Immune thrombocytopenia.

Immune thrombocytopenia (ITP) comprises a heterogeneous group of disorders characterized by autoimmune-mediated platelet destruction and impairment of thrombopoiesis. ITP may occur in the absence of an evident predisposing etiology (primary ITP) or secondary to a growing list of associated conditions (secondary ITP), and must be differentiated from other causes of thrombocytopenia. This review focuses on primary ITP in adults. The traditional goal of therapy in this population is to achieve a hemostatic platelet count of 30×10(9)/L or above for most patients while minimizing treatment-related

Neonatal thrombocytopenia.

Thrombocytopenia (platelets <150 x 10(9)/L) is one of the most common haematological problems in neonates, particularly those who are preterm and sick. In those preterm neonates with early thrombocytopenia who present within 72 h of birth, the most common cause is reduced platelet production secondary to intrauterine growth restriction and/or maternal hypertension. By contrast, the most common causes of thrombocytopenia arising after the first 72 h of life, both in preterm and term infants, are sepsis and necrotizing enterocolitis. The most important cause of severe thrombocytopenia (platelets

Neonatal thrombocytopenia.

Thrombocytopenia occurs in up to a third of preterm neonates admitted to intensive care units. In these babies, thrombocytopenia typically presents in one of two patterns: early-onset thrombocytopenia occurring within 72 h of birth and late-onset thrombocytopenia which develops after 72 h. Early-onset thrombocytopenia is most commonly caused by disorders associated with placental insufficiency (e.g. maternal hypertension), is mild-moderate, self-limiting and requires no treatment; it is caused by reduced platelet production. Late-onset thrombocytopenia is usually due to bacterial sepsis or nec

Fetal thrombocytopenia.

In idiopathic thrombocytopenic purpura, the low risk of fetal bleeding in the perinatal period does not justify routine cordocentesis. Platelet counts of second newborns correlates well with that of their siblings. In neonatal alloimmune thrombocytopenia, the initial platelet count at cordocentesis in the second trimester is low in the next pregnancy. Weekly high doses of intravenous gammaglobulin to the mother results in a rise of fetal platelet count in approximately 70% of cases and may protect the fetus against intracranial haemorrhage in cases without significant platelet rise. With the e

Lupoid thrombocytopenia.

Lupoid thrombocytopenia is herein defined as a disorder in which patients with thrombocytopenia have a positive Hep 2 antinuclear antibody. Although these patients may have other clinical or laboratory findings consistent with systemic lupus erythematosus, they do not satisfy the revised diagnostic criteria set down by the American Rheumatism Association (1982). In all other respects they have similar findings to patients with idiopathic thrombocytopenic purpura. Six patients are described with their clinical and laboratory features. It is proposed that this disorder should be distinguished fr

Adalimumab-Induced Thrombocytopenia: A Case Report of Drug-Induced Thrombocytopenia.

In this case report, we present a patient with severe thrombocytopenia induced by adalimumab after 4 weeks of treatment for rheumatoid arthritis. This case adds to present literature regarding antitumor necrosis factor-alpha (TNF-α)-induced thrombocytopenia and explores the use of thrombopoietin receptor agonist medications in refractory drug-induced immune thrombocytopenia (DITP). A 57-year-old female with a history of rheumatoid arthritis presented to the emergency department for progressive petechial rash 4 days after her third dose of adalimumab. It was determined that the patient had immu

Viruses and thrombocytopenia.

Thrombocytopenia, characterized by a decrease in platelet count, is a multifaceted clinical manifestation that can arise from various underlying causes. This review delves into the intriguing nexus between viruses and thrombocytopenia, shedding light on intricate pathophysiological mechanisms and highlighting the pivotal role of platelets in viral infections. The review further navigates the landscape of thrombocytopenia in relation to specific viruses, and sheds light on the diverse mechanisms through which hepatitis C virus (HCV), measles virus, parvovirus B19, and other viral agents contrib

Thrombocytopenia in pregnancy.

Hematologists are often consulted for thrombocytopenia in pregnancy, especially when there is a concern for a non-pregnancy-specific etiology or an insufficient platelet count for the hemostatic challenges of delivery. The severity of thrombocytopenia and trimester of onset can help guide the differential diagnosis. Hematologists need to be aware of the typical signs of preeclampsia with severe features and other hypertensive disorders of pregnancy to help distinguish these conditions, which typically resolve with delivery, from other thrombotic microangiopathies (TMAs) (eg, thrombotic thrombo

Infections and Thrombocytopenia.

