BACKGROUND: Patients with syncope of unknown etiology after negative noninvasive and electrophysiological testing may suffer from recurrent disability. Syncopal episodes are often too infrequent and unpredictable for detection by conventional ambulatory monitoring techniques.
METHODS AND RESULTS: A long-term subcutaneous monitoring device was implanted in patients with negative ambulatory monitoring, tilt table and electrophysiological testing to establish cardiac rhythm during spontaneous syncope. Sixteen patients aged 57 +/- 19 years with a mean of 8.4 +/- 4.4 previous episodes of syncope u

Head-up tilt testing has proven effective in identifying individuals prone to vasodepressor syncope (VDS). VDS refers to the transient loss of consciousness/cerebral anoxia seen with hypotension produced by autonomic imbalance. In this case, the hypotension is the result of parasympathetic domination. Most episodes appear to be triggered by reduced venous return which stimulates the cardiac mechanoreceptors in the inferior-posterior left ventricle. Once activated, these receptors send out afferent signals along the unmyelinated C of the vagus nerve and cause vasodilation. Once venous return is

Forty patients with recurrent syncopal episodes of uncertain etiology were evaluated with a 60-degree head-up tilt table test for 60 minutes. There were 21 men and 19 women, with a mean age of 36 +/- 19 years, a mean of 7.6 syncopal episodes per patient, and a mean duration of symptoms of 4.2 +/- 6 years. Twelve patients had evidence of organic heart disease. Ten control subjects with no history of syncope were studied similarly. Syncope did not occur in any of these controls. Syncope occurred in 15 patients (37.5%) after a mean standing time of 42 +/- 12 minutes. Syncope was due to a typical

OBJECTIVES: This study was conducted to develop a time-efficient tilt table test.
BACKGROUND: Current protocols of tilt table testing are quite time-consuming. This study was designed to assess the diagnostic value, tolerance and procedural time of a single-stage isoproterenol tilt table protocol.
METHODS: A single-stage isoproterenol tilt table test was compared with the passive tilt table test. The study was prospectively designed in a randomized and crossover fashion.
RESULTS: The study population consisted of 111 patients with a history of syncope (mean age 55 +/- 20 years). Of the tota

To study the outcomes of a series of consecutive tilt table tests combined with video-EEG (TTVE) at a single center, and assess their cost-effectiveness compared with other neurophysiological tests. We retrospectively reviewed medical records of patients who underwent TTVE studies between March 1st, 2013 to April 1st, 2018. Detailed clinical history, including patient demographics, reasons for referral, anti-seizure medications, and neurophysiological studies obtained prior to the TTVE studies were extracted from chart reviews. The fee for each neurophysiological test was identified from the C

BACKGROUND: Motor activity occurring during neurocardiogenic syncope can mimic true neurologic events.
OBJECTIVE: To assess the prevalence and type of apparent neurologic events associated with tilt table testing.
METHODS: The records of consecutive patients undergoing tilt table testing for the evaluation of syncope were reviewed. Patients underwent a 70 degrees upright tilt for 40 minutes, followed by a 20-minute tilt while receiving isoproterenol hydrochloride. The results of tilt table tests were considered positive when clinical symptoms were reproduced in association with a decline in

UNLABELLED: The usefulness of the head-up tilt testing (HUT) has been previously addressed in diagnosing vasovagal neuroregulatory syncope in the teenage population. However, data concerning sensitivity and specificity is deficient due to the lack of control groups. We compared the response to HUT in young patients referred because of syncope or near syncope (n = 44, mean age 16 +/- 3 years SD) to healthy young volunteers with a normal physical examination and no previous history of syncope (n = 18, mean age 16 +/- 2 years) and to determine the sensitivity and specificity of HUT. Head-up tilt table testing was performed in 50 patients for evaluation of syncope. A positive test occurred in 29 patients (58 percent). Of these 29 patients, 15 were categorized as vasovagal; 11 as orthostatic hypotension; and three as chronotropic incompetence. Isoproterenol facilitated the occurrence of a positive test in 13 patients. Medications were used to treat 15 of the 29 patients having a positive test result. Syncope recurred in two patients over a 9 +/- 5 month follow-up period. In patients having unexplained syncope, head-up tilt table testing may provide a diagnosis and can be us

Background Syncope is a common clinical condition with diverse underlying mechanisms, making its evaluation challenging. Tilt-table testing is widely used to assess autonomic and cardiovascular responses; however, the role of electrocardiographic (ECG) parameters in differentiating syncope types remains unclear. Methodology This retrospective study analyzed patients who underwent tilt-table testing for syncope evaluation in 2023-2024. A total of 59 patients were included and divided into three groups: orthostatic hypotension (n = 20), vasovagal syncope (n = 19), and negative test results (n =

