We report a case of bilateral lateral costal branches (LCB) of the internal thoracic artery (ITA). On the left side, the ITA branched from the subclavian artery as a common trunk with the thyrocervical trunk. The left LCB flew into the collateral branch of the fifth intercostal artery after reaching the upper end of the sixth rib and after exiting the left ITA at the upper part of the first rib. The left ITA was disconnected near the second rib because it had been used for coronary artery bypass surgery. The right ITA arose from the anterior surface of the right subclavian artery just after th

Because it is a non-invasive method, Magnetic Resonance Angiography has more and more indications in the thorax, particularly with the help of use of contrast media. Different types of sequences used (Cine-MR, Gradient-echo 2D and Gadolinium enhanced breath-hold and non breath-hold 3D) are described as well as acquisition parameters and reconstructions from natives images. Post-processing techniques (MIP, MPR, 3D Surface Rendering) and their limitations depending upon the region explored are explained. Major indications (congenital anomalies of the great vessels, aneurysms, dissections, post-o

MR imaging has rapidly assumed an important role in the diagnosis of congenital anomalies of the aortic arch and pulmonary arteries. Because it is noninvasive and does not involve the use of ionizing radiation, it is particularly useful in children and in clinical settings that require serial follow-up studies, such as in patients with aortic aneurysm and in postoperative patients. In the evaluation of the pulmonary arteries of patients with pulmonary artery or right ventricular outflow tract obstructive lesions, MR imaging has become a frequently used noninvasive imaging technique for determi

OBJECTIVES: The in situ internal thoracic artery (ITA) is recognized as the best conduit for coronary artery bypass surgery. The ITA-if it is used as an in situ graft-has a much higher late patency rate than any other arterial graft, including a free ITA graft. We sought to determine if the use of the ITA as an in situ/free graft and its storage in preservation solutions, have an effect on endothelial function.
METHODS: The ITA was harvested as either a free or in situ graft in a porcine model. Free grafts were stored in different preservation solutions (saline, Custodiol and Tiprotec [both K

We report a very rare case of the right renal artery originating from the thoracic aorta. A 37-year-old man visited our hospital for follow-up regarding a gallbladder polyp that had been detected incidentally via ultrasonography as part of a clinical survey. Contrast-enhanced computed tomography revealed incidentally that the right renal artery originated from the thoracic aorta at the level of the 11th thoracic vertebra. No excess right renal artery was found arising from the abdominal aorta, and a normal left renal artery was observed. Although this anomaly is very rare, urological and thora

Automated computer-aided analysis of lung vessels has shown to yield promising results for non-invasive diagnosis of lung diseases. To detect vascular changes which affect pulmonary arteries and veins differently, both compartments need to be identified. We present a novel, fully automatic method that separates arteries and veins in thoracic computed tomography images, by combining local as well as global properties of pulmonary vessels. We split the problem into two parts: the extraction of multiple distinct vessel subtrees, and their subsequent labeling into arteries and veins. Subtree extra

STUDY DESIGN: Observational study using retrospective contrast-enhanced computed tomography (CT) analysis.
OBJECTIVE: This study aimed to investigate the anatomical characteristics and variations of the thoracic segmental arteries.
SUMMARY OF BACKGROUND DATA: Few cadaver studies of segmental arteries to the thoracic spine have been reported; however, no previous studies have reported the use of contrast-enhanced CT in the upper to middle thoracic spine. Detailed anatomical information of the thoracic segmental arteries is essential to avoid vascular injuries in thoracic spine surgery, such a

BACKGROUND: Little is known about the impact of renal failure on atherosclerotic changes in the internal thoracic artery (ITA).
METHODS: A total of 20 consecutive patients on chronic dialysis who underwent coronary artery bypass grafting (CABG) during April 1998 through September 1999 were investigated. The 20 control patients were selected from the same interval to rigorously match risk factors. Atherosclerosis of the ITA collected from each patient was analyzed using the subjective evaluation proposed by Kay and colleagues.
RESULTS: There were no cases of greater than 25% atherosclerotic l

A narrow vascular pedicle on the thoracic roentgenogram is an important sign in the clinical diagnosis of complete transposition of the great arteries (Shaher, 1973a). Rarely a wide vascular pedicle is present in this condition, mainly due to associated lesions. The differential diagnosis in these cases is discussed, based on our experience with 14 cases.

