Laboratory diagnosis of community-acquired bloodstream infection in therapeutic pathology.

Community-acquired bloodstream infections (CBSIs) occur in the out-of-hospital setting (44%) and increase the overall mortality from bloodstream infections (BSIs) by 7.2% per year. The development of CBSIs depends on both comorbid and polymorbid diseases and the patients' age. The causes of CBSIs are: respiratory, hepatobiliary gastrointestinal and urogenital tracts and dental interventions. The etiology of CBSIs is characterized by the isolation of coagulase-negative staphylococci (CNS) (32%), E. coli (27%). To investigate community-acquired bloodstream infection in therapeutic patients. The

Community-associated methicillin-resistant Staphylococcus aureus: reconsideration of therapeutic options.

Methicillin resistance, long recognized as characteristic of nosocomial Staphylococcus aureus, has increasingly been identified in community-acquired strains in the past 15 years. The genotypes of community-associated methicillin-resistant S. aureus (MRSA) are different from nosocomial strains, and unlike nosocomial strains, they have a distinctive methicillin-resistance chromosomal cassette (designated type IV), are usually susceptible to multiple classes of antimicrobials other than beta-lactams, carry a distinctive virulence factor (the Panton-Valentine leukocidin), cause mainly skin and so

Severe community-acquired pneumonia: current management and future therapeutic alternatives.

Despite advances in modern medicine, severe community-acquired pneumonia (CAP) continues to be a potentially deadly disease. Mortality rate reaches up to the 50% in patients requiring admission to the Intensive Care Unit (ICU) when developing septic shock. Areas covered: We aim to describe the optimal management of severe CAP, including antibiotic therapy, future antimicrobial options, and non-antibiotic (so-called adjunctive) therapies. A literature search was performed to identify all clinical trials, observational studies, meta-analysis, and reviews about this topic from PubMed. Expert comm

Changes in women's sexual risk behaviors after therapeutic community treatment.

This study examines sexual risk behaviors among 197 women mandated to substance abuse treatment in therapeutic communities. The women's risk behaviors after treatment are compared with their behaviors prior to treatment, and risk behaviors among those who completed treatment are compared with those who did not. The women had a high prevalence of sexual risk behaviors (i.e., multiple partners, unprotected sex, sex trade, not asking partners to use condoms, sex with injection drug users [IDUs]) and relationship violence and conflict before treatment and a significant decrease in unprotected sex,

Attachment and Therapeutic Alliance in Substance Use Disorders: Initial Findings for Treatment in the Therapeutic Community.

Background: There is convincing evidence that individuals suffering from Substance Use Disorder (SUD) often present insecure attachment patterns. In contrast, a strong therapeutic alliance in treatment of SUD has been found to lead to a more positive treatment outcome. However, insecure attachment has been observed to be linked with weaker therapeutic alliance strength. The primary aim of this explorative study was to gain initial insights regarding the influence of attachment and personality characteristics on therapeutic alliance and therapy motivation in SUD patients undergoing treatment at

Intercellular Communication in the Heart: Therapeutic Opportunities for Cardiac Ischemia.

The maintenance of tissue, organ, and organism homeostasis relies on an intricate network of players and mechanisms that assist in the different forms of cell-cell communication. Myocardial infarction, following heart ischemia and reperfusion, is associated with profound changes in key processes of intercellular communication, involving gap junctions, extracellular vesicles, and tunneling nanotubes, some of which have been implicated in communication defects associated with cardiac injury, namely arrhythmogenesis and progression into heart failure. Therefore, intercellular communication player

Counselor emotional exhaustion and turnover intention in therapeutic communities.

Counselor turnover is a significant problem facing substance abuse treatment agencies. Understanding the role of organizational culture in predicting burnout and turnover intention may yield important information on how to address turnover in treatment organizations. Using data collected from 817 counselors employed in a national sample of 253 therapeutic communities (TCs), structural equation modeling was used to estimate the associations between emotional exhaustion, turnover intention, and three measures of organizational culture: centralized decision making, distributive justice, and proce

Therapeutic effects of flunitrazepam in dystonias and torticollis. Preliminary communication.

A new form of clinical treatment is proposed for dystonias and torticollis using flunitrazepam (FN), a powerful agonist of all benzodiazepine receptors of GABA neurons. FN has a specific effect in dystonic patients, specially those in which the hypnotic effect of this drug is absent or diminished, thus suggesting the existence of two different neurochemical categories of dystonias.

Intercellular Communication: Novel Perspectives on Stroke Therapeutic Strategies.

