Artificial intelligence (AI) is transforming healthcare and offers new tools in clinical research, personalized medicine, and medical diagnostics. Thyroid function tests represent an important asset for physicians in the diagnosis and monitoring of pathologies. Artificial intelligence tools can clearly assist physicians and specialists in laboratory medicine to optimize test prescription, tests interpretation, decision making, process optimization, and assay design. Our article is reviewing several of these aspects. As thyroid AI models rely on large data sets, which often requires distributed

This study aimed to establish age- and gender-specific reference intervals for thyroid function tests-fT3 (pmol/L), fT4 (pmol/L), and TSH (μIU/L)-in both pediatric and adult populations, and for anti-TPO (kIU/L) only in adults. It also compared partitioned and continuous modeling approaches to determine the most appropriate method for clinical interpretation. In this retrospective observational study, laboratory data from individuals who visited Sivas Cumhuriyet University Hospital between 2017 and 2022 were analyzed. Partitioned reference intervals were determined using the refineR method. Co

Iodine is an essential component of thyroid hormone. Because thyroid hormone synthesis is affected by iodine deficiency on the one hand and by excess iodine intake on the other, thyroid function biomarkers may be useful for assessing iodine status and studying the effects of iodine supplementation. However, reference intervals for some of the most useful thyroid function biomarkers, including serum concentrations of thyroid-stimulating hormone (TSH), free thyroxine (FT4), and thyroglobulin, vary widely due to variability in the commercially available immunoassays for these tests. Recognizing t

Three women are described from a study of patients with postpartum thyroiditis whose sera gave spuriously increased concentrations of free thyroid hormone because of antibody binding of radiolabeled thyroxin (T4) and triiodothyronine (T3) analogs. All of the women showed increased serum concentrations of thyroid autoantibodies. The antibody binding of radiolabeled analogs and its effect on free T4 and free T3 assays disappeared by 48 weeks postpartum. Postpartum women who develop thyroid autoantibodies have approximately 2% prevalence of increased binding of radiolabeled analogs, which can res

Thyroid status was studied in 24 patients above the age of 40 years with Down's syndrome. Three patients had thyroid function tests indicating hypothyroidism. Eight patients had thyroid autoantibodies in serum and 8 patients had a higher than normal level of thyroid stimulating hormone in serum. None of the patients had figures indicating thyrotoxicosis. None of the patients showed any of the clinical signs usually seen in patients with hypothyroidism. It is concluded that biochemical tests indicating hypothyroidism are much more often seen in patients with Down's syndrome than in normal subje

From the foregoing discussion, it is clear that no single test provides sufficient information to justify its use alone as a single screening test. In vitro tests have now replaced in vivo procedures in the vast majority of patients. Because of the frequency of abnormalities in TBP concentration, the estimation of total T4 should be accompanied by a T3 resin uptake to provide the free thyroxine index or alternatively, a normalized T4 test (Quantisorb or ETR) is preferable. In patients with suspected hyperthyroidism, the initial laboratory evaluation should be an estimate of free T4 and a total

In spite of data supporting the use of the serum thyrotropin (TSH) concentration as the best test to detect abnormal thyroid function, measurement of circulating thyroid hormones with or without a serum TSH continues to be frequently requested to evaluate thyroid function. We have analyzed how combinations of thyroid function tests were ordered by referring physicians and the results of the tests in order to offer some suggestions as to how to use thyroid function tests in a cost effective manner. During 1995, 19,181 inpatient and outpatient requests (45,865 different tests) for thyroid functi

Effects of thyroid-stimulating hormone (TSH) and thyrotropin-releasing hormone (TRH) on plasma concentrations of thyroid hormones, and effects of ACTH and dexamethasone on plasma concentrations of cortisol, were studied in adult male ferrets. Thirteen ferrets were randomly assigned to test or control groups of eight and five animals, respectively. Combined (test + control groups) mean basal plasma thyroxine (T4) values were different between the TRH (1.81 +/- 0.41 micrograms/dl, mean +/- SD) and TSH (2.69 +/- 0.87 micrograms/dl) experiments, which were performed 2 months apart. In a sample of hospitalized patients, serum levels of T3, T4 and TSH were measured on the next day after admission. We included 26 patients admitted to Internal Medicine ward (group I) and 27 patients admitted to the intensive care unit (group II), comparing the results with those of 25 normal volunteers (group I). Mean values of T3 and T4 tend to be lower in patients than in healthy subjects, especially in those patients more aggravated, but differences were not statistically significant. In group II there were 5 patients with low T3, 4 with low T4 and two with both alterations. In group III,

