Outcomes and Complications of Thoracentesis in Hospitalized Patients.

OBJECTIVES: Despite the common performance of thoracentesis, predicting risk for adverse outcomes and abnormal postprocedural findings remains difficult. Although guidelines and experts have suggested that routine postprocedure imaging has low yield, compliance with these guidelines has not been well studied. In addition, previous studies have shown that pleural effusions are associated with high short-term mortality rates, longer hospitalizations, and higher readmission rates, increasing the importance of systematic study of procedural results. We aimed to determine the rate of imaging abnorm

The Impact of Thoracentesis On Postprocedure Pulse Oximetry.

BACKGROUND: Although thoracentesis can offer considerable symptomatic relief to the patient, its physiologic impact on oxygen saturation has not been well established in the literature. This study aimed to evaluate the impact of thoracentesis on postprocedure pulse oximetry (SpO2) in an inpatient population. METHODS: A retrospective study of patients undergoing thoracentesis from January 2012 to November 2017 was performed. Inclusion criteria were age above 18 and thoracentesis performed in an inpatient setting. Records were reviewed for patient demographics, procedure reports, and laboratory

Thoracentesis and the risks for bleeding: a new era.

PURPOSE OF REVIEW: Thoracentesis is a commonly performed procedure throughout the world. Convention dictates that patients should have laboratory values such as international normalized ratio (INR) and platelets corrected or medications that affect bleeding withheld prior to performing this procedure. By transfusing blood products or withholding medications, patients are exposed to risks that are different than but equally if not more significant than the risk of hemothorax from thoracentesis. This review highlights recent studies that suggest the parameters of performing thoracentesis should

Ultrasound-guided thoracentesis: is it a safer method?

STUDY OBJECTIVES: The objectives of this study are as follows: (1) to determine the incidence of complications from thoracentesis performed under ultrasound guidance by interventional radiologists in a tertiary referral teaching hospital; (2) to evaluate the incidence of vasovagal events without the use of atropine prior to thoracentesis; and (3) to evaluate patient or radiographic factors that may contribute to, or be predictive of, the development of re-expansion pulmonary edema after ultrasound-guided thoracentesis. DESIGN: Prospective descriptive study. SETTING: Saint Thomas Hospital, a

Complications following symptom-limited thoracentesis using suction.

BACKGROUND: Thoracentesis using suction is perceived to have increased risk of complications, including pneumothorax and re-expansion pulmonary oedema (REPO). Current guidelines recommend limiting drainage to 1.5 L to avoid REPO. Our purpose was to examine the incidence of complications with symptom-limited drainage of pleural fluid using suction and identify risk factors for REPO. METHODS: A retrospective cohort study of all adult patients who underwent symptom-limited thoracentesis using suction at our institution between January 1, 2004 and August 31, 2018 was performed, and a total of 10

Extra-lobar Pulmonary Sequestration Requiring Intrauterine Thoracentesis.

Congenital lung malformations can result in significant morbidity and mortality in children. Pulmonary sequestration is an uncommon congenital malformation of the lung that can cause complications even in fetal life. We herein present a newborn with extra-lobar sequestration (ELS) that lead to hydrops fetalis necessitating fetal intervention.

Pathophysiology of pneumothorax following ultrasound-guided thoracentesis.

STUDY OBJECTIVES: Pneumothorax following ultrasound-guided thoracentesis is rare. Our goal was to explain the mechanisms of pneumothorax following ultrasound-guided thoracentesis in a setting where pleural manometry is routinely used. METHODS: We reviewed the patient records and procedure reports of 401 patients who underwent ultrasound-guided thoracentesis. When manometry was performed, pleural space elastance was determined. A model assuming dependence of the pleural space elastic properties on respiratory system elastic properties was used to isolate cases with presumed normal pleural spac

Outcomes of Thoracentesis for Acute Heart Failure in Hospitals.

Data on in-hospital outcomes for hospitalizations undergoing thoracentesis (THR) for any cause has been conflicting. For hospitalizations with acute heart failure (HF), however, to date, no study has evaluated the outcomes of THR. Accordingly, our current study addresses this knowledge gap. We analyzed data from the Nationwide Inpatient Sample (2005-14). The study population included all adults (>18 years) with the principal discharge diagnosis of HF and the presence of procedure code for THR. Hospitalizations with pneumonia, acute kidney injury, and co-morbidities such as malignancy, lymphoma

Factors affecting the development of pneumothorax associated with thoracentesis.

This study is a retrospective survey of the variables that may influence the development of pneumothorax after thoracentesis. In a 30-month period, a computer search of hospital records identified 342 thoracenteses, of which 154 were done with conventional techniques by the clinical services, and 188 were done with sonographic guidance. Other factors surveyed included the patients' age, sex, underlying pulmonary disease, and overall clinical condition; the size of the effusion; the type of tap (diagnostic or therapeutic); the amount and type (exudate or transudate) of fluid acquired; and the s

Giant left atrium encountered during right-sided thoracentesis.

