Pseudoaneurysm of anterior tibial artery following tibial nailing: a case report.

Interlocking nailing is a widely accepted and performed treatment for tibial shaft fractures. The addition of percutaneously placed transfixation screws increases the stabilisation provided by intramedullary nailing; however, the technical complexity associated with the procedure has introduced new potential complications. We report a pseudoaneurysm of the anterior tibial artery caused by a proximal interlocking screw after intramedullary nailing surgery to repair a tibial shaft fracture. The patient experienced complete relief of symptoms following removal of the nail and the screws, excision

Posterior tibial artery pseudoaneurysm following tibial fracture. A case report.

The presence of distal pulses is often times misleading and the severity of the fracture roentgenographically is frequently unrelated to the possibility of a vascular injury. Pseudoaneurysms following long bone injuries, although not as commonly seen in blunt as in penetrating trauma, are potentially limb threatening injuries requiring a high index of suspicion and the liberal use of angiography for early diagnosis and repair. A case of a pseudoaneurysm of the posterior tibial artery associated with a grade I open fracture following blunt trauma and a review of the literature concerning arteri

[A case of anterior tibial artery pseudoaneurysm].

We present a case of traumatic pseudoaneurysm of the anterior tibial artery in a 74-years-old male. The patient had history of a trauma on h … infection suggested ligature of the artery, without vascular reconstruction. Peroneal and posterior tibial arteries were patent. The final postoperative result was satisfactory, absence of significant ischemia of the left lower extremity, and healed operative incision.

Optimal management of tibial arterial trauma.

In an attempt to define optimal management, we have studied the outcome of 29 isolated tibial arterial injuries during the past 4 years. Twenty-five patients suffered blunt and four had penetrating trauma. Twenty-seven patients had preoperative arteriography which showed at least one interrupted tibial artery. In nine patients, immediate and successful reconstruction was done. Since the foot was viable in 20 patients, immediate reconstruction was not carried out in spite of the diagnosis of tibial arterial injury. Three of 20 underwent primary amputation. Fifteen of the remaining 17 patients r

Catheterization of the posterior tibial artery in the neonate.

Catherization of the posterior tibial artery provides an alternative to cannulation of other arteries. We successfully catheterized this artery 17 times in 15 neonates whose weights ranged from 800 to 3,000 g. The catheters were placed percutaneously 13 times and by cutdown four times, and they remained in place for as long as 12 days (mean, 96 hours). Compromise of distal circulation was found in only two babies, both of whom had had umbilical artery catheters removed because of cyanosis of the toes that had resolved itself prior to posterior tibial artery cannulation. Our patients experience

Tibial Artery Stenting in the VQI Database.

OBJECTIVES: The development of dedicated tibial artery stents has been challenging despite reports of successful use of coronary stents in the infrapopliteal circulation. This study compares the outcomes of tibial stenting and plain balloon angioplasty (POBA) prior to FDA approval of a tibial scaffold. METHODS: The Vascular Quality Initiative (VQI) Peripheral Vascular Intervention module was reviewed for patients treated for tibial peripheral arterial disease (PAD) between 2011-2024. Trends in tibial interventions were derived, and patient characteristics undergoing stenting or POBA were comp

Tibial fractures with infrapopliteal arterial injuries.

Tibial fractures with associated infrapopliteal arterial injuries have been inadequately documented in the literature. Eighteen patients with these injuries were admitted to Boston City Hospital during a 10-year period. Three patients required below-knee amputation, and two of these had ischemic intervals of greater than 8 h before vascular treatment. Six patients had delayed diagnoses of arterial injuries, but none required amputation. There were 15 open and three closed tibial fractures. Nine fractures were badly comminuted. Eight of the 14 viable limbs had delayed unions, but only one was a

Transcollateral retrograde approach with rendezvous technique for recanalization of chronically occluded tibial arteries.

