Nerve injuries about the shoulder in athletes are being recognized with increasing frequency. Prompt and correct diagnosis of these injuries is important to treat the patient and to understand the potential complications and natural history, so as to counsel our athletes appropriately. This 2-part article is a review and an overview of the current state of knowledge regarding some of the more common nerve injuries seen about the shoulder in athletes, including long thoracic nerve, spinal accessory nerve, burners and stingers, and thoracic outlet syndrome. Each of these clinical entities will b

UNLABELLED: The sympathetic nervous system controls and regulates the activities of the heart and other organs. Sympathetic nervous system dysfunction leads to disease. Therefore, intraoperative real-time imaging of thoracic sympathetic nerves (ITSN) would be of great clinical significance for diagnosis and therapy. The aim of this experimental study was to evaluate the feasibility and validity of intraoperative ITSN using indocyanine green (ICG).
METHODS: ITSN using ICG was performed on 10 rabbits to determine its feasibility. Animals were allocated to two groups. The rabbits in one group re

Neurons in spinal segments T1-T3 of cats projecting to the region of the nucleus reticularis gigantocellularis were tested for their responses to electrical stimulation of the vagus nerve. The effects of vagal efferent activity were blocked with methylatropine, thereby limiting vagal input to the effects of afferent fibers. Seventeen spinoreticular (SR) neurons were inhibited by vagal stimulation; one was excited; four were both inhibited and excited, and nine were unaffected. Half of the SR neurons were not spontaneously active; they were excited by somatic stimuli for 20- to 30-s periods in

To improve surgical pain control through cryoablation of intercostal nerves and reduce narcotic usage in patients undergoing open thoracic or thoracoabdominal aortic aneurysm (TAA or TAAA) repair. From 2012 to 2018, 117 patients underwent open repair of TAA or TAAA. Of those patients, 25 (21%) received cryoablation (2016-2018) of their intercostal nerves and 92 (79%) did not (2012-2018). The primary outcome was pain scores and narcotic usage from extubation day 1 to 10 or the day of discharge. The median age (57 years), demographics, and preoperative comorbidities were not significantly differ

BACKGROUND AND OBJECTIVES: The use of ultrasound for in-plane interscalene block shifts needle insertion to a more posterior approach through the middle scalene muscle, when compared with classic nerve stimulator techniques. Branches from the brachial plexus, including the dorsal scapular and long thoracic nerves, are often anatomically located within the middle scalene muscle. The aim of this study was to use ultrasound to identify and characterize the frequency and position of the dorsal scapular and long thoracic nerves located in the middle scalene muscle.
METHODS: We recruited 50 subject

Vascularized nerve transplants can lead to satisfactory functional reconstruction for nerve defects. These include defects following traumatic nerve severance, iatrogenic severance during tumour resection and extensive defects in poorly vascularized transplant sites. No previous description of the long thoracic nerve as a vascularized nerve graft is available. The aim of this study was to demonstrate the anatomic and initial clinical application of such a graft. The long thoracic nerve was dissected in 84 cases to examine its length, diameter, ramification and type of perfusion. On removal of

BACKGROUND: Function of the thoracic sympathetic chain (TSC) reportedly recovers after surgical clips are removed. Hence, this study was designed to study nerve regeneration after unclipping the TSC.
METHODS: The bilateral TSCs of six goats were studied; the goats were separated into three groups (groups I, II, and III) during excision, clipping, and unclipping. During surgery, the TSCs were excised with a scalpel in group I and clipped in groups II and III. In group III, the clips were removed 1 month postoperatively and observed for possible nerve healing for 1 month. All TSCs were examined

BACKGROUND: The anatomy and function of the long thoracic nerve are not fully understood. The purposes of this study were to clarify the anatomy of the long thoracic nerve and to propose a clinical test to assess the function of the upper division of the long thoracic nerve.
METHODS: The long thoracic nerve and the serratus anterior muscle were studied in fifteen fresh cadavera. Six patients had an operation to treat a brachial plexus injury, and the long thoracic nerve was electrically stimulated. The resulting shoulder motion was then observed.
RESULTS: The long thoracic nerve was formed b

The anatomic course of 40 long thoracic nerves was studied in relation to anatomic landmarks and reference lines, that is, the axillary lines and first 2 ribs. After its supraclavicular course, the nerve passes beneath the clavicle within the axillary sheath and then emerges from the axillary sheath. As it passes inferiorly and posteriorly from the point of emergence to the posterior angle of the second rib (that is, the attachments of serratus anterior muscle), it makes a posterior angle of 30.7 degrees +/- 4.3 degrees on average, relative to the anterior axillary line. It then continues to d

