Thromboembolism in Children.

OBJECTIVE: To study pediatric patients who were diagnosed with thrombosis between January 2009 and March 2020. METHODS: Patients were evaluated with respect to thrombophilic risk factors, localization of the thrombus, response to treatment, and recurrence rates during the last 11 y. RESULTS: Among 84 patients, 59 (70%) had venous thrombosis and 20 (24%) had arterial thrombosis. The number of documented thrombosis amongst hospitalized children in authors' hospital has increased over the years. It has been observed that the annual incidence of thromboembolism has increased after 2014. Thirteen

Entrapped paradoxical thromboembolism.

A 67-year-old obese woman (BMI 38.3) was found at autopsy to have deep venous thrombosis of the left calf with bilateral peripheral and saddle pulmonary thromboemboli and a 165 mm long segment of paradoxical thromboembolus wedged between the right and left atria through a patent foramen ovale. Death was due to acute right-sided decompensation caused by obstruction of the pulmonary outflow tract from bilateral pulmonary thromboemboli. Paradoxical thromboemboli pass from the venous system into the systemic circulation through intracardiac or intrapulmonary shunts causing ischemic injury to the b

[Ultrasound in thromboembolism].

Imaging of pulmonary embolism (PTE) remains a great challange. Mortatity is even high. Possibilities of lung ultrasound, echocardiography, and compression leg vein sonography are discussed. Hemodynamic instable patients should achieve echocardiography at bed side with an accuracy of 90% and immeadiatly treated. In case of a suspected PTE, compression sonography of the leg veins is recommended. It can be performed within 2 min but, on average, yields a positive result in only one-half of the cases of PTE. During the last years, pulmonary sonography took root as a complementary method to MSCT ei

[Thromboembolism in travelers].

The association between long haul travel and the risk of venous thromboembolism are suspected for long time. Mostly air travel related thrombosis series have been reported in the literature. Risk factors can be classified as: 1. travel related factors (coach position, immobilization, prolonged air travel, narrow seat and room, diuretic effect of alcohol, insufficient fluid intake, dehydration, direct pressure on leg veins, rare inspiration). 2. air plane related risk factors (low humidity, relative hypoxia, stress). 3. patient related factors (hereditary and acquired thrombophylia, previous de

Thromboembolism in pregnancy.

Eighteen patients were given carefully controlled intravenous heparin for thromboembolic disease of pregnancy. Thirteen were treated in later gestation and postparium, mainly following warfin therapy. One was treated in early pregnancy and 4 in the postpartum period. Heparin was delivered by constant infusion and dosage regulated by maintaining the thrombin time at 2 to/ times normal. Therapy was stopped for a few hours before and after delivery so that the thrombin time was normal or nearly normal at birth. Two minor episodes of thrombosis recurred in 1 patient while on warfarin, and signific

Asian venous thromboembolism guidelines: updated recommendations for the prevention of venous thromboembolism.

The Asian venous thromboembolism (VTE) prophylaxis guidelines were first published in 2012. Since its first edition, the Asian Venous Thrombosis Forum (AVTF) working group have updated the Asian VTE epidemiology and reviewed issues that were not addressed in the previous guidelines. The authors noted that the rising incidence of VTE across Asia may be attributable to aging population, dietary changes, and increasing incidence of obesity and diabetes. The new additions in the guideline include role of thrombophilia in VTE, bleeding risk in Asians, individual risk assessment, updates in the prev

Thromboembolic Disease.

Venous thromboembolism (VTE) encompasses deep vein thrombosis and pulmonary embolism, both of which can present on a spectrum from subtle symptoms to life- and limb-threatening emergencies. Some risk factors for VTE overlap cardiovascular risk factors and statin therapy can somewhat reduce the VTE risk. When presentations are not life-threatening, clinical prediction scores using the Well's criteria are best used to determine diagnostic testing. The mainstay of VTE treatment is anticoagulant therapy, although life- and limb-threatening presentations can also require thrombolytic therapy.

Thromboembolic disease.

In 52 patients with pulmonary embolism, the deep venous circulation of the lower extremities and the pulmonary arterial blood flow were studied with albumin macroaggregates labeled with technetium-99m. A gamma camera with a data processor was used for this study. At the beginning, the perfusion lung scintigrams were abnormal in all the patients, 42 of them also had abnormal phlebograms and in half of them the perfusion by lung scintigram became normal when the phlebograms were obtained. These findings establish the high incidence of deep thrombophebitis in the legs of the patients with pulmona

Pregnancy-Associated Venous Thromboembolism.