Arterial thoracic outlet syndrome (aTOS) is rarely diagnosed in children. A 15-year-old boy with recent-onset claudication was found to have aTOS on investigation. Bony abnormality other than the cervical rib might also cause aTOS. Enhanced awareness of aTOS as a cause of vascular symptoms in children is warranted.

Thoracic aortopathy, especially aneurysm, dissection, and rupture, is responsible for significant morbidity and mortality. Uncontrolled hypertension and aging are primary risk factors for such conditions, and they contribute to an increase in the mechanical stress on the wall and an increase in its structural vulnerability, respectively. Select genetic mutations also predispose to these lethal conditions, and the collection of known mutations suggests that dysfunctional mechanosensing and mechanoregulation of the extracellular matrix may contribute to pathogenesis and disease progression. BACKGROUND: Endovascular repair of thoracic aortic disease continues to evolve since FDA approval of the first thoracic endograft in the United States in March 2005. Certain anatomic characteristics, including adequacy of arterial access, remain a significant limiting factor in thoracic endovascular repair. In clinical trials evaluating thoracic endografts, vascular access complications occurred in >20% of cases. The purpose of this study is to evaluate the arterial access and the selective use of the ascending aorta for thoracic endograft placement.
METHOD: A retrospective review was perform

OBJECTIVE: In the absence of ischemic events, arterial pathology at the thoracic outlet (TO) is rarely identified because findings of chronic arterial pathology may be masked by symptoms of neurogenic compression. This study describes the clinical presentations and significance of arterial compression at the TO.
METHODS: This was a retrospective analysis of the clinical records and imaging studies of 41 patients with objective findings of arterial compression at the TO. Sixteen were diagnosed from 1990 to 2003, during which 284 patients underwent surgery for TO decompression with selective ar

OBJECTIVES: Arterial complications due to compression of the thoracic outlet are uncommon. The objective of this study was to review our fairly extensive experience with this problem with particular reference to its management.
METHODS: Patients entered into the Vascular Clinic database were reviewed over an 11 year period. Twenty six records were found. In 24 patients the vasculopathy was caused by a cervical rib (complete in 15) and in two by an anomaly of the first rib. In all patients the basic arteriopathy was a fibrous structure with a post-stenotic aneurysm in 13. Seventeen presented w

PURPOSE: This article reviews experience with arterial injury caused by thoracic outlet syndrome. Special emphasis is placed on the influence of athletic or work activities on the axillary-subclavian artery system and the mechanism by which the humeral head compresses the axillary artery and the circumflex humeral arterial branches.
METHODS: Retrospective review identified 34 patients (age range 13 to 67 years) treated for upper extremity symptoms or ischemic complications of thoracic outlet syndrome from 1983 to 1993. Evaluation included assessment of occupational and recreational activities

Arterial thoracic outlet syndrome (TOS) is a condition in which anatomic abnormalities in the thoracic outlet cause compression of the subclavian or, less commonly, axillary artery. Patients are usually younger and typically have an anatomic abnormality causing the compression. The condition usually goes undiagnosed until patients present with signs of acute or chronic hand or arm ischemia. Workup of this condition includes a thorough history and physical examination; chest x-ray to identify potential anatomic abnormalities; and arterial imaging, such as computed tomographic angiography or dup

Thoracic outlet syndrome (TOS) most commonly manifests in overhead athletes (e.g., baseball pitchers, swimmers, weight lifters) due to nerve compression caused by skeletal abnormalities. We present the case of a 43-year-old recreational runner with unilateral upper extremity pain while running. Vascular imaging identified an aberrant subclavian artery origin with positional compression in the absence of cervical bone anomalies confirming arterial TOS. A first rib resection and anterior scalenectomy led to symptom resolution. This case emphasizes the importance of a broad differential and compl
