The current standard of care for patients with Glioblastoma multiforme (GBM) is surgery, followed by radiation in combination with Temozolomide (TMZ), followed by adjuvant TMZ. The most appropriate length of treatment of adjuvant TMZ after the completion of radiation is unclear. We present the case of a 45 year old female with GBM who received adjuvant TMZ on an experimental study for 24 1-month cycles (150 mg/M(2) 7 days on/7 days off continuously-a higher TMZ dose than standard). After 24 months on study she was stable without GBM regrowth, but presented with a right tonsil and pharynx mass

Glioblastoma multiforme (GBM) is a highly aggressive brain tumor characterized by rapid development of chemoresistance and poor patient survival. Due to the limited efficacy of current therapeutic protocols, alternative approaches are needed. One potential strategy is combined treatment with DNA-damaging and microtubule-targeting agents. This study investigated the biological effects of temozolomide (TMZ) and flubendazole (FLU) co-treatment in cultured GBM cells. Three GBM cell lines (A172, T98G, and U118MG) with different molecular profiles were treated with TMZ, FLU, or their combination. Tr

Survival for patients with aggressive pituitary tumours (APT) and pituitary carcinomas (PC) has significantly improved following the increasing use of temozolomide (TMZ) since the first reports of response in 2006. TMZ was established as first line chemotherapy for APT/PC in the 2018 ESE guidelines on the management of APT/PC. There is no controversy over its use as salvage therapy however there is increasing interest in exploring TMZ use earlier in the treatment algorithm for APT/PC. Overall response rates as reported in systematic reviews are around 40% but stable disease in another 25% illu

Glioblastoma (GBM) is a challenging diagnosis with almost universally poor prognosis. Though the survival advantage of postoperative radiation (RT) is well established, around 90% of patients will fail in the RT field. The high likelihood of local failure suggests the efficacy of RT needs to be improved to improve clinical outcomes. Radiosensitizers are an established method of enhancing RT cell killing through the addition of a pharmaceutical agent. Though the majority of trials using radiosensitizers have historically been unsuccessful, there continues to be interest with a variety of approa

Temozolomide (TMZ), an alkylating agent, is widely used for treating primary and recurrent high-grade gliomas. However, the efficacy of TMZ is often limited by the development of resistance. Recently, studies have found that TMZ treatment could induce autophagy, which contributes to therapy resistance in glioma. To enhance the benefit of TMZ in the treatment of glioblastomas, effective combination strategies are needed to sensitize glioblastoma cells to TMZ. In this regard, as autophagy could promote cell survival or autophagic cell death, modulating autophagy using a pharmacological inhibitor

A combined therapy of the alkylating agent temozolomide (TMZ) and radiotherapy is standard treatment, and it improves the survival of patients with newly diagnosed glioblastoma (GBM). The DNA repair enzyme O(6)-methylguanine-DNA methyltransferase (MGMT) removes the most cytotoxic lesions generated by TMZ, O(6)-methylguanine, establishing MGMT as one of the most important DNA repair mechanisms of TMZ-induced DNA damage. Thus, the expression of MGMT, its activity, and its promoter methylation status are associated with the response of GBM to TMZ, confirming that MGMT promotes clinical resistance

We report a 64-year-old male who presented to our department in 2007 with bitemporal hemianopia secondary to a large recurrent pituitary prolactinoma in the sellar region and metastatic deposits elsewhere in his neuraxis. He underwent a transphenoidal excision of the large pituitary adenoma which was uncomplicated, although he did suffer from diabetes insipidus post-operatively. He had initially been diagnosed with prolactinoma some years before and had undergone multiple craniotomies to debulk recurrent metastatic lesions. As the patient had already undergone radiotherapy in the past, a novel

We report a case of a male in his 60s with a past medical history of glioblastoma status post chemoradiotherapy and surgical resection. He had completed neoadjuvant temozolomide and was 26 days post-chemotherapy on presentation. He presented with fever, weakness, and confusion. Further evaluation revealed cytomegalovirus (CMV) ventriculoencephaltis. He was started on valganciclovir, but his hospital course was complicated by a pulmonary embolism. Repeat imaging showed tumor progression, and he was a poor surgical candidate. This case highlights the complexities of managing patients with opport