The incidence of long thoracic nerve palsy after radical mastectomy has been documented to be approximately 10%. No cases have been reported after the more recent treatment for breast cancer, lumpectomy with axillary dissection. This more recent surgical procedure is customarily followed by aggressive radiation therapy to the remaining breast tissue. This is the first case report of a patient with radiation-induced long thoracic nerve palsy. The patient was a young woman who underwent left breast quadrantectomy and axillary dissection for breast cancer. After radiation therapy, she had isolate

Minute dissection was performed on the ventral primary rami of the first thoracic to first lumbar nerves in two halves of an adult male cadaver in order to determine points of origins of muscular branches from the intercostal nerves. Generally, the muscular branches originated independently, but the branches supplying the intercostalis intimus and internus tended to originate as a common trunk. The muscular branching patterns were compiled in an illustration, and based on these patterns the serial homology of the thoracic and abdominal muscles was proposed. Although the obliquus externus abdom

A 17-year-old patient presented with a long thoracic nerve palsy following an idiopathic onset of weakness to the serratus anterior muscle. With no evidence of recovery 3.5 months following onset of serratus anterior weakness, the patient underwent a thoracodorsal to long thoracic nerve transfer to reinnervate the serratus anterior muscle. Follow-up examination 6.5 years following the nerve transfer revealed no scapular winging, full range of motion of the shoulder and no reported functional shoulder restriction. We conclude that a thoracodorsal to long thoracic nerve transfer results in good

Veterinary radiology & ultrasound : the official journal of the American College of Veterinary Radiology and the International Veterinary Radiology Association2013eng
Brachial plexus avulsions commonly occur in cats due to traumatic injuries involving the shoulder. Ultrasound may be an effective method for detecting injured nerves. Additional applications may include characterization of brachial plexus neoplasms and guidance of anesthetic nerve blocks. Aims of this study were to describe ultrasonographic approaches and the normal appearance of this plexus and other major nerves of the thoracic limb in cats. Eight feline cadavers were used to determine anatomic landmarks, obtain cross-sectional anatomic images of the target nerves, and compare these with ult

OBJECTIVE: To identify the feasibility of the anatomical landmark-guided 'hit-rib' technique for distributing injectate solution in the thoracic lateral cutaneous fascial plane (TLCPLANE) and to evaluate injectate dispersion.
STUDY DESIGN: Prospective, experimental, randomized cadaveric study.
ANIMALS: A group of 10 adult Beagle dog cadavers.
METHODS: In three cadavers, the feasibility of TLCPLANE dispersion of injectate was evaluated using the hit-rib technique. Hypodermic needles (23 or 26 gauge) were inserted perpendicularly through the skin until contact was made with target ribs (fourt

Primary palmar hyperhidrosis (PPH) is a condition characterized by high levels of palmar perspiration in excess of physiological need. The etiopathogenesis of PPH is thought to be related to hyperactivity of the sympathetic system, but the exact mechanism is still obscure. The aim of this study was to observe the ultrastructure of the thoracic sympathetic nerves and measure the expression of neuregulin-1 (Nrg-1) in thoracic sympathetic nerve tissue in patients with PPH relative to control subjects. Samples of T3 sympathetic ganglia were obtained from 58 subjects: 30 PPH patients who underwent

Circumscapular pain is a frequent complaint in clinical practice. The dorsal scapular and long thoracic nerves course through the neck, where they may become entrapped between or within adjacent scalene muscles. Additionally, a high frequency of brachial plexus "piercing" variants have recently been documented, and it is unclear how they influence branching patterns distally along the brachial plexus. In the project reported here we strived to identify and quantify variations in dorsal scapular nerve and long thoracic nerve secondary to brachial plexus piercing variation. Ninety brachial plexu

OBJECTIVE: To describe in detail the characteristics of the travels and relationship of splanchnic nerves, and to provide an anatomical basis for splanchnic nerve block.
METHODS: 1) For 20 thoracic splanchnic nerve anatomies, the relationship between the intersection of the greater splanchnic nerve (GSN) and lesser splanchnic nerve (LSN) and the T12 vertebrae was examined. 2) 6 patients with cancer pain in the epigram, unresponsive to conservative treatment, underwent splanchnic nerve radiofrequency ablation. The procedure was performed with computed tomography guidance, and the patients were

Scapular winging due to long thoracic nerve palsy can occur through traumatic injuries and nontraumatic events. The traditional view is that most patients will achieve spontaneous recovery within 2 years of winging onset. However, there is evidence that points to a less clear-cut natural history, with residual winging, muscle weakness, and fatigability being exhibited in a significant percentage of patients. Reports from proponents of a more proactive approach have shown that the surgical decompression of the long thoracic nerve beyond 12 months, through thoracic, supraclavicular, or combined
