Thyroid nodules are common. Most are benign lesions since clinically important thyroid carcinoma is a relatively rare disease. The most sensitive and specific test for the diagnosis of thyroid cancer is fine-needle aspiration biopsy, but its diagnostic accuracy depends upon whether or not one excises all suspicious nodules, thus including them as correctly diagnosed. Nevertheless, fine-needle aspiration biopsy is the most sensitive, specific, and cost-effective test for thyroid cancer. Therapy depends upon the cause of the thyroid nodule.

The approach to management of an isolated thyroid nodule requires some understanding of the natural history of thyroid cancer and other forms of nodular thyroid disease. The histologic classification of thyroid cancer is an important determinant of survival, as are the size of the primary tumor, presence of thyroid capsule invasion, and presence of distant metastases. Therapeutic radiation and radioactive fallout increase the risk that a thyroid nodule is malignant. Autonomously functioning thyroid nodules are usually benign follicular adenomas and may cause thyrotoxicosis.

PURPOSE OF REVIEW: To review the current medical evidence in the diagnosis and management of thyroid nodules.
RECENT FINDINGS: The widespread use of imaging modalities in recent years has led to frequent discovery of incidental thyroid nodules. These nodules are mostly benign (over 90%), hence precise insight in evaluating nodules of concern and following up other nodules is important to avoid unnecessary surgeries and its complications.
SUMMARY: Certain nodules, which are symptomatic or suspicious on clinical evaluation, need further evaluation with ultrasonography and cytopathology before

CONTEXT: Approximately 60% of adults harbor 1 or more thyroid nodules. The possibility of cancer is the overriding concern, but only about 5% prove to be malignant. The widespread use of diagnostic imaging and improved access to health care favor the discovery of small, subclinical nodules and small papillary cancers. Overdiagnosis and overtreatment is associated with potentially excessive costs and nonnegligible morbidity for patients.
EVIDENCE ACQUISITION: We conducted a PubMed search for the recent English-language articles dealing with thyroid nodule management.
EVIDENCE SYNTHESIS: The i

In tuberculosis (TB) endemic countries like India, this is a common practice to attribute patient's illness to TB even without microbiological confirmation. Furthermore, a false diagnosis may prove fatal as the underlying disease is left untreated, and the burden is amplified by avoidable potential side effects of antitubercular drugs. We present a case of a 42-year-old female who presented to us with a typical presentation of disseminated TB but found to have metastatic thyroid carcinoma.

In this report we examine the surgery of solitary thyroid nodules, outlining the operations performed and the diagnostic-instrumental procedures aimed at identifying the degree of malignancy of the lesion. If intraoperative cytological and histological examination of a nodule show that it is benign, we perform lobectomy. In a total of 400 cases, we performed 52 (13%) nodule resections, 276 (69%) hemithyroidectomies, and 72 (18%) extended resections comprising the isthmus and Laluette pyramid. Twenty-five patients (5%) were affected by differentiated cancers arising in the nodule. In this group

Thyroid gland size increases during pregnancy due to physiological changes. 2-3% of pregnancies, a thyroid nodule (TN) may either newly develop or an existing one may increase in size. Factors such as age, parity, and iodine status can influence the development of TN. Surveillance of TN in pregnancy is essentially similar to that of the general population as it is contraindicated. Fine needle aspiration cytology (FNAC) can be delayed until after delivery unless malignancy is suspected. Surgery is reserved for severe cases, those with rapid growth, or those with suspicious features. Surgery is

Thyroid nodules are heterogeneous tumors with variable genetic signatures. Thyroid cancers are monoclonal lesions with a defined histomorphology that largely depends on the underlying somatic mutation. While the mutation rate is generally low in differentiated thyroid cancers, poorly differentiated and anaplastic thyroid cancers show a high mutation load. The identification of somatic mutations in fine needle aspirates can be helpful for the differential diagnostics of thyroid nodules; however, a prognostic contribution is less certain. The molecular pathology of thyroid tumors is helpful for

BACKGROUND: Elastography is an imaging method to examine the elasticity of tissue. In the meantime, various elastography methods have been developed, which are subdivided according to the type of stimulus applied. In principle, a distinction should be made between strain elastography (SE) and shear wave elastography (SWE). Both methods provide another means of assessing thyroid disease in addition to conventional B-mode sonography.
OBJECTIVE: The aim is to provide an overview of elastography techniques including physical basics and their importance in the clarification algorithm of thyroid no

