Thalamic cholinergic innervation and postural sensory integration function in Parkinson's disease.

The pathophysiology of postural instability in Parkinson's disease remains poorly understood. Normal postural function depends in part on the ability of the postural control system to integrate visual, proprioceptive, and vestibular sensory information. Degeneration of cholinergic neurons in the brainstem pedunculopontine nucleus complex and their thalamic efferent terminals has been implicated in postural control deficits in Parkinson's disease. Our aim was to investigate the relationship of cholinergic terminal loss in thalamus and cortex, and nigrostriatal dopaminergic denervation, on postu

Evidence of thalamic dysfunction in Huntington disease by proton magnetic resonance spectroscopy.

Our objective was to investigate thalamic neuronal dysfunction in patients with Huntington disease (HD). We performed localized single-voxel proton magnetic resonance spectroscopy (MRS) of the thalamus in 22 HD patients and 25 healthy individuals. The mean age of patients was 48.5 years (ranging from 32 to 71 years). Age at onset varied between 20 and 66 years (mean 38.9 years). The expanded CAG repeat ranged from 40 to 52 (mean 45.2) CAGs. The mean age of control group was 35.4 years, ranging from 19 to 67 years. N-acetylaspartate (NAA) relative to creatine (NAA/Cr) values in the thalamus of

Thalamic volume is associated with dopaminergic degeneration and depressive symptoms in Parkinson's disease.

BACKGROUND: The thalamus is a core node of the basal ganglia-thalamocortical circuit implicated in Parkinson's disease (PD) and depression. This study examined associations among thalamic volume ratio (TVR), nigrostriatal dopaminergic degeneration, and depressive symptoms in PD. METHODS: We included 170 PD patients with 4‑year follow‑up data from a prospective cohort. TVR was derived from automated thalamic segmentation; depressive symptoms were assessed using the Geriatric Depression Scale (GDS), and dopamine transporter (DAT) availability was quantified. Multivariable regression, linear mix

Thalamic anastomosis as a potential marker of perioperative ischemic risk in moyamoya disease.

Revascularization surgery is a well-established intervention for both ischemic and hemorrhagic moyamoya disease (MMD). Perioperative ischemic stroke occurs in 5-18% of surgeries, however, evidence-based information for its prevention and related factors remain insufficiently established. The development of collateralization pathways in MMD may play a significant role in optimizing cerebral perfusion as much as possible, therefore, during cerebral revascularization surgery, the tolerance to ischemia may vary depending on the developmental pattern of the preoperative collateral circulation. Thalamic white matter macrostructure and subnuclei volumes in Parkinson's disease depression.

Depression is a common non-motor feature of Parkinson's disease (PD) which confers significant morbidity and is challenging to treat. The thalamus is a key component in the basal ganglia-thalamocortical network critical to the pathogenesis of PD and depression but the precise thalamic subnuclei involved in PD depression have not been identified. We performed structural and diffusion-weighted imaging (DWI) on 76 participants with PD to evaluate the relationship between PD depression and grey and white matter thalamic subnuclear changes. We used a thalamic segmentation method to divide the thala

Abnormal development of thalamic microstructure in premature neonates with congenital heart disease.

Preterm birth is associated with alteration in corticothalamic development, which underlies poor neurodevelopmental outcomes. Our hypothesis was that preterm neonates with CHD would demonstrate abnormal thalamic microstructure when compared to critically ill neonates without CHD. A secondary aim was to identify any association between thalamic microstructural abnormalities and perioperative clinical variables. We compared thalamic DTI measurements in 21 preterm neonates with CHD to two cohorts of neonates without CHD: 28 term and 27 preterm neonates, identified from the same neonatal intensive

Isolated lateral thalamic infarction: the role of posterior cerebral artery disease.

