Faecal incontinence is a common multifactorial condition with a range of invasive treatment options, all of which may be associated with significant complications. Posterior tibial stimulation by continence nurses is an established treatment for urinary incontinence; however, its use in faecal incontinence, while rapidly evolving, is limited to eight small and differing studies. In this article, the background of current management options for faecal incontinence is discussed, as are the physiology and evidence underlying neuromodulation. The evidence base for posterior tibial nerve stimulatio

Ankle arthroscopy provides a minimally invasive approach to the diagnosis and treatment of certain ankle disorders. Neurological complications resulting from ankle arthroscopy have been well documented in orthopaedic and podiatric literature. Owing to the superficial location of the ankle joint and the abundance of overlying periarticular neurovascular structures, complications reported in ankle arthroscopy are greater than those reported for other joints. In particular, all reported neurovascular injuries following ankle arthroscopy have been the direct result of distractor pin or portal plac

Localized hypertrophic neuropathy (LHN) is a disorder of unknown cause that presents with progressive, predominantly motor weakness in the distribution of a single nerve, with the histological appearance of the "onion bulb" formation. Because of its rarity, the condition is often ignored in the differential diagnosis of mononeuropathy. We report a case of LHN involving the posterior tibial nerve and review the literature.

AIM/INTRODUCTION: Percutaneous posterior tibial nerve stimulation is an office-based treatment for urinary urgency with and without incontinence. However, given the need for ongoing therapy for patients with urgency incontinence, devices that allow for at-home stimulation of the posterior tibial nerve have been developed. Implantable tibial neuromodulation (iTNM) devices offer a permanent solution to in-office percutaneous stimulation. Currently, a subfascial iTNM system is approved for men and women with urinary urgency with and without incontinence. Given the recent approval of this device,

Combined compression of both the common peroneal nerve and the proximal tibial nerve at the level of the popliteal fossa is rare. Recently, an anatomic site of compression of the proximal tibial nerve at the soleal sling (originating arch for the soleus muscle) has been described in cadavers. The present report includes three patients who had a combined compression of the common peroneal nerve at the fibular neck (fibular tunnel syndrome) and compression of the proximal tibial nerve at the soleal sling (soleal sling syndrome). In each case, blunt trauma was the precipitating event. Neurolysis

The results following revision tarsal tunnel release in 12 patients (13 feet), including three men and nine women aged 28 to 66 years, are presented. The indication for surgery was incapacitating focal pain, associated with paresthesias and hyperesthesias, refractory to nonoperative treatment modalities. Electrodiagnostic studies were abnormal in nine and normal in four cases. Revision surgery was performed a mean 3.5 years (range 1-10 years) after the initial tarsal tunnel release. Epineurolysis was performed in nine of the 13 cases where the nerve was encased in a scar. An insufficient previ

BACKGROUND: Nerve reconstruction following lower-extremity nerve injuries usually leads to worse outcomes in comparison with upper-extremity injuries due to the long distances of nerve regeneration. This study was performed to consider the clinical application of distal nerve transfer for the treatment of long gaps of the tibial nerve (TN) and in established compartment syndrome. It aimed to determine the anatomic suitability of transferring the sural nerve (SN) in combination with the superficial peroneal nerve (SPN) to the TN at the level of the tarsal tunnel for restoration of plantar sensa

Segmental peripheral nerve injuries, particularly those involving long nerve gaps, pose a significant challenge in reconstructive surgery. Conventional strategies, such as nerve autografts or processed allografts, are often limited by inadequate length or poor regenerative outcomes, especially in traumatized wound beds. Nerve flaps offer the theoretical advantage of enhanced axonal regeneration through improved perfusion and support of Schwann cell viability but are rarely used due to technical complexity and limited donor options. We present a unique case of a free sural nerve flap used to re

BACKGROUND: Partial tibial nerve transfer to the motor branches of tibialis anterior is an emerging reconstructive technique for the treatment of traumatic common peroneal nerve (CPN) injury; however, few papers in the literature describe clinical outcomes.
METHODS: A prospective single-surgeon series of nine consecutive patients who underwent partial tibial nerve transfers to the motor branches of tibialis anterior for traumatic CPN injuries between 2008 and 2014. Eight patients were male and the average age at operation was 28.2 years old (range 21-39). All nine patients experienced high-en