Thyroid function tests including thyroxine (T4), T3 test, thyroid-stimulating hormone (TSH), free T4 index and antithyroid antibodies were studied in 51 patients before the start of long-term lithium therapy and after 4 and 12 months on lithium. After 4 months on lithium, changes occurred in all laboratory parameters - T4 and T3 test levels decreased and TSH increased. After 12 months T4 and T3 test increased to pretreatment levels and TSH remained higher than before the start of lithium. During 1 year of lithium treatment only 1 patient developed clinical signs of hypothyroidism and started l

Numerous drugs may cause changes in the serum concentrations of T4 and of T3. If such alterations are not recognized an incorrect diagnosis may result. In moderate degrees of hypo- and hyperthyroidism thyroid hormone levels may be spuriously normal, or the influence of pharmacological substances may lead to false diagnosis of thyroid disease in euthyroid patients. Since prediction of such alterations remains uncertain, it may be necessary to perform additional investigations when a potential artefact is recognized. On the other hand many pharmacological agents, especially those which interact

OBJECTIVE: Thyroid disorders are common. Serum thyroid stimulating hormone is frequently measured and is the single best initial biomarker to diagnose thyroid disease. Automated immunoassays used to evaluate thyroid function are susceptible to interferences that can affect test results and therefore clinical decisions. In this comprehensive review, our aim is to discuss common assay and drug interferences leading to abnormal thyroid function tests.
METHODS: Authors conducted a literature review of PubMed to include studies on drug related and laboratory assay interferences leading to primary

Performing thyroid function tests (TFT) at 2 weeks of age in neonates of mothers with hypothyroidism, despite having a newborn screening program, is a debated approach. We examined whether there is an additional clinical benefit in TFT at 2 weeks of age in neonates born to mothers with hypothyroidism, in addition to the neonatal screening program. We performed a retrospective study which included all newborns of mothers with a diagnosis of hypothyroidism and gave birth in a single regional hospital between the years 2010 and 2016. Data were collected from a computerized medical record system o

Bexarotene is a very rare cause of central hypothyroidism (CH) and its effects have been reported to be dose-dependent; however, the available data in the literature on dose-dependent effects are variable. The standard practice of monitoring thyroid function using thyroid-stimulating hormone (TSH) to adjust levothyroxine (LT4) dose does not apply to bexarotene since it causes CH. In CH, TSH is not reliable. Hence free tetraiodothyronine (fT4) level is used to monitor and adjust the LT4 dose. We report a case of an 81-year-old Caucasian male with cutaneous T-cell lymphoma (CTCL) who was treated

BACKGROUND: A number of abnormalities has been identified among drug addicted users especially heroin addicts. However, there are a few studies to assess the opium effects on thyroid hormones. the aim of the present study is to investigate the effect of opium on the thyroid function tests.
METHOD: In this case-control, 50 male addicts, aged 20-50 years, with history of addiction to opium lasting more than two years, and 50 male non-addicts as control group were randomly selected. 10 cc blood sample was taken for measurements of TSH, total T4 and T3, free T4 and T3, and T3 resin uptake (T3RU)

Subclinical hypothyroidism, subclinical hyperthyroidism and the sick euthyroid syndrome are three conditions that can be effectively assessed using newer tests. Subclinical hypothyroidism is defined as an elevated sensitive thyrotropin level with a normal free thyroxine level. Replacement therapy with levothyroxine is recommended for patients who have a serum thyrotropin level over 20 microU mL (20 mU per L), patients with thyroid antimicrosomal antibodies, or patients with goiter. A diagnosis of subclinical hyperthyroidism is made when a suppressed serum thyrotropin level occurs in associatio

In a series of 4,366 patients screened at Ludhiana, Punjab, the incidence of hypothyroidism was of the order of about 20 per cent and of hyperthyroidism about 15 per cent in both sexes. The female-to-male ratio was a little over 3:1. These statistics conform broadly to conventional Western figures. The relevance of the thyroid function tests in detecting subclinical thyroid disorders in relation to other systems cannot be overemphasised.

To evaluate the effects of salicylates (ASA) on thyroid function tests (TFT's) in patients already taking phenytoin (DPH), 6 adults received a daily dose of DPH to achieve a steady state serum DPH. ASA was then added stepwise (325, 650, 975 mg q 4hr) at 2-day intervals. TFTs, total serum and free salivary DPH were measured. Therapeutic steady state phenytoin levels caused a significant decrease in serum free thyroxine (FT4), total T4, total T3 (TT3) and a significant but slight increase in T3RU. The TSH remained normal. When ASA was co-administered with DPH, the serum free DPH increased, the t