BACKGROUND: The diagnostic test for vasovagal syncope (VVS), the most common cause of syncope is head-up tilt test (HUTT) assessment. During the test, subjects experienced clinical symptoms such as nausea, sweating, pallor, the feeling of palpitations, being on the verge of passing out, and fainting. The study's goal is to develop an algorithm to classify VVS patients based on physiological signals blood pressure (BP) and electrocardiography (ECG) obtained from the HUTT.
METHODS: After 10 min of supine rest, the subject was tilted at a 70-degree angle on a tilt table for approximately a total

The value of the tilt test in assessing unexplained syncope in children and adolescents was investigated in 79 patients (41 girls and 38 boys, 12.3 +/- 2.9 years). The test was performed with a tilt of 60 degrees for 45 minutes. A pharmacological provocation with isoprenaline was carried out in negative passive test. The tilt test was considered to be positive in cases of symptomatic falls of the systolic blood pressure with or without bradycardia. Asystole was defined as a ventricular pause > or = 5 seconds. During the investigation, 52 patients (66%) reproduced symptoms related to vasovagal

BACKGROUND: Vasovagal syncope is triggered by complex cerebral and cardiovascular reflex mechanisms leading to general vasodilatation and bradycardia resulting in cerebral hypoperfusion.
METHODS: We prospectively studied the correlation of postural blood pressure test (PBT) and head-up tilt table test (TT) in patients with recurrent vasovagal syncope. Sixty-eight patients (37 male, 52.4 +/- 18.6 years) were included in the trial. They were asked to participate in a PBT and a TT. Independent samples t-test, ANOVA and Fisher's exact test were used for statistical analysis. p values <0.05 were c

OBJECTIVE: To elucidate the usefulness of clinical orthostatic blood pressure testing (COBP) as a screening tool for autonomic dysfunction.
STUDY DESIGN: In this retrospective case review, the records of 156 consecutive patients with nonotologic dizziness as the primary complaint seen in an academic neurotology clinic between 2005 and 2009 were reviewed. The objective of this study was accomplished by comparing the diagnostic yield of COBP with that of head-upright tilt table testing (HUT) and assessing the sensitivity and specificity of COBP in predicting an abnormal HUT in patients with non

Broad complex tachycardia (BCT) during head up tilt test (HUTT) is infrequent. Electrophysiology Study (EPS) plays an important part in further differentiation of BCT. We present a case of BCT during HUTT in a patient presenting with presyncope which later on EPS with 3D mapping was diagnosed as ventricular tachycardia. This case highlights the unusual occurrence of BCT during HUTT, the differential diagnosis of BCT and the utility of EPS to reliably identify the type and origin of BCT.

BACKGROUND: Syncope is a common medical condition. The reflex or neurally mediated syncope (NMS) is the most frequent type. The tilt table test (TTT) helps distinguish syncope from other common causes of complete loss of consciousness, such as epilepsy, define syncope subtypes and guide management. This study aimed to assess the TTT yield in patients with suspected NMS and to compare the nitroglycerin (NTG) and isoproterenol (Isuprel) provocative protocols.
METHODS: This study was a retrospective analysis of the data of 426 consecutive patients who underwent TTT at the Heart Center at King Fa

BACKGROUND: Syncope due to carotid sinus hypersensitivity (CSH) may be underdiagnosed due to a low level of diagnostic suspicion and fear of complications caused by massage of the carotid sinus.
OBJECTIVE: To investigate the relevance of carotid sinus massage in the diagnosis of non-convulsive faints and unexplained falls.
METHODS: Two-hundred and fifty-nine patients with a mean age of 50+/-24 years referred for the investigation of non-convulsive faints or unexplained falls were evaluated with a head up tilt table test (HUTT) without ("passive") and with pharmacological stimulation. Clinica

BACKGROUND: Autonomic imbalance is the proposed mechanism of syncope during a tilt table test (TTT). We have recently demonstrated that skin sympathetic nerve activity (SKNA) can be noninvasively recorded using electrocardiographic electrodes.
OBJECTIVE: The purpose of this study was to test the hypothesis that increased SKNA activation precedes tilt-induced syncope.
METHODS: We studied 50 patients with a history of neurocardiogenic syncope undergoing a TTT. The recorded signals were band-pass filtered at 500-1000 Hz to analyze nerve activity.
RESULTS: The average SKNA (aSKNA) value at base