Giant left atrium is an uncommon pathology to encounter during bedside chest ultrasound, but is an important structure to be aware of when considering thoracentesis. This cardiac structure could easily be mistaken for loculated pleural fluid. This case also supports growing evidence that expert users can safely perform thoracentesis without completely reversing therapeutic anticoagulation.

Reflections on the applicability of palliative thoracentesis in Primary Care.

Under normal conditions, there is a small volume of fluid in the pleural space as a result of the balance between its inflow from the pleural capillaries and its drainage through the lymphatics. Pleural effusion (PE) will occur when this balance is disturbed. The initial step in its study is to determine whether PE it is a transudate or an exudate. The first is caused by an alteration in the hydrostatic or oncotic pressure of both the pleural capillaries and the pleural space, without any structural damage to the pleura and with a simple differential diagnosis. In the second, there will be an

Subcutaneous and muscle layer seroma complicated with thoracentesis.

Diagnostic thoracentesis is a basic and relatively safe diagnostic method for patients with pleural effusion. However, complications of thoracentesis are rare and not well known because of the low incidence. Herein, we report a case of subcutaneous and muscle layer seroma following thoracentesis.

Sensitivity and complications of thoracentesis and thoracoscopy: a meta-analysis.

BACKGROUND: Thoracentesis and thoracoscopy are used to diagnose malignant pleural effusions (MPE). Data on how sensitivity varies with tumour type is limited. METHODS: Systematic review using PubMed was performed through August 2020 to determine the sensitivity of thoracentesis and thoracoscopy for MPE secondary to malignancy, by cancer type, and complication rates. Tests to identify sources of heterogeneity were performed. Study quality was assessed using Quality Assessment of Diagnostic Accuracy Studies (QUADAS)-2 and National Institutes of Health quality assessment tools. Publication bias

Therapeutic thoracentesis symptoms and activity: a qualitative study.

INTRODUCTION: Malignant pleural effusions (MPEs) commonly occur in patients with advanced cancer. Drainage of fluid is used to relieve symptoms and improve quality of life. OBJECTIVE: To improve our understanding of how therapeutic aspiration affects symptoms and activities in patients with MPE. METHODS: Patients presenting to the Pleural Clinic at Norfolk and Norwich University Hospital with a confirmed or suspected MPE participated in up to three semistructured interviews during their diagnostic/therapeutic pathway. Interviews were analysed using framework analysis by two researchers indep

Should patients be advised not to fly post thoracentesis?

Air travel poses a risk to patients with a pneumothorax due to in-flight pressure changes and guidance is available providing advice on air travel in patients with a pneumothorax. Pneumothorax is a recongnised complication of pleural thoracentesis, however chest radiographs have been shown to have limited sensitivity in diagnosing pneumothoraces and small pneumothoraces may not be recognised. There is, therefore a risk post thoracentesis, of exacerbating an unrecognised pneumothorax by air travel. This case outlines the presentation of a 55 year old lady, with a normal chest radiograph after a

Diagnosis of Burkitt lymphoma in pediatric patients by thoracentesis.

The diagnosis of Burkitt lymphoma by thoracentesis has been rarely reported in the literature, particularly in children. From 1995 to 2004, we diagnosed six pediatric patients with mature B-cell neoplasms using thoracentesis as the initial diagnostic procedure. The cytology, immunophenotyping, and cytogenetic results of the pleural fluid cells were consistent with mature B-cell malignancies. We conclude that thoracentesis for the diagnosis of Burkitt lymphoma in children is safe, fast, and accurate. It should be strongly considered as an initial diagnostic procedure for pediatric patients with

Thoracentesis in patients with hematologic malignancy: yield and safety.

BACKGROUND: Pleural effusions occur in patients with hematologic malignancies, particularly during periods of hospitalization. Thoracentesis is often performed to diagnose infection and to exclude the presence of complicated parapneumonic effusions. The efficacy and safety of thoracentesis in this setting has not been well-studied. DESIGN: Retrospective chart review of hospitalized patients with hematologic malignancies undergoing thoracentesis. The aim of this study was to assess the role of thoracentesis in establishing a diagnosis of infection in this population and to determine the risk o

[Effect of thoracentesis on arterial blood gas tension].

The effect of thoracentesis on arterial blood gas tension was studied in 30 patients with unilateral free pleural effusions of various causes. Arterial blood gases were measured before thoracentesis and at 20 minutes, two hours after the procedure. P(A-a)O2 was determined at the same time. The PaO2, PaCO2 and HCO3 showed significant decrease but the concurrent significant increase of the P(A-a)O2 before thoracentesis. The PaO2 showed increase and P(A-a)O2 decrease but the PaCO2 not changes after thoracentesis 20 minutes and two hours. The results suggested that pleural effusion might result in
