approval of two medications, valbenazine (Ingrezza®) and deutetrabenazine (Austedo®), there are now treatments for this disfiguring consequence of dopamine-blocking medications. The current article distinguishes the movement

Tardive dyskinesia remains a significant, potentially stigmatizing or crippling adverse effect for any patient treated with an antipsychotic medication. While second- and third-generation antipsychotics have exhibited lower annual incidence rates for tardive dyskinesia than classic or first-generation agents, 3.9% versus 5.5%, the estimated incidence rate is only modestly lower. When coupled with the fact that second- and third-generation antipsychotic medications have come to be employed in treating a wider range of disorders (e.g., autism spectrum disorders, mood disorders, personality disor