One of the leading causes of maternal death in the United States is venous thromboembolism (VTE), which is influenced by acquired, genetic, and environmental factors. Pregnancy-associated physiological changes that exhibit the characteristics of Virchow's triad: reduced fibrinolysis, increased coagulation factors, trauma, and venous stasis all raise the risk of VTE. Furthermore, multiple gestations, advanced maternal age, cesarean delivery, and obesity are all pregnancy-associated risk factors that also increase the risk of VTE. Management of VTE during pregnancy can be challenging due to the

Hyperglycemia and Venous Thromboembolism.

Patients with diabetes mellitus (DM) have chronically increased blood glucose and multiple physiologic alterations that place them at elevated risk for vascular disease. Traditionally, this vascular risk has mainly referred to chronic atherosclerosis and embolic arterial disease. Retrospective studies have suggested an increased risk of a pulmonary embolism (PE) and deep vein thrombosis (DVT), collectively termed venous thromboembolism (VTE), in patients with DM, but this association has been difficult to demonstrate with comorbidities such as obesity in meta-analysis. Clinical studies have de

[Venous thromboembolism - update 2019].

Venous thromboembolism - update 2019 Abstract. Abstract:The implication of new diagnostic algorithms in our daily clinical routine has simplified the diagnostic certainty, particularly in the case of exclusion of venous thromboembolism (VTE), so that even the general practitioner can already rule out venous thromboembolism with sufficient certainty. The introduction of risk scores in patients with a confirmed diagnosis can help to identify those patients who are at particular risk so you can monitor them. On the other hand, outpatient treatment of patients with pulmonary embolism in the low-ri

[Pregnancy-Related Venous Thromboembolism].

During normal pregnancy, the pro-coagulant activity is increased, while the anticoagulant activity is reduced, resulting in a prothrombotic environment that predisposes toward venous thromboembolism (VTE). VTE occurs frequently in pregnant women and is a leading cause of maternal morbidity and mortality. Hemostatic abnormalities were examined in 14 pregnant women with deep vein thrombosis (DVT). We studied five families with congenital antithrombin (AT) deficiency and two families with congenital protein C (PC) deficiency. One woman with PC deficiency showed protein S (PS) Tokushima. The AT ac

Pregnancy and venous thromboembolism.

PURPOSE OF REVIEW: This review provides a concise and complete overview of diagnostic work-up and treatment of venous thromboembolism in pregnancy, with attention to recent research developments and recent applicable guidelines. This may be useful for all the players of the multidisciplinary interaction needed in this disease management, namely cardiologists and gynecological/obstetric teams. RECENT FINDINGS: Venous thromboembolism is, in the developed world, a major cause of maternal morbidity and mortality during pregnancy or early after delivery, with a reported incidence ranging from 0.49

[Perioperative venous thromboembolism prophylaxis].

The overall thromboembolic risk results from patient-related risk and surgical risk. The latter has been decreasing regularly over the last decades due to the advances in perioperative management. The benefits of perioperative venous thromboembolism prophylaxis have been firmly established. Mechanical prophylaxis should be considered as first-line therapy in patients with bleeding risk. The association with a drug therapy increases the antithrombotic efficiency. However, the effect of mechanical prophylaxis on pulmonary embolism and mortality has not been demonstrated. Assessing renal function

Venous thromboembolism in pregnancy.

The risk of venous thromboembolism is increased during pregnancy. Although the absolute overall risk of deep venous thrombosis (DVT) or pulmonary embolism (PE) in pregnancy is low, clinicians are highly vigilant to the development of this disease in pregnancy because of the severe consequences to both mother and child if this condition is not diagnosed, treated and prevented. Although prompt recognition and diagnosis of DVT or PE is critical to reduce maternal morbidity, diagnosis of both DVT and PE currently relies on data from studies in nonpregnant patients. However, there are some recent s

Venous thromboembolism in pregnancy.

In Western nations, venous thromboembolism (VTE) is an important cause of morbidity and the most common cause of maternal death during pregnancy and the puerperium. Pregnancy is a hypercoagulable state in which coagulation is activated and thrombolysis inhibited. This prothrombotic risk is compounded when hereditary and acquired thrombophilias and other prothrombotic risk factors are present. The risk of venous thrombotic events is increased fivefold during pregnancy and 60-fold in the first 3 months after delivery (postpartum period) compared with nonpregnant women. Many of the signs and symp