The innervation of the vasculature of the dog hindpaw separately controls the series and parallel coupled vessels by means of the tibial, deep fibular, and superficial fibular nerves. The latter primarily affects veins. The venous effects of the tibial and deep fibular nerves have not been adequately defined. The right hindpaw of anesthetized dogs was vascularly and neurally isolated in a volume recorder. The animals were heparinized and the preparation autoperfused (constant pressure). Total tissue volume changes were determined by the volume recorder. Total vascular volume changes were calcu

Premature birth induces a profound change in the environmental factors affecting nerve maturation. The proprioceptive sensory and motor nerve conduction velocities (NCV) of the posterior tibial nerve, which reflect peripheral nerve maturation, have been measured in 3 groups of newborns. Two groups of premature (PT) babies, studied when they reached the expected date of birth (group I, gestational age (GA) at birth, 28-31 weeks, n = 8; group II, GA at birth, 32-35 weeks, n = 6) were compared to 9 normal full-term (FT) newborns. As previously shown, the motor NCV of PT babies at a post-conceptio

Standard treatment for a neuroma-in-continuity with partial retained function is neurolysis with or without grafting. The present study tests the outcome of a novel partial nerve lesion bypassed with an end-to-side bridge graft, intended to increase the number of axons crossing the defect while not disturbing intact axons. An 8-mm portion of tibial nerve was resected in 20 rats. Three weeks later, half had the defect repaired with an end-to-side bridge allograft and perineurial windows; controls had only neurolysis. Recovery was evaluated using walking-track analysis, allodynia testing, muscle

A partial nerve lesion and associated neuroma can be either left alone or repaired with a graft. A by-pass graft around the undisturbed lesion with end-to-side attachments might be a good alternative. This study in rats examines these strategies using walking-track analysis, muscle weights, and histology. After a tibial nerve partial lesion (3 mm) and a 21-day delay, the reexposed lesion was either not repaired, repaired with an interposed allograft, or a by-pass allograft. Functional results showed that all three groups had a steady improvement over the 8-week period, but without significant

Overactive bladder syndrome (OAB) is a prevalent chronic condition that greatly affects patient quality of life. Transcutaneous tibial nerve stimulation (TTNS) has recently been proven to be a safe and effective noninvasive treatment modality for OAB. However, existing electrical stimulators are relatively large, relying on adhesive hydrogel electrodes, and may require professional assistance, which could hinder patient adherence and treatment consistency. To address these, a compact and wearable electrical stimulator (TENSmini) with integrated textile electrodes was developed. Although labora

BACKGROUND: Extramedullary plasmacytomas (EMPs) are rare plasma cell tumors forming outside the bone marrow. EMPs spread to soft tissues with a predilection for the head and neck region. Therefore, EMPs are rarely included in the differential for peripheral nerve sheath tumors (PNSTs). The authors discuss a case of tibial nerve sheath invasion by an EMP in the context of multiple myeloma (MM).
OBSERVATIONS: A 70-year-old female with MM presented with right leg pain and a suspected PNST of the tibial nerve. Imaging revealed a T1-hypointense lesion in the popliteal fossa. Resection was attempte

Purpose: To explore whether stimulation of C-fibers in tibial nerves can induce bladder inhibition by optogenetic transdermal illumination. Methods: Ten rats were injected with AAV2/6-hSyn-ChR2(H134R)-EYFP into the tibial nerves. Transurethral cystometry was performed 4 weeks after the virus injection. Illumination (473-nm blue light at 100 mW) was performed with the fiber positioned above the right hind paw near the ankle. The light transmission efficiency was examined with a laser power meter. The effects on cystometry were compared before and after illumination with the bladder infused with

Diagnosis of tibial neuropathy has been traditionally based on clinical examination and electrodiagnostic studies; however, cross-sectional imaging modalities have been used to increase the diagnostic accuracy and provide anatomic mapping of the abnormalities. In this context, magnetic resonance neurography (MRN) offers high-resolution imaging of the tibial nerve (TN), its branches and the adjacent soft tissues, and provides an objective assessment of the neuromuscular anatomy, abnormality, and the surrounding pathology. This review describes the pathologies affecting the TN and illustrates th