Stroke is one of the leading causes of death and disability globally, with ischemic stroke comprising approximately 87% of all cases. Although reperfusion therapies such as tissue-type Plasminogen Activator (tPA) are currently available, their clinical utility is severely limited by narrow therapeutic time windows and the risk of hemorrhagic transformation. Recent advances have revealed that ischemic stroke is not merely a localized vascular occlusion but a systemic pathological event governed by the Neurovascular Unit (NVU), including neurons, glial cells (astrocytes and microglia), vascular

Are prison-based therapeutic communities effective? Challenges and considerations.

PURPOSE: The purpose of this paper is to detail the impact and efficacy of Her Majesty's Prison and Probation Service ((HMPPS) Therapeutic Communities (TCs) (both democratic and hierarchical). This paper outlines recent developments in the TC literature, to provide readers with an up-to-date overview of the outcomes of prison-based TC treatment, while highlighting the strengths and challenges of this treatment approach. Trends within the research are discussed, and the authors draw attention to any gaps in the current knowledge. DESIGN/METHODOLOGY/APPROACH: This paper uses a narrative literat

How a community-engaged therapeutic garden immersion shapes rural intentions among medical students and strengthens student-community partnerships.

BACKGROUND: Rural workforce maldistribution persists, and short rural immersions that embed students in community life may strengthen rural intentions and deepen understanding of social determinants. This study examined a community-engaged, therapeutic gardening immersion as a novel place-based rural learning context in two Western Australian Wheatbelt towns. METHODS: A mixed-methods design was used. A pre-placement survey of Year 3 medical students (n = 22) captured baseline intentions and preferences. Post-placement, semi-structured interviews were conducted with students (n = 22) and commu

Semantic Networks, Schema Change, and Reincarceration Outcomes of Therapeutic Community Graduates.

BACKGROUND: Therapeutic community (TC) clinical theory assumes that peer interaction forms a framework for social learning that will displace ingrained cognitive schema that underlie substance abuse. There has been no direct test of this hypothesis. METHODS: We analyzed the content of a large corpus of written affirmations (pushups) and corrections (pull-ups) exchanged between 2342 male TC graduates. We encoded the content of the written communications as semantic networks of words, in which words that appear in the same brief document are connected and are referred to as word combinations. L

Modified therapeutic community for co-occurring disorders: single investigator meta analysis.

This paper presents the results of a meta-analysis for a single investigator examining the effectiveness of the modified therapeutic community (MTC) for clients with co-occurring substance use and mental disorders (COD). The flexibility and utility of meta-analytic tools are described, although their application in this context is atypical. The analysis includes 4 comparisons from 3 studies (retrieved N = 569) for various groups of clients with COD (homeless persons, offenders, and outpatients) in substance abuse treatment, comparing clients assigned either to an MTC or a control condition of

Therapeutic choices in the treatment of acute community-acquired bacterial rhinosinusitis.

BACKGROUND: Acute bacterial rhinosinusitis (ABRS) is a common medical problem affecting a large percentage of the population. The disease will most often follow a viral upper respiratory tract infection. METHODS: Appropriately diagnosed, ABRS is best treated with antibiotics, but other medications may play a role in some circumstances. Increasing antibiotic resistance to the common bacteria causing ABRS has occurred in the past 20 years, driven largely by inappropriate or unnecessary antibiotic use. RESULTS: There are many antibiotics available that may be used for the treatment of ABRS. The

Graduates and splitees from therapeutic community drug treatment programs: a comparison.

As greater numbers of drug abusers seek treatment and drug rehabilitation program resources become more and more scarce, selection processes should become based on scientific research rather than on the intuition of a few influential personnel. Many therapeutic community drug treatment programs have a preferential policy for accepting voluntary over court-ordered commitments. Our findings indicate that such a policy discriminates against those individuals who are most likely to succeed in completing their treatment. Our data also indicate that the success of male drug addicts in completing the

Role of pharmaceutical care in therapeutic regimens within the community pharmacy.

The concept of pharmaceutical care (PC) has existed as a professional philosophy for more than 30 years. However, for a long period of time, little had been done for its integration into the regular practice of healthcare provision. The COVID-19 pandemic and the resulting increase in patient influx in the community pharmacies (CP) encouraged the exploration and establishment of new healthcare services provided within the CP. Nevertheless, these services of PC are still novel, and more can be done to expand the community pharmacists' current role in primary healthcare. This can be achieved by i

Modified therapeutic community for homeless mentally ill chemical abusers: emerging subtypes.

This paper is one of a series reporting on a clinical field trial evaluating the efficacy of the modified therapeutic community (TC) approach for the treatment of homeless mentally ill chemical abusers (MICAs). The social and psychological characteristics of the treatment sample were described in an earlier paper; the purpose of the present report was to categorize subtypes of homeless MICA clients to predict with greater accuracy their treatability in modified TCs. An index that consistently correlated with treatment-relevant variables was identified for each of three dimensions; Homelessness