BACKGROUND AND PURPOSE: Lateral thalamic infarction (LTI) is usually caused by small vessel disease (SVD), i.e., occlusion of the deep perforator. However, focal atherosclerotic posterior cerebral artery disease (PCAD) may produce LTI via thrombotic occlusion of the perforator. We aimed to investigate the prevalence of PCAD in LTI and differences in clinical and imaging findings between LTIs associated with PCAD and SVD. METHODS: We retrospectively evaluated 58 consecutive patients with isolated LTI who underwent diffusion-weighted imaging (DWI) and MR angiography (MRA) within 7 days after st

Lyme disease with lymphocytic meningitis, trigeminal palsy and silent thalamic lesion.

We describe a follow-up in a 15-year-old boy with neuroborreliosis diagnosed by clinical symptoms, CSF and serum analysis. MRI revealed a thalamic lesion and an enhancement of the right trigeminal nerve clinically associated with mild hypasthesia in the right maxillary region. Both, clinical symptoms and radiological findings disappeared within 2 months after treatment. Borrelia burgdorferi specific IgM and IgG in CSF and IgG in serum became negative between 6 and 12 months after diagnosis. We show that neuroborreliosis at an early stage may present only with moderate neurological deficits and

Clinical findings in sporadic Creutzfeldt-Jakob disease correlate with thalamic pathology.

The pathogenesis underlying the typical findings in Creutzfeldt-Jakob disease (CJD) such as periodic EEG changes or myoclonus is not fully understood. The thalamus possesses a high density of inhibitory neurones and serves as a crucial pacemaker of rhythmic EEG activity. As inhibitory neurones expressing parvalbumin (PV) are reduced in the cerebral cortex and hippocampus in sporadic CJD (sCJD), we studied the distribution and number of PV-immunoreactive neurones in sCJD thalami in order to determine whether damage to them could account for certain clinical findings. Immuno histochemical analys

Microstructure-informed deep learning improves thalamic atrophy segmentation and clinical associations in multiple sclerosis and related neuroimmunological diseases.

Thalamic atrophy is a sensitive imaging marker of neurodegeneration in multiple sclerosis (MS) and related disorders, though thalamus segmentation remains method-dependent. Quantitative magnetic resonance imaging (qMRI) may enhance thalamic boundary contrast, particularly in the context of deep learning. We benchmarked thalamic segmentations from two atlas-constrained algorithms, FreeSurfer and FIRST, and two deep learning algorithms, DBSegment and MindGlide (an MS-trained model), against ground truth (GT) labels, tested whether quantitative R1 maps improve performance, and evaluated clinical

The morphology of thalamic subnuclei in Parkinson's disease and the effects of machine learning on disease diagnosis and clinical evaluation.

In Parkinson's disease (PD), the thalamus plays an important role in pathogenesis and disease symptoms; however, the morphological changes in thalamic subnuclei have not been clearly investigated. And there are still many challenges in individual PD diagnosis, especially clinical condition evaluations. Structural magnetic resonance imaging (MRI) was performed on 131 PD patients and 69 healthy controls (HC), and the volumes of 25 thalamic subnuclei were evaluated by FreeSurfer and a newly developed thalamus segment algorithm. Then, the individual PD diagnosis and clinical condition prediction w

"Switchboard" malfunction in motor neuron diseases: Selective pathology of thalamic nuclei in amyotrophic lateral sclerosis and primary lateral sclerosis.

The thalamus is a key cerebral hub relaying a multitude of corticoefferent and corticoafferent connections and mediating distinct extrapyramidal, sensory, cognitive and behavioural functions. While the thalamus consists of dozens of anatomically well-defined nuclei with distinctive physiological roles, existing imaging studies in motor neuron diseases typically evaluate the thalamus as a single structure. Based on the unique cortical signatures observed in ALS and PLS, we hypothesised that similarly focal thalamic involvement may be observed if the nuclei are individually evaluated. A prospect

Thalamic volume alterations mediate glymphatic function and cognitive dysfunction in Alzheimer's disease.

