Tube thoracostomy: the struggle to the "standard of care".

Tube thoracostomy for thoracic injuries has been standard for only the last 40 years. Its theoretic roots trace back to World War II, where the goal of treatment was restoration of intrathoracic organ function. Thoracentesis was used to evacuate the hemopneumothorax resulting from chest trauma and that compromised pulmonary function. Experience gained in military and civilian hospitals contributed to the development of tube thoracostomy as an alternative treatment for patients with chest trauma. Progress stalled due to technologic problems and unacceptable complications associated with tube th

Necrotizing fasciitis as a complication of tube thoracostomy.

Necrotizing fasciitis is a serious, rapidly progressive infection of subcutaneous tissue and fascia, most often related to trauma or surgery. We report a case of necrotizing fasciitis which occurred as a complication of tube thoracostomy. Physicians performing tube thoracostomy should be aware of this potential complication, since definitive treatment requires not only antibiotic therapy, but also surgical debridement.

Extensive Subcutaneous Emphysema Post-traumatic Tube Thoracostomy.

Bronchocutaneous fistula (BCF) is a pathologic communication between the bronchus and the subcutaneous tissue. Its diagnosis is made mainly by chest imaging, and bronchoscopy can help in accurately localizing the fistula. Treatment options include conservative and non-conservative approaches. We report a case of iatrogenic bronchocutaneous fistula occurring after traumatic chest tube placement in an 81-year-old man, treated efficiently with conservative management.

Tube thoracostomy: One-off cause of chylothorax.

Chylothorax secondary to thorax drainage is a rare entity, with only five previously reported cases. A patient that sustained injuries after an accident involving a power tool, originally seen at another service, presented with a metal fragment embedded in the 4th intercostal space of the right hemithorax and ipsilateral pneumothorax. The patient underwent thoracic drainage and was referred to our hospital. After one day, the drained fluid became milky and laboratory tests confirmed chylothorax. Videothoracoscopy disclosed compression of the thoracic duct topography by the drainage tube.

Bradycardia after Tube Thoracostomy for Spontaneous Pneumothorax.

We present the case of an elderly patient who became bradycardic after chest tube insertion for spontaneous pneumothorax. Arrhythmia is a rare complication of tube thoracostomy. Unlike other reported cases of chest tube induced arrhythmias, the bradycardia in our patient responded to resuscitative measures without removal or repositioning of the tube. Our patient, who had COPD, presented with shortness of breath due to spontaneous pneumothorax. Moments after tube insertion, patient developed severe bradycardia that responded to Atropine. In patients requiring chest tube insertion, it is import

Tube Thoracostomy Management in the Combat Wounded.

The intent of this study was to characterize the management and subsequent complications of combat injury tube thoracostomies and to determine risk factors for the development of pneumonia (PNA) and retained hemothorax (RH). One hundred fifteen patients with 173 tube thoracostomies met the inclusion criteria and were analyzed. The mean injury severity score was 30.8 + 11.6, 23.5 per cent had traumatic amputations, 49.7 per cent had a hemothorax, and 50.3 per cent had a pneumothorax as indications for tube thoracostomy (TT) placement. Within 24 hours of injury, 89.6 per cent were intubated, the

Complications of percutaneous thoracostomy in neonates and infants.

OBJECTIVE: Percutaneous thoracostomy tubes are widely used in neonates, infants and children. The technique has a low complication rate. Lung perforation by a pigtail catheter is described in a single case report. STUDY DESIGN: This is a multi-center case series of neonates and infants who experienced thoracic organ injury following percutaneous chest tube placement between 2006 and 2015. RESULT: Eleven patients had chest tube-related thoracic organ injury. In six, tubes were placed during resuscitation. Gestational ages ranged from 24+6 weeks to term. Most of the chest tubes were pigtail ca

Chylothorax: a rare complication of tube thoracostomy.

BACKGROUND: Chylothorax resulting from chest tube injury to the thoracic duct is very rare and underreported. OBJECTIVE: The purpose of this case report is to exemplify this rare but potentially significant complication of chest tube thoracostomy. CASE REPORT: An 86-year-old woman presented with sepsis and a massive right pleural effusion; she developed a chylous effusion with the pleural fluid triglyceride level of 158 mg/dL 2 days after a traumatic chest tube insertion. All investigations excluded common causes of non-traumatic chylothorax. The chylothorax improved after fasting and implem

Ulnar neuropathy after tube thoracostomy for pneumothorax.