Thyroid nodules are common and are commonly benign. The reported prevalence of nodular thyroid disease depends on the population studied and the methods used to detect nodules. Nodule incidence increases with age, and is increased in women, in people with iodine deficiency, and after radiation exposure. Numerous studies suggest a prevalence of 2-6% with palpation, 19-35% with ultrasound, and 8-65% in autopsy data. With widespread use of sensitive imaging in clinical practice, incidental thyroid nodules are being discovered with increasing frequency. Ultrasonography is the most accurate and cos

Thyroid nodules in the elderly are more frequent and more frequently malignant. Fine needle aspiration is the first step to diagnose these type of nodules, and thyroid scans and ultrasounds may be obtained in special cases. Thyroid suppression is frequently not effective in decreasing the size of the nodule and may cause subclinical or clinical thyrotoxicosis. It is not uncommon to discover that what initially was thought to be a solitary nodule is only the dominant nodule of a multinodular goiter.

Thyroid nodules are infrequently encountered in children. During the era of low-dose therapeutic irradiation, the incidence of malignancy in these lesions was 40% to 70%. Recent studies suggest that this incidence is declining, resulting in a concomitant relative increase in the proportion of benign nodular conditions. There is also a heightened awareness that secondary thyroid neoplasms may occur in children surviving primary malignant diseases. Records of 38 children treated surgically at Texas Children's Hospital between 1972 and 1984 have been reviewed to determine the incidence of benign

A retrospective analysis of thyroid scintiscanning at one hospital over a 3-year period revealed nineteen patients in whom a solitary, hyperfunctioning thyroid nodule was producing thyrotoxicosis. Most of the patients had clinical features referable to hyperthyroidism and in the majority a solitary nodule was palpable in the neck which was found to correlate well with the location of the ‘hot’ nodule on scanning. Cardiac failure was present in 21% and atrial fibrillation occurred in 37%. The 131I uptake by the thyroid was of little value in deciding whether or not the patient was toxic, but th

Thyroid nodules are present in up to 30% of the German population. The causative role of iodine deficiency which is still endemic in this country has long been established. Recent progress has shed some light on the pathogenesis of nodular thyroid disease which still remains less well understood than goitrogenesis. Most thyroid nodules appear to be of clonal origin. Functional abnormalities have been related to alterations within the TSH signaling cascade, particularly mutations in the TSH receptor and stimulating G-protein-alpha-subunit. Proliferation which is dissociable from thyroid functio

BACKGROUND We present a rare case of an ectopic thyroid nodule hyperplasia, confirmed postoperatively after excision and histopathological examination of a chronic cervical mass. We discuss the different clinical and therapeutic features of this rare thyroid dysgenesis caused by a defect of migration of the gland along its normal way of descent. CASE REPORT A 48-year-old man with a history of hypertension and asthma presented with dysphagia and a progressively growing firm mass at the anterior part of his neck over the past 6 months. Ultrasound and computed tomography suggested that the mass w

Fine needle aspiration biopsy (FNAB) guided by ultrasonography is routinely used to identify thyroid nodules prior to surgery. Although FNAB has great diagnostic accuracy and safety, it is limited by its relatively low diagnostic accuracy in follicular lesions, such as non-diagnostic or atypia of unclear significance (AUS)/follicular lesion of uncertain significance (FLUS). Additional diagnostic tests are required to overcome these challenges in evaluating thyroid nodules. Thyroid nodules can now be diagnosed with spring-activated single- or double-action needles following the introduction of

In addition to the basic techniques used in radiofrequency ablation of thyroid nodules, a variety of more advanced techniques have been developed. To address hyper-vascular nodules, vascular ablative techniques have been described. Protective hydro-dissection can be used around the carotid artery and the trachea to improve safety. Rescue hydro dissection can be used to reverse nerve injury. Additionally, several new ablation devices show promise at increasing efficacy and safety. These techniques and devices will be reviewed to provide an understanding of these advanced techniques and novel de

OBJECTIVES: The accuracy of a cytological diagnosis obtained by fine needle aspiration is influenced by several factors including the technique used and the experience of both the aspirator as well as the cytologist. In this project we planned to evaluate the interobserver differences of thyroid nodule cytopathology in our medical centre.
METHODS: The study was conducted using retrospective pathology reports from a single academic centre from August 2013 to September 2017. We compared the sensitivity, specificity, negative and positive predictive values, malignancy rates, and accuracy of two