INTRODUCTION: Glymphatic dysfunction is implicated in Alzheimer's disease (AD), but its mechanism remains unclear. This study investigated the associations of the analysis along the perivascular space (ALPS) index, brain reserve, and cognitive outcomes across the AD continuum. METHODS: This study enrolled two cohorts from the local hospital (n = 95) and the Alzheimer's Disease Neuroimaging Initiative (ADNI; n = 178). We calculated the diffusion tensor image ALPS to assess the whole-brain glymphatic function and evaluated its associations with cognition and brain structure. RESULTS: A higher

Thalamic Nuclei Atrophy in Type 1 Diabetes Mellitus Across Disease Duration.

BACKGROUND: Thalamic volume loss is one of the most consistent neuroimaging findings in type 1 diabetes mellitus (T1DM). However, the thalamus comprises multiple nuclei with distinct functions, and whether specific nuclei show differential vulnerability remains unclear. PURPOSE: To investigate the characteristics of intra-thalamic nuclear atrophy in T1DM and evaluate its association with disease duration. FIELD STRENGTH/SEQUENCE: 3.0 T/T1-weighted magnetization-prepared-rapid-acquisition-of-gradient-echo (MPRAGE). STUDY TYPE: Prospective. POPULATION: Forty-two subjects with T1DM (18-68 yea

Thalamic nuclei volumes are related to disease stage in patients with amyotrophic lateral sclerosis.

OBJECTIVE: To explore atrophy patterns in thalamic nuclei at different phases of amyotrophic lateral sclerosis (ALS) and determine any correlations between thalamic nucleus volume and either cognitive impairments or motor disabilities. METHODS: We used the King's clinical staging system for ALS to divide 76 consecutive patients with ALS by disease stage. We investigated patterns of thalamic atrophy in the patients and in 94 healthy controls (HCs). Cognitive functions were evaluated with the Mini-Mental State Examination (MMSE), Frontal Assessment Battery, Boston Naming Test, and Auditory Verb

Early Detection of Alzheimer's Disease in Postmenopausal Women Using Thalamic Subnuclear Volumetry.

Alzheimer's disease (AD) and aging are intrinsically interconnected with each other and are mediated by molecular, cellular, and biological systems. In particular, a specific pattern of brain volume atrophy is the most profound risk factor for cognitive impairment, including AD, that is directly linked to aging. Thus, this study aimed to investigate knowledge on the early detection of AD in postmenopausal women, focusing on the volume changes of the subcortical regions, including the thalamic subnuclei, in women with AD vs. postmenopausal women. Twenty-one women with AD and twenty-one postmeno

Thalamic-insomnia phenotype in E200K Creutzfeldt-Jakob disease: A PET/MRI study.

BACKGROUND: Insomnia and thalamic involvement were frequently reported in patients with genetic Creutzfeldt-Jakob disease (gCJD) with E200K mutations, suggesting E200K might have discrepancy with typical sporadic CJD (sCJD). The study aimed to explore the clinical and neuroimage characteristics of genetic E200K CJD patients by comprehensive neuroimage analysis. METHODS: Six patients with gCJD carried E200K mutation on Prion Protein (PRNP) gene, 13 patients with sporadic CJD, and 22 age- and sex-matched normal controls were enrolled in the study. Thalamic Dorsomedial Nucleus Free Water Correlates with Cognitive Decline in Parkinson's Disease.

BACKGROUND: Brain diffusion tensor imaging (DTI) has been shown to reflect cognitive changes in early Parkinson's disease (PD) but the diffusion-based measure free water (FW) has not been previously assessed. OBJECTIVES: To assess if FW in the thalamic nuclei primarily involved with cognition (ie, the dorsomedial [DMN] and anterior [AN] nuclei), the nucleus basalis of Meynert (nbM), and the hippocampus correlates with and is associated with longitudinal cognitive decline and distinguishes cognitive status at baseline in early PD. Also, to explore how FW compares with conventional DTI, FW-corr
