The content of interleukin-8 (IL-8) in the synovial fluid and its production by blood and synovial fluid mononuclear cells (PBMC and SFMC) was compared in rheumatoid arthritis (RA) and various other inflammatory rheumatic disorders. The study included 125 patients and 20 healthy individuals. We determined the levels of total IgE, IgG and IgM in the sera of HgCl2 and gold sodium thiomalate (GST) treated A.SW, C57Bl/6, and DBA/2 mice. In HgCl2 treated A.SW mice, IgE and IgG levels increased up to 30 times above normal. In C57Bl/6 mice HgCl2 induced a slight IgE increase, but no change in IgG. We report a case of a 52-year-old female with therapy-resistant pemphigus vulgaris. She was treated with prednisone, azathioprine and gold sodium thiomalate. After administration of a total dose of 110 mg gold sodium thiomalate, she suddenly developed pityriasis rose-like drug eruptions and interstitial pneumonia and died despite intensive care including pulse therapy. A 20-year-old man with acquired epidermolysis bullosa of 3 years' duration was treated intramuscularly with gold sodium thiomalate. After a total dose of 1 000 mg gold sodium thiomalate, administered over a period of 9 months, the patient has shown an almost complete remission, without any apparent side effects of the chrysotherapy.

The presence of gold in the kidney of a 58-year-old woman was confirmed by use of ultrastructural and microprobe examinations. A series of animal experiments were performed in which small single doses of gold were administered systemically and intraarticularly in rabbits. Both the colloidal suspension and the soluable salt gold sodium thiomalate (Myochrysine) were used. Selective lesions occurred in the proximal convoluted tubules and the mitochondria appeared to be the target organelle. They contained gold after gold sodium thiomalate but not after colloidal gold.

We report on a 45-year-old patient, who first developed pemphigus vegetans of the Neumann type 9 years ago and who presented with typical manifestations of the disease 6 years after the first exacerbation. On systemic therapy with corticosteroids progression of the disease ceased. Additional application of azathioprine, cyclophosphamide or gold thiomalate, however, did not make it possible to reduce the high corticosteroid dosage without exacerbation of the disease. Two patients had diffuse, reversible pulmonary injury possibly owing to gold sodium thiomalate treatment: a 32-year-old woman with chronic inflammatory arthritis compatible with seronegative rheumatoid arthritis and a 32-year-old man with shoulder arthralgia. We report in detail an experience with irreversible interstitial pneumonitis developed during gold sodium thiomalate therapy. Factors that could influence the unfavourable course of this adverse, usually reversible pharmacological reaction, are analysed, and the importance of early recognition and prompt discontinuation of the drug is emphasised.

PURPOSE: Leishmaniasis is a major public health problem worldwide in many parts of the world. Current anti-leishmanial drugs have only limited clinical efficacy. A patient who had pemphigus vulgaris received therapy with gold thiomalate in addition to dexamethasone. Three weeks after the initiation of parenteral gold the patient developed acute hepatitis. Causes other than gold toxicity were ruled out, and the patient recovered completely after discontinuation of gold therapy. This is the first case of acute hepatonecrosis resulting from gold therapy of pemphigus to be reported in the dermatologic literature.

Gold entered the eye and was accumulated in the aqueous humor, iris-ciliary body, and cornea (measured by flameless atomic absorption spectroscopy) of rabbits given gold sodium thiomalate by subcutaneous infusion for 4 weeks. It was not detected following such exposure in the vitreous humor, lens or retina. The concentration of gold in the cornea was quite high, about 25% of that in the liver. The concentration in the aqueous humor was only about 2.5% of that in the plasma, which is similar to the fraction of gold (5%) that is not bound to plasma proteins.

15N NMR studies of the interaction of 15N cyanide ion with gold(I)-thiomalate (Autm) and gold(I)-thioglucose (Autg) have been carried out at pH* 7.40. The chemical shifts of the two 15N ions containing species Au(C15N)2- and RS-Au-C15N- (where RS- = tm- or tg-) were identified at 265.94 and 260.30 ppm, respectively. From the broadened 15N NMR signals, approximate life times of the RS-Au-CN- species were calculated.