Neoadjuvant and adjuvant treatment in lung neuroendocrine tumors (NETs) is a field that has not been explored in-depth, with little information on the impact on disease-free survival. This case study highlights the effectiveness of neoadjuvant treatment with capecitabine plus temozolomide (CAPTEM) in a woman with well-differentiated atypical carcinoid. The patient was asymptomatic at diagnosis and was referred to the outpatient NET clinic at Sotiria Hospital in Athens, following an incidental finding on a chest x-ray. 18F-fluorodeoxyglucose (FDG) PET/CT and 68Ga-Dotatoc PET/CT revealed another

BACKGROUND: Standard therapy for newly diagnosed glioblastoma is radiotherapy plus temozolomide. In this phase 3 study, we evaluated the effect of the addition of bevacizumab to radiotherapy-temozolomide for the treatment of newly diagnosed glioblastoma.
METHODS: We randomly assigned patients with supratentorial glioblastoma to receive intravenous bevacizumab (10 mg per kilogram of body weight every 2 weeks) or placebo, plus radiotherapy (2 Gy 5 days a week; maximum, 60 Gy) and oral temozolomide (75 mg per square meter of body-surface area per day) for 6 weeks. After a 28-day treatment break,

BACKGROUND: Glioblastoma multiforme (GBM) is among the deadliest of brain tumors. While temozolomide (TMZ) is the standard treatment, tumors quickly develop resistance, leading to patient relapse. The blood-brain barrier (BBB) and complex tumor microenvironments further limit drug efficacy.
METHODS: We reviewed literature from 2015 to 2024 via PubMed and Web of Science, focusing on nanoparticle-based strategies designed to overcome TMZ resistance. We specifically analyzed how these carriers tackle mechanisms like MGMT overexpression and drug efflux.
RESULTS: Emerging nanomedicines-including

Malignant pheochromocytomas (PHEOs)/paragangliomas (PGLs) are rare tumors for which clinical outcomes remain poorly defined and therapeutic options are limited. Approximately 27% carry pathogenic germline succinate dehydrogenase (SDHx) mutations; the presence of such mutations has been correlated with response to temozolomide (TMZ). We aimed to investigate the association between germline mutations in SDHx and response to TMZ. We retrospectively identified patients with metastatic malignant PHEO/PGLs treated with TMZ- based chemotherapy at Dana-Farber Cancer Institute between 2003 and 2020. Th

Drug delivery to the brain is challenging because of the low permeability of blood-brain barrier, and therefore, optimum concentration of chemotherapeutics in the target area specifically for glioblastoma, an aggressive brain tumor, opens a new path of research. To achieve the goal, the oral alkylating agent temozolomide was incorporated into niosomes, and the surface was modified with chlorotoxin, a small 36 amino acid peptide discovered from the venom of scorpion Leiurus quinquestriatus. Active targeting using nanosized particles facilitates an increase in the accumulation of drugs in the ce

Malignant glioblastoma multiform (GBM) is the most common primary malignancy of the brain in the U.S. Temozolomide (TMZ) is the cornerstone of management along with surgical resection and radiotherapy. Because of the reduction in the CD4+ lymphocyte count as a side effect of TMZ use, this patient population is under risk for opportunistic infections like Pneumocystis jiroveci. A male patient with newly diagnosed glioblastoma multiform presented with non-productive cough and chest pain. Before presentation, the patient received the standard therapy including surgical resection, radiation and TM

OBJECTIVE: To report pancytopenia caused by temozolomide, a second-generation alkylating agent.
CLINICAL PRESENTATION AND INTERVENTION: A 22-year-old patient presenting with seizures and confusion was seen in the emergency room. Cranial magnetic resonance imaging revealed a mass. After surgery, the patient was diagnosed with glioblastoma multiforme and was given temozolomide at 150 mg/m(2) on days 1 through 5 every 4 weeks. During the last cycle of temozolomide, grade 3 thrombocytopenia persisted. Possible causes of pancytopenia including vitamin B(12) deficiency were investigated.

Glioblastoma remains highly aggressive and has the ability to acquire resistance to Temozolomide, the standard chemotherapeutic agent. Deubiquitinating enzymes are emerged as critical regulators of tumor survival and drug resistance. Our study investigates the functional roles of MGMT, USP5, and USP8 in Temozolomide-resistant glioma, with a primary focus on the regulation of MGMT upon USP5 and USP8 intervention. Temozolomide-resistant cell lines U87 TMZ & LN229 TMZ were developed by treating cells with escalating dosages of upto 400 µM TMZ in U87MG and LN229 upto 200 µM TMZ, respectively. Expr