PURPOSE: To describe a novel technique using an antegrade wire in a retrograde microcatheter advanced through a transcollateral vessel for recanalization of challenging infrapopliteal chronic total occlusions. TECHNIQUE: A 75-year-old diabetic man presented with critical limb ischemia manifested as nonhealing ulcers on the toes. Baseline angiography revealed a blunt, long, total occlusion of the anterior tibial artery. A retrograde microcatheter was advanced over a guidewire tracking the collateral channel from the planter artery. Antegrade and retrograde microcatheters were aligned inside th

Partial Tibial Nonunion due to Entrapment of Anterior Tibial Artery: A Case Report.

INTRODUCTION: Arterial injury/entrapment is an uncommon sequelae of long bone fractures, particularly in closed injuries. The proximity of the anterior tibial artery to the distal tibia poses a risk of entrapment with a fracture at this level. To the best of our knowledge, this is the first case report of a partial nonunion due to entrapment of the anterior tibial artery in a healed tibia fracture in a pediatric patient. CASE REPORT: We describe a case of a 16-year-old male who presented with leg pain 2 years after a left distal third oblique tibia fracture. The fracture was initially treated

[Entrapment of the anterior tibial artery in a distal tibial fracture after intramedullary nailing].

We present a patient with a closed displaced distal tibia fracture with entrapment of the anterior tibial vessels in the fracture after tibial nailing. This complication was initially not recognised. After several debridements of the forefoot on the same side due to open metatarsal fractures and severe soft tissue injury, a free latissimus dorsi flap was used for covering the dorsum pedis. Preoperative angiography showed occlusion of the anterior tibial artery at the fracture line which was interpreted as a secondary occlusion due to an intima lesion of the vessel after injury. The entrapment

Pseudoaneurysm of the Posterior Tibial Artery following Removal of Tibial Plate- A Rare Case.

INTRODUCTION: Fractures of the distal humerus are a rare entity accounting for approximately 2% of the humeral fractures. Non union of the distal humerus is further rare and poses a major challenge. These fractures usually respond to open reduction and bone grafting but some may fail multiple surgical attempts at union and hence are labelled as "resistant" non union. We report a case of resistant non union of the distal humerus which was managed by total elbow arthroplasty. CASE PRESENTATION: A 49-year-old male presented to the out patient department with history of a compound comminuted frac

Pseudoaneurysm of the anterior tibial artery after interlocking tibial nailing: an unexpected complication.

Anterior tibial pseudoaneurysm is a rare complication after interlocking screw insertion in tibial nailing. We present the case of a 28-year-old male patient with this complication with a 6-week delay after tibial nailing of a right tibial fracture type 42-A1 of the Association for the Study of Internal Fixation (AO/ASIF) classification. On presentation to our emergency department, the patient's complaints were solemnly intermittent pain and occasional swelling of his proximal lower leg. Deep vein thrombosis, compartment syndrome, and implant dislocation were ruled out, and the patient was dis

The objectives of this study were to ch … N: This model demonstrates the apparent inability of these smooth muscle cells from atherosclerotic tibial arteries to relax to pharmacologic and physiologic stimuli. In addition, as seen by transmission electron microscopy, these cells maintain their atherosclerotic phenotype after multiple passages.

Bypasses to plantar arteries and other tibial branches: an extended approach to limb salvage.

During the past 6 years, we have encountered 24 cases in which all major infrapopliteal arteries were occluded as determined by adequate pre … lateral tarsal branch, and four were to unnamed branches of the proximal one third of the posterior tibial arteries (two) or anterior tibial arteries (two). All bypasses were performed with reversed saphenous vein with origins at or distal to the superficial femoral artery. Eight bypasses (four plantar and four unnamed branches) became thrombosed up to 30 months …

Hemodynamic studies before and after extended bypass grafts to the tibial and personeal arteries.

Thirty-ones limbs of 29 consecutive patients undergoing bypass grafts to the tibial or peroneal arteries were studied. Twenty-five limbs had ischemic rest pain with or without localized gangrene, and six had incapacitating claudication. The mean segmental pressure indices were upper thigh, 1.16 +/- 0.36; above knee, 0.71 +/- 0.30; below knee 0.51 +/- 0.23; and ankle, 0.28 +/- 0.21. Indices at the ankle were significantly lower (P less than 0.005) than those obtained from limbs that were candidates for simple femoropopliteal bypass. Three grafts failed immediately (initial patency rate of 90%).