BACKGROUND: Chest tube placement is a frequently required surgical procedure among patients in the emergency department. Although the procedure is often simple, occasionally there are known complications, including unresolved pneumothorax or hemopneumothorax, extrathoracic placement of the tube, and damage to the intercostal neurovascular bundle. OBJECTIVES: We report an unusual yet noteworthy complication of ulnar neuropathy after chest tube placement for pneumothorax. Awareness of the association between acute ulnar neuropathy and chest tube placement will alert the physician to reposition

Tube thoracostomy-related necrotizing fasciitis: a case report.

Spontaneous pneumothorax is a serious complication of pulmonary tuberculosis that requires immediate treatment. Necrotizing fasciitis is a serious, rapidly progressive infection of the subcutaneous tissue and fascia, most related to trauma or surgery. Here, we report a case of pulmonary tuberculosis with spontaneous pneumothorax. A standard procedure of tube thoracostomy was performed for lung re-expansion. Two days after the tube was removed, necrotizing fasciitis developed from the puncture site. Computed tomography of the chest showed focal thickness with gas formation and loss of the fat p

Morbidity of percutaneous tube thoracostomy in trauma patients.

OBJECTIVES: This prospective study was designed to evaluate the complications of percutaneous tube thoracostomy (PTT) performed for chest trauma in our institution and to determine predictive factors. METHODS: One hundred and thirty-four primary PTTs were performed in 128 patients for blunt (83) and penetrating (45) chest traumas. Failure was defined as undrained hemothorax or pneumothorax, post-tube removal complications and empyema. Univariate and multivariate hazard analyses were used to assess the association between potential risk factors and complications. RESULTS: The overall complica

Complications after emergency tube thoracostomy: assessment with CT.

PURPOSE: To assess the spectrum of complications after emergency tube thoracostomy (TT) and show the role of computed tomography (CT) in detection of these abnormalities. MATERIALS AND METHODS: CT scans, chest radiographs, and clinical data were reviewed in 51 patients (77 tubes) who underwent emergency TT after trauma. CT scans were analyzed for chest tube malposition (CTM) and persistent pneumo- or hemathoraces. RESULTS: The complication most often seen after emergency TT, as demonstrated with CT, was CTM (20 of 77 tubes [26%]). Only seven of the CTMs seen at CT were evident on chest radio

Iatrogenic Horner's syndrome secondary to chest tube thoracostomy.

We report a 22-year-old male who underwent a left thoracostomy and left lower lobectomy for a shotgun wound of the left anterior chest. During surgery, two chest tubes were placed in the left hemithorax at the level of the first and second intervertebral spaces. On the 10th postoperative day, the patient developed a left-sided miosis and ptosis. The diagnosis of a left preganglionic Horner's syndrome was confirmed by pharmacologic testing. The Horner's syndrome was unchanged 3 months after discharge. We conclude that the chest tubes damaged the preganglionic sympathetic fibers of the left orbi

The size paradox: An analysis of tube thoracostomy in trauma.

BACKGROUND: Debate continues over chest tube (CT) size for traumatic hemothorax (HTX) and pneumothorax (PTX). We compared CT failure and opioid use between large-bore chest tubes (LB-CT) and small-bore chest tubes (SB-CT). METHODS: A retrospective study comparing trauma patients with SB-CT (≤14Fr) or LB-CT (≥24Fr) was performed. CT failure includes HTX, PTX, or empyema requiring intervention. Secondary outcomes included opioid use (MME), mortality, and favorable discharge. RESULTS: Of 252 patients, 65.1 % had SB-CT. SB-CT were older with lower ISS. Failure rate was lower for SB-CT (9.2 vs 2

Delayed perforation of the esophagus by a closed thoracostomy tube.

We report on a previously undocumented complication of a trocar-free thoracostomy tube--delayed perforation of a normal esophagus. The complication presented clinically with fever and copious enteric drainage four days after thoracostomy tube insertion. Diagnosis was established by a contrast study of the esophagus. Retrospectively, the postinsertion chest radiograph showed the offending thoracostomy tube tip impinging on the posterior mediastinum, displacing an indwelling nasogastric tube. Early recognition and repositioning of the thoracostomy tube is the key in preventing this rare but seri

FINGER: A Novel Approach to Teaching Simple Thoracostomy.

For decades, most prehospital clinicians have only been armed with needle thoracostomy to treat a tension pneumothorax, which has a significant failure rate. Following recent changes by the US military, more ground and air transport agencies are adopting simple thoracostomy, also commonly referred to as finger thoracostomy, as a successful alternative. However, surgical procedures performed by prehospital clinicians remain uncommon, intimidating, and challenging. Therefore, it is imperative to adopt a training strategy that is comprehensive, concise, and memorable to best reduce cognitive load