Febrile patient with thrombocytopenia is commonly encountered by physicians especially during monsoon and perimonsoon period. Infections with protozoa, bacteria and viruses can cause thrombocytopenia with or without disseminated intravascular coagulation. Commonly dengue, malaria, scrub typhus and other rickettsial infections, meningococci, leptospira and certain viral infections present as fever with thrombocytopenia. Occasionally these patients can go on to develop a stormy course with multiorgan dysfunction requiring intensive care unit admission associated with high morbidity and mortality

Thrombocytopenia in pregnancy.

Thrombocytopenia during pregnancy is quite common. Evaluation of blood counts of pregnant women has shown that thrombocytopenia is the second most common haematological problem in pregnancy, after anaemia. While mostly thrombocytopenia has no consequences for either the mother or the foetus, in some cases it is associated with substantial maternal and/or neonatal morbidity and mortality. It may result from a number of diverse aetiologies. Adequate knowledge of these causes will help the clinicians in making proper diagnosis and management of thrombocytopenia in pregnancy. The evaluation of thr

Thrombocytopenia in sarcoidosis.

BACKGROUND: Haematological manifestations in sarcoidosis are uncommon. The prevalence of thrombocytopenia in sarcoidosis is not well assessed. AIM: To describe the main characteristics and outcome of sarcoidosis associated with thrombocytopenia. METHODS: We described 2 personal cases and a complete record of all reports of thrombocytopenia in sarcoidosis was persuaded through a medline multi language computer search from 1972 until now. CASES REPORTS: In the first observation the clinical course was similar to immune thrombocytopenic purpura. Steroids were efficient. In the second, we have

Thrombocytopenia in malaria.

Malaria continues to be a cause of high mortality and morbidity. Imported cases of malaria are increasing in New York City. Yet, New York physicians, when evaluating patients for fever, frequently missed the diagnosis of malaria. We evaluated the role of platelet count for predicting malarial infection. The study included patients seen between 1996 and 2000 in a New York community hospital for fever who had traveled to a malaria-endemic area. Forty patients with malaria were identified. Our study found the sensitivity of platelet count for diagnosing malaria was 100%, and the specificity was 7

Thrombocytopenia in pregnancy.

Thrombocytopenia, classically defined as a platelet count of lower than 150,000/mL, has been observed in 7% to 10% of unselected pregnancies in the past 20 years because platelet counts are included with automated blood cell counters in routine prenatal screenings. Severe thrombocytopenia with a platelet count of lower than 50 x 10(9)/L is rare, occurring in less than 0.1% of pregnancies.

The etiology of thrombocytopenia.

Hemostasis depends on an adequate number of well-functioning platelets in the circulating blood; decreasing platelet levels raise the risk for bleeding. Thrombocytopenia is a laboratory finding, not a diagnosis, and once identified, a detailed history, physical examination, and further laboratory tests are used to establish an etiology. This article reviews the assessment, diagnosis, and treatment of thrombocytopenia.

Congenital thrombocytopenias.

Congenital thrombocytopenias are rare bleeding disorders but must be included in the differential diagnosis when investigating a young infant with chronic thrombocytopenia. Several of these syndromes have associated defects of immune, renal, or skeletal systems in addition to thrombocytopenia. These can be categorized into two groups depending on the presence or absence of bone marrow hypoplasia. The majority of these disorders are associated with a mild bleeding tendency and thus may be missed until the affected individuals experience excessive postoperative or posttraumatic hemorrhage. Splenectomy for thrombocytopenia.

Of 478 patients who underwent splenectomy for hematologic disease between 1947 and 1978, 142 had thrombocytopenia as the dominant indication, while in 32 this was an associated preoperative factor. Of 120 patients with idiopathic thrombocytopenic purpura (ITP) 88% had long-term remission subsequent to removal of the spleen. Ninety percent of the responders had normal platelet counts at the end of 1 week subsequent to the operation. Six of 13 patients with thrombotic thrombocytopenic purpura (TTP) were long-term survivors and were considered cured. Thrombocytopenia, associated with a variety of
